Аортобифеморальное бифуркационное шунтирование -«золотой стандарт» хирургического лечения при синдроме Лериша. Однако мультифокальный атеросклероз, в особенности в сочетании с многососудистым поражением коронарных артерий у пациентов высокого риска, представляет собой нерешенную тактическую проблему для хирурга, в частности в случае острого коронарного синдрома. В связи с этим в указанной группе пациентов актуально рассмотрение вопроса об альтернативных методах реваскуляризации, а именно о экстраанатомической реваскуляризации артерий нижних конечностей от восходящей аорты в сочетании с реваскуляризацией миокарда. Анализ отечественной и зарубежной литературы показывает малое количество похожих клинических наблюдений, чем и обусловлена актуальность описания данного клинического случая. Мы представляем пациента 59 лет с юкстаренальной окклюзией аорты, ишемической болезнью сердца, нестабильной стенокардией, постинфарктным кардиосклерозом, тяжелой сердечной недостаточностью, которому одномоментно успешно выполнены аутовенозное аортокоронарное шунтирование передней межжелудочковой артерии и асцендобифеморальное шунтирование в условиях параллельного искусственного кровообращения. Описанная методика позволила успешно одномоментно реваскуляризировать миокард и нижние конечности без необходимости значительного расширения объема операции за счет исключения брюшного этапа и вместе с тем обеспечила адекватный для кровоснабжения нижних конечностей путь притока в виде восходящей аорты. Представленный метод может быть использован как альтернатива этапному лечению мультифокального атеросклероза у пациентов высокого риска.
