Рассеянный склероз (РС) является хроническим воспалительным демиелинизирующим за-болеванием ЦНС, которое ввиду своей распространенности (50 случаев на 100 тыс. населения) и преимущественным поражением лиц трудоспособного возраста имеет важное медикосоциальное значение. На сегодняшний день РС представляется мультифакториальным заболеванием, ведущим патогенетическим механизмом которого, лежащим в основе воспаления, является аутоиммунный процесс. Многочисленные доказательства аутоиммунной природы РС получены в серии экспериментальных и клинических работ.
Резюме. Рассеянный склероз -хроническое демиелинизирующее заболевание центральной нервной системы с гетерогенными проявлениями, характеризующееся аутоиммунным воспалением и прогрессированием нейродегенерации. Одной из особенностей аутоиммунного воспаления при рассеянном склерозе является полиспецифический интратекальный гуморальный иммунный ответ против нейротропных вирусов (кори, краснухи и ветряной оспы) -MRZ-реакция. В ее основе лежит поликлональная активация зрелых В-лимфоцитов в центральной нервной системе с интратекальным синтезом антител к анамнестическим антигенам, что не связано с репликацией вирусов в центральной нервной системе и синтезом антител в крови. Иммуноглобулины, продуцируемые против нейротропных вирусов, являются составной частью олигоклонального пула антител в цереброспинальной жидкости. Поскольку иммуноглобулины способны проникать через гематоэнцефалический барьер, для определения их интратекального синтеза рассчитывают индексы, учитывающие не только концентрации антител в сыворотке крови и цереброспинальной жидкости, но и проницаемость гематоэнцефалического барьера для данных антител [К (альбумин), К (IgG)]. Цель исследования состояла в оценке информативности определения показателей интратекального синтеза антител против нейротропных вирусов (MRZ-реакции) при рассеянном склерозе. В исследование включены 60 пациентов: первая группа -35 больных рассеянным склерозом, вторая -25 пациентов с различными воспалительными и невоспалительными заболеваниями центральной нервной системы. Neuromyelitis optica spectrum disorder (NMOSD) is an aggregate of inflammatory and autoimmune disorders of the central nervous system characterized by recurrent, disabling clinical course and damages predominantly targeting optic nerves, brain stem and spinal cord. NMOSD is stratified into two types: seropositive for aquaporin-4 antibodies (AQP4-IgG) and seronegative, which is reported in 25 % of cases. This article presents modern conceptualizations of NMOSD and describes authors’ own experience of clinical observation of patients.
