Известно, что нарушения в функционировании дофаминергической и норадренергической систем головного мозга вовлечены в механизмы развития шизофрении. Важным ферментом, участвующим в обмене дофамина и норадреналина, является дофамин-β-гидроксилаза. Установленное изменение активности данного фермента у больных шизофренией может быть обусловлено генетическими особенностями. Цель исследования: изучение ассоциации полиморфизма гена дофамин-β-гидроксилазы rs1611115 (C-1021T) с выраженностью симптомов шизофрении у больных с первым эпизодом шизофрении на фоне терапии галоперидолом и рисперидоном. Материал и методы. Всего в исследование было включено 212 пациентов основной группы, из них 110 мужчин и 112 женщин. Всем пациентам в соответствии с МКБ-10 был выставлен диагноз: «Шизофрения параноидная, период наблюдения менее года, F20.09». Исследовательская выборка были разделена на две клинические группы: 1-я группа (n=105) ‒ больные принимали галоперидол, 2-я группа (n=107) – больным назначали рисперидон. Группу контроля составили 152 человека. Геномную ДНК человека выделяли из лейкоцитов цельной крови. Анализ SNPDBHC-1021Trs1611115 проводили методом полимеразно-цепной реакции. Результаты. У пациентов с шизофренией в 1,13 раза чаще присутствовали носители референсного аллеля CSNPDBHrs1611115 (C-1021T), у которых риск развития заболевания был увеличенным в 1,8 раза. Было установлено, что у носителей генотипов С/Т+Т/Т SNPrs1611115 (C-1021T) позитивные симптомы шизофрении были более выраженными, чем у носителей генотипа С/С. У носителей этих генотипов при терапии галоперидолом и рисперидоном в более короткие сроки редуцировались позитивные и общие симптомы, а при использовании галоперидола – негативные. Заключение. Полученные нами данные демонстрируют возможную роль генетических дефектов дофамин-β-гидроксилазы в механизмах развития шизофрении.При этом выраженность клинических симптомов у больных с первым эпизодом шизофрении до начала лечения и их динамика в процессе терапии антипсихотиками также ассоциированы с SNPдофамин-β-гидроксилазы rs1611115 (C-1021T).
