Multiple mutations in RET proto-oncogene are not common findings in patients with multiple endocrine neoplasia type 2A syndrome (MEN2A). Screening for RET mutation in MEN2A family members is usually limited by the known affected exon. However the second unrevealed mutation in RET proto-oncogene can coexist and modify the phenotype of MEN2 patients, including age of onset of medullary thyroid carcinoma, penetrance of pheochromocytoma etc. We here describe a family with MEN 2A syndrome with combination of three different germ-line RET mutations in its members (RET codon C634R, C634R+I852M, I852M+Y791F, Y791F). The earliest onset of medullary thyroid carcinoma was in a patient harboring the C634R+I852M double mutation at age 24 years. A 49-y.o.patient with C634R mutation has persistent medullary thyroid carcinoma after thyroidectomy at 35 years old. A carrier of Y791F mutation had no clinical evidence of disease at age of 28 years. In a child with compound I852M+Y791F mutation preventive thyroidectomy revealed C-cell hyperplasia at age of 4 years. The clinical significance of double RET mutation in the described family is not clear. Literature data of multiple germ-line RET mutations in patients with multiple endocrine neoplasia type 2A syndrome are presented.
Генетически обусловленный медуллярный рак щитовидной железы (МРЩЖ) относится к злокачественным опухолям с плохим прогнозом, и это делает необходимым разработку как новых диагностических методик доклинического выяв-ления заболевания, так и внедрение в лечебную практику эффективных способов, блокирующих развитие опухоли. В статье представлен первый в России обобщенный опыт профилактических тиреоидэктомий (ТЭ), выполненных в период с 1998 по 2015 г. у 10 носителей герминальных мутаций в гене RET из семей с синдромом множественных эндо-кринных неоплазий 2А типа (МЭН 2А) и семейным МРЩЖ. Цель: оценить результаты хирургического лечения носителей герминальных мутаций в гене RET. Материал и методы. В период с 1998 по 2015 г. в двух центрах -НИИ ДОГ РОНЦ им. Н.Н. Блохина и МРНЦ им. А.Ф. Цыба -было проведено профилактическое хирургическое лечение 10 пациентов -бессимптомных носителей герминальных мутаций в гене RET. Возраст пациентов -от 2 до 23 лет. Из всех пациентов 9 были из семей с синдромом МЭН 2А, один -с семейным МРЩЖ. Во всех случаях хирургическое лечение проведено в объеме ТЭ, двум пациентам дополнительно выполнена лимфодиссекция VI уровня. Период наблюдения после операции составил от 6 мес до 16 лет. Результаты. ДНК-диагностика выявила у 8 пациентов мутацию в экзоне 11, в одном случае -в экзоне 10 и у одного паци-ента были выявлены две мутации в экзонах 13 и 14. Возраст пациентов варьировал от 2 до 23 лет. Уровень базального кальцитонина был повышен у 7 из 10 пациентов. Проведенная профилактическая ТЭ у двух пациентов была дополнена селективной лимфодиссекцией. Гистологическое исследование удаленной щитовидной железы (ЩЖ) обнаружило очаги медуллярного рака у 6 из 10 пациентов, у двоих выявлена С-клеточная гиперплазия и у двоих обнаружены признаки выраженного и слабо выраженного склероза в ткани ЩЖ. Самый ранний возраст выявления МРЩЖ составил 3 года -у ребенка -носителя мутации в кодоне 634, из семьи, где у родственников наблюдалось агрессивное течение МРЩЖ. В ходе динамического наблюдения рецидив выявлен у одного больного, ребенка 15 лет, носителя аналогичной мутации. Заключение. При выявлении мутации в гене RET и риске развития МРЩЖ профилактическая ТЭ должна быть проведе-на как можно раньше, что подтверждается высокой частотой выявления очагов рака у пациентов в проведенном иссле-довании (6 случаев манифестации заболевания из 10 пациентов). Предмет исследования: исследования проводились в области производства ячеистого армированного бетона автоклавного твердения (газобетона). В качестве исходных данных представлены совместные экспериментальные исследования по подбору армирующего материала предприятий, выпускающих газобетон (г. Электросталь) и композитную арматуру (г. Псков). Цель: задача исследования сводится к подбору альтернативного металлической арматуре материала, который позволит снизить технологический цикл изготовления армированных ячеистобетонных изделий, будет способствовать облегчению конструкции и увеличит жизненный цикл изделия. Материалы и методы: рассматривается использование композитной арматуры, изготовленной с применением различных отвердителей (ангидридного и алифатического амина), влияющих на поведение композита в среде ячеистого автоклавного бетона (щелочная среда реакции, условия повышенной термостойкости). Результаты: результаты выполненных исследований показали существование возможности замены армирующих элементов при производстве ячеистого автоклавного газобетона. Выводы: на основании проведенных испытаний было принято решение продолжить лабораторные исследования на предмет термостойкости композитной арматуры с применением аминного отвердителя в среде ячеистого бетона автоклавного твердения. Подобная работа была проведена впервые, являясь ценной для усовершенствования существующей технологии производства армированного газобетона.
