Представлен обзор современных данных литературы по изучению роли различных цитокинов воспаления в патогенезе формирования и разрыва церебральных артериовенозных мальформаций (цАВМ). Разрыв цАВМ с формированием внутричерепного кровоизлияния - это наиболее частое и грозное клиническое проявление, приводящее к развитию стойкого неврологического дефицита, ранней инвалидизации и летальным исходам. В патогенезе развития цАВМ важнейшую роль играет воспалительный процесс, приводящий к истончению сосудистой стенки. Обнаружена зависимость между некоторыми генетическими полиморфизмами, которые усиливают экспрессию ряда ангиогенных и провоспалительных факторов, играющих ключевую роль в патогенезе разрыва цАВМ. Представлен анализ экспериментальных исследований, проводимых в мире, по влиянию генетических факторов и провоспалительных цитокинов на развитие, прогрессирование и разрыв цАВМ. Приведены современные данные о молекулярных терапевтических подходах к лечению цАВМ. Обозначены актуальные, остающиеся нерешенными вопросы, что обусловливает необходимость проведения дальнейших экспериментальных и клинических исследований при лечении данной нозологической формы. This review analyzes current reports on the pathogenetic role of inflammatory cytokines in formation and rupture of cerebral arteriovenous malformations (cAVM). The authors analyzed reports from PubMed, MedLine, and eLibrary databases that addressed the role of inflammatory cytokines in the molecular pathogenesis of cAVM formation and rupture. The cAVM rupture leading to intracranial hemorrhage is the most frequent and formidable clinical manifestation resulting in persistent neurologic deficits, early disability, and death. In the pathogenesis of cAVM development, an important role belongs to the inflammatory process, which induces thinning of the vascular wall. Relationships were found between some genetic polymorphisms that enhance expression of a number of angiogenic and proinflammatory factors that play a key role in the pathogenesis of cAVM rupture. The review presents experimental studies conducted worldwide and focusing on the influence of genetic factors and proinflammatory cytokines on cAVM development, progression and rupture. The authors provided modern molecular therapeutic approaches to the treatment of cAVM. The topical, still unsolved issues were identified, which warrant further experimental and clinical studies on the treatment of this nosological form.
В обзоре освещены данные современной литературы, посвященные изучению молекулярно-генетических механизмов гетеротопической оссификации (ГО). ГО представляет собой процесс образования пластинчатой кости в мягких тканях, не имеющих остеогенных свойств. В патогенезе формирования очагов ГО важнейшую роль играет ряд внутриклеточных сигнальных путей, ответственных за направление дифференцировки мезенхимальных стволовых клеток по пути остеогенеза. Представлен анализ экспериментальных исследований, изучающих молекулярные механизмы ГО. Авторами изложены современные данные о молекулярных терапевтических подходах к лечению и профилактике ГО. Обозначены актуальные, остающиеся нерешенными вопросы, что диктует необходимость проведения дальнейших экспериментальных и клинических исследований проблемы гетеротопической оссификации. This review addressed molecular and genetic mechanisms of heterotopic ossification (HO). HO is a process of lamellar bone formation in soft tissues lacking osteogenic properties. Several intracellular signaling pathways for differentiation of mesenchymal stem cells along with the osteogenesis pathway play an important role in the pathogenesis of HO formation. This review analyzed experimental studies of HO molecular mechanisms. The authors presented state-of-the-art information about molecular therapeutic approaches in HO treatment and prevention and identified relevant but still unresolved issues, which warrant further experimental and clinical studies.
Представлен обзор современных источников литературы, посвященных изучению молекулярных и клеточных механизмов резистентности глиобластомы к антиангиогенным лекарственным средствам. Ангиогенез представляет собой важнейший патофизиологический механизм роста и прогрессирования глиобластомы за счет активного развития микрососудистой сети. Ускоренное развитие микрососудистой сети в глиобластоме происходит благодаря синтезу опухолевыми клетками большого количества фактора роста эндотелия сосудов (Vascular Endothelial Growth Factor, VEGF). Среди основных молекулярных и клеточных механизмов лекарственной устойчивости глиобластомы к анти-VEGF агентам принято относить VEGF-независимые пути ангиогенеза, активность клеток костного мозга и перицитов, а также сосудистую кооперацию, периваскулярную инвазию и феномен аутофагии. Изложены современные данные о рациональном и наиболее эффективном использовании анти-VEGF-лекарственных средств у пациентов с глиомами высокой степени злокачественности. Обозначены актуальные, остающиеся нерешенными вопросы, что обусловливает необходимость проведения дальнейших экспериментальных и клинических исследований, посвященных изучению механизмов лекарственной устойчивости глиобластомы к анти-VEGF-препаратам. This state-of-the-art review focuses on molecular and cellular factors associated with glioblastoma resistance to antiangiogenic drugs. Angiogenesis is an important pathophysiological mechanism for the growth and progression of glioblastoma facilitated by active development of microvasculature. The accelerated development of the microvascular network in glioblastoma occurs due to the synthesis of a large number of vascular endothelial growth factor (VEGF) by tumor cells Among the major molecular and cellular factors, glioblastoma drug resistance to anti-VEGF agents is commonly attributed to VEGF-independent pathways of angiogenesis, bone marrow cell and pericyte activity as well as to vascular co-option, perivascular invasion, and the phenomenon of autophagy. The authors provided current data on the rational and most effective use of anti-VEGF drugs for patients with high-grade gliomas. Relevant unsolved problems associated with drug resistance of glioblastoma to anti-VEGF drugs were highlighted.
