Резюме Актуальность. Церебральные артериовенозные мальформации (АВМ) не являются статичными врожденными поражениями, а могут расти, рецидивировать и появляться de novo после полной резекции, эмболизации или радиохирургии. Понимание сложной молекулярной биологии АВМ имеет решающее значение для прогнозирования их поведения в процессе лечения и улучшения его результатов. Цель. Изучить динамику факторов ангиогенеза в процессе эмболизации церебральных АВМ для разработки стратегии их персонализированного лечения. Материалы и методы. В исследование включены 314 пациентов с АВМ, получавших хирургическое лечение в отделении хирургии сосудов мозга РНХИ им. проф. А. Л. Поленова. Определяли уровень сосудистого эндотелиального фактора роста (VEGF), ангиопоэтина-2 (ANG-2) и матриксной металлопротеиназы-9 (ММP-9) в сыворотке крови до и через 24 часа после эндоваскулярной эмболизации с использованием иммуноферментного анализа (Personal Lab, Adaltis, Italy). Результаты. Первичные пациенты с АВМ (48,4 %) продемонстрировали повышение VEGF, MMP-9, ANG-2. Высокий уровень VEGF и MMP-9 демонстрировали АВМ III градации по Spetzler-Martin, АВМ с геморрагическим типом течения, с глубоким характером дренирования, с афферентами из наружной сонной артерии. Возвращение к контрольным значениям всех повышенных факторов роста после тотальной эмболизации является подтверждением отсутствия потенции к рецидивированию АВМ. Отсутствие снижения факторов ангиогенеза после радикальной, по ангиографическим критериям, эмболизации является признаком субтотального выключения АВМ. Заключение. Сформулирована персонифицированная концепция эмболизации у пациентов с высоким риском роста и рецидивирования.

В обзоре даны понятия метаболомики и метаболома, представлены методы исследования и интерпретации данных, рассмотрены основные применения метаболомики в области диагностики заболеваний ЦНС, поиска прогностических маркеров и новых терапевтических мишеней, описан метаболический состав спинномозговой жидкости в норме и его изменения при патологии.
