Type 1 diabetes is a chronic autoimmune disease that occurs as a result of the destruction of insulin-producing beta cells of the pancreas. Among all diseases of the endocrine system, DMT1 is the most significant medical and social problem in children and adolescents. Patients with diabetes are at higher risk of developing gastrointestinal disorders. Involvement of the gastrointestinal tract may occur as a complication of the underlying disease or may result from the operation of general autoimmune pathogenetic mechanisms. As is known, DMT1 is associated with affection of the entire gastrointestinal tract from the oral cavity and oesophagus to the large intestine and anorectal area. In this connection, the symptoms experienced by patients can vary significantly. Frequent complaints may include dry mouth, thirst, dysphagia, early satiety, heartburn, constipation, abdominal pain, nausea, vomiting, and diarrhoea. Poor glycaemic control worsens the course of the gastrointestinal tract pathology. Gastroenterological diseases aggravate the course of diabetes. Early diagnosis of comorbid diseases and intervention can prevent complications and potential negative effects on patients' quality of life. Post-infectious irritable bowel syndrome — one of the most frequent conditions diagnosed in pediatric gastroenterological practice. According to numerous research parisitosis contributes to the chronology of gastrointestinal tract pathology. it is known that the infection Lamblia intestinalis can increase the risk of developing post-infectious IBS, changing species composition and diversity of the intestinal microbiota, modulating metabolism of the host, the nature of the immune response and adversely affecting the mucosal barrier and the motility of the digestive tract. Importantly, the prevalence of giardiasis in patients with symptoms of IBS and dyspepsia at the moment, it remains at a high level. Further research into the pathogenesis of Lamblia intestinalis infection in patients with IBS will be of great importance in medicine.
Ведущими симптомами хронических иерсиниозов, которые, по мнению ряда авторов, относятся к «терапевтическим» инфекциям, являются симптомы гастроэнтерологического заболевания при отсутствии типичных проявлений инфекционного процесса.Цель исследования. Установить особенности клинической картины хронических иерсиниозов у детей и оценить эффективность методов лабораторной диагностики заболеваний. Характеристика детей и методы исследования. В группу пациентов с хронической иерсиниозной инфекцией (ХИИ) вошел 131 ребенок, 68 мальчиков и 63 девочки в возрасте от 2 лет 8 мес. до 17 лет 11 мес. (средний возраст группы составил 10 лет) с типичной гастроэнтерологической патологией. Критерием включения больных в изучаемую группу являлось лабораторное подтверждение диагноза иерсиниозов в ходе настоящего исследования. Лабораторное подтверждение диагноза псевдотуберкулез и кишечный иерсиниоз проводили с использованием метода полимеразной цепной реакции (ПЦР) для выявления патогенных иерсиний в фекалиях, специфические антитела определяли комплексом методов. Результаты. В статье приводится описание клинических вариантов хронического течения иерсиниозов у детей, вызываемых Yersinia pseudotuberculosis и патогенными Yersinia enterocolitica.
