РЕЗЮМЕ. Умови сучасного суспільства вимагають створення чіткого діагностичного алгоритму, який дозволить швидко та точно виявляти жінок високих груп ризику виникнення та прогресування гормональних порушень на тлі автоімунного тиреоїдиту, які будуть впливати на несприятливий репродуктивний прогноз. Метою було розробити математичну модель прогнозування гормональних порушень у жінок репродуктивного віку на тлі автоімунного тиреоїдиту, які сприяють негативному репродуктивному прогнозу. Матеріал і методи. Нами обстежено 147 жінок репродуктивного віку (середній вік (33±1,87) р.) з автоімунним тиреоїдитом. Усім пацієнтам було проведено комплексне клініко-лабораторне обстеження. Для побудови прогностичної моделі гормональних порушень у жінок з автоімунним тиреоїдитом використовували множинний регресійний аналіз. Для статистичної обробки використовували статистичний пакет Statistica 10.0 і табличний редактор Microsoft Excel 2007. Результати. Було виділено 11 достовірних факторів, до яких увійшли: вік, ТТГ, АТПО, АТТГ, 25(OH)D, АМГ, ЛПНЩ, ЛПВЩ, статус куріння, тривожний і депресивний синдроми, для прогнозування гормональних порушень при автоімунному тиреоїдиті, які сприяють несприятливому репродуктивному прогнозу у молодих жінок, та побудовано математичну модель, гістограму залишкових відхилень та діаграму розсіювання залишків. Висновки. Розроблена модель прогнозування гормональних порушень при автоімунному тиреоїдиті, які призводять до несприятливого репродуктивного прогнозу, є якісною та інформативною. Використання даної моделі у клінічній практиці дозволить вчасно виділити групу жінок із високим ризиком прогресування гормональних порушень на тлі захворювання, які негативно впливають на репродуктивну здатність.
РЕЗЮМЕ. Останніми роками в Україні та інших країнах світу спостерігається збільшення частоти тиреопатій автоімунного генезу. Патофізіологічне розуміння особливостей автоімунних порушень при автоімунному тиреоїдиті даcть змогу обґрунтувати необхідність призначенння тих чи інших препаратів. Мета – визначити рівень ФНПα, ІЛ-6, ІЛ-10, ІЛ-17, ІЛ-23 у пацієнтів із автоімунним тиреоїдитом, а також встановити взаємозв’язок між рівнем інтерлейкінів та функцією щитоподібної залози при автоімунному тиреоїдиті. Матеріал і методи. Нами обстежено 98 пацієнток віком 18–43 роки із автоімунним тиреоїдитом та 30 жінок контрольної групи. Функціональний стан щитоподібної залози досліджували за допомогою визначення рівнів тиреотропного гормону, тироксину вільного, трийодтироніну вільного у сироватці крові, визначали рівень антитіл до тиреопероксидази та тиреоглобуліну. Стан системного та місцевого запального процесу оцінювали за параметрами ФНП-α, ІЛ-6, та ІЛ-10, ІЛ-17, ІЛ-23. Результати та висновки. У всіх хворих з автоімунним тиреоїдитом було виявлено зміни у цитокіновому статусі, при цьому були деякі відмінності цитокінового статусу залежно від клінічного варіанта автоімунного захворювання щитоподібної залози. Виявлено позитивний кореляційний зв’язок між рівнем ФНП-α та рівнем АТПО (r=0,62, p<0,05), між рівнем ІЛ-6 та рівнем АТПО ( r=0,44, p<0,05), рівнем ІЛ-10 та АТПО (r=0,36, p<0,05), рівнем ІЛ-17 та рівнем АТПО (r=0,42, p<0,05), рівнем ІЛ-23 та рівнем АТПО (r=0,42, p<0,05). Досліджено позитивний кореляційний зв’язок між рівнем ФНП-α та рівнем ТТГ (r=0,58, p<0,05), між рівнем ІЛ-6 та рівнем ТТГ (r=0,41, p<0,05), рівнем ІЛ-10 та ТТГ (r=0,32, p<0,05), рівнем ІЛ-17 та рівнем ТТГ (r=0,37, p<0,05), рівнем ІЛ-23 та рівнем ТТГ (r=0,39, p<0,05).
