Данная статья посвящена наиболее распространенной группе врожденных нарушений иммунитета. Представлена актуальная общая информация о группе нарушений с преимущественной недостаточностью антител, ее классификация. Эпидемиология первичных иммунодефицитов в Республике Беларусь имеет свои особенности, но не имеет критических отличий от общемировой. Безусловно, уровень выявления пациентов с врожденной недостаточностью антител ниже предполагаемого в связи с низкой настороженностью клиницистов, возможной манифестацией в различном возрасте, вариабельностью клинических проявлений, дорогостоящей молекулярной диагностикой. При этом существует заместительная терапия, которая может сделать инфекционный синдром контролируемым, уменьшить риск осложнений, которые могут приводить впоследствии к нарушению функции органов и систем и инвалидизации пациента, снизить активность аутоиммунной патологии и при этом смягчить иммуносупрессию, снизить вероятность ранней манифестации новообразований и в целом улучшить качество жизни пациента. По возможности предпочтение отдается подкожному иммуноглобулину и индивидуальному подходу к пациенту с получением заместительной терапии на дому. В связи с этим крайне важно привлечь внимание медицинской общественности к данной группе патологий для ранней диагностики и своевременной персонифицированной терапии. This article is devoted to the most common group of inborn errors of immunity. Current general information about the group of disorders with predominant antibody deficiencies and its classification are presented. The epidemiology of primary immunodeficiency in the Republic of Belarus has its own characteristics, but has no critical differences from the global one. Certainly, screening rates for patients with congenital antibody deficiency are lower than expected, due to clinicians’ low alertness, variable age of manifestation, variability of clinical manifestations, and costly molecular diagnostics. Meanwhile, a substitution therapy exists allowing this infectious syndrome to be controllable, reducing risks of complications further leading to organ and system dysfunction and patient disability, decreasing autoimmune pathology activity while mitigating immunosuppression, reducing the possibility of early neoplasm manifestation, and in general improving patient’s quality of life. Whenever possible, subcutaneous immunoglobulin and an individualized approach to each patient receiving substitution therapy at home are preferable. Therefore, it is extremely important to draw attention of the medical community to this group of pathologies for early diagnosis and timely personalized therapy.
