Эссенциальный тремор (ЭТ) -наиболее распространенное экстрапирамидное рас-стройство, преимущественно характеризующееся по-стурально-кинетическим тремором рук и других частей тела. Прогрессирование заболевания нарушает социаль-ную активность, снижает самооценку, а иногда и при-водит к инвалидизации пациента. Цель работы. По-скольку подбор эффективной медикаментозной терапии ЭТ в большинстве случаев проводится эмпирически, за-нимает длительное время, вызывая дополнительные не-гативные психоэмоциональные последствия, недоверие к лечащему врачу и отказ от лечения, для повышения эффективности терапии нами предложен подход к вы-делению подтипов ЭТ на основе гетерогенности фарма-кологического ответа. Результаты. В работе обобще-ны данные литературы, на основе которых возможно по нейроанатомическим, электрофизиологическим и био-химическим показателям выделить подтипы ЭТ с детер-минированным фармакологическим ответом. Показано, что перед назначением фармакотерапии пациентам с ЭТ необходимо проводить поверхностную электромиогра-фию для выявления типа паттерна сокращений мышц-антагонистов. Кроме этого, целесообразно определение содержания глутамата и гамма-аминомасляной кислоты в периферической крови. Заключение. Гетерогенность фармакологического ответа среди пациентов с ЭТ обу-словливает необходимость более тонкой классификации внутри синдрома по нейроанатомическим, электрофизио-логическим и биохимическим показателям. Предлагае-мый в обзоре подход позволит повысить эффективность терапии и улучшить качество жизни пациентов с ЭТ.
The aim of the study is the modern scientific literature estimation in the field of the investigation of neurodegenerative and metabolic disorders mediated by the trace amines and their receptors. The analysis of modern ideas about the feedback of neurodegenerative and metabolic diseases in which the trace amines and their receptors are involved was carried out. The important role of trace amines and their receptors in the regulation of the dopamine system, in connection with metabolic and neurodegenerative diseases, including Parkinsons disease, ADHD, schizophrenia, obesity, metabolic syndrome and other pathological conditions, has been shown. Trace amines and their receptors have a direct effect on dopamine systems, being regulators of various metabolic and neurodegenerative processes, participating in energy metabolism, neurogenesis, and other vital processes.
This study aimed to identify clinical and electrophysiological signs that determine the phenotypic heterogeneity of essential tremor using a cluster analysis. Seventy-five patients with essential tremor were examined. To identify the heterogeneity of the disease, a cluster analysis was applied (k-means method). Clustering was performed according to clinical and electromyographic parameters. The heterogeneity of essential tremor is justified by amplitude characteristics, topography of tremor, duration disease, and severity of impaired of daily activity. Detecting heterogeneity in essential tremor can broaden understanding of pathogenesis disease and justify heterogeneity of the pharmacological response in patients with essential tremor.
