BackgroundThe body is dependent on the exogenous supply of omega-3 polyunsaturated fatty acids (n3-PUFA). These essential fatty acids are key players in regulating metabolic signaling but also exert anti-inflammatory and anti-carcinogenic properties. The liver is a major metabolic organ involved in fatty acid metabolism. Under experimental conditions, n3-PUFA exert beneficial effect on hepatic steatosis, regeneration and inflammatory insults such as ischemic injury after surgery. Some of these effects have also been observed in human subjects. However, it is unclear whether perioperative administration of n3-PUFA is sufficient to protect the liver from ischemic injury. Therefore, we designed a randomized controlled trial (RCT) assessing n3-PUFA (pre-) conditioning strategies in patients scheduled for liver surgery.Methods/DesignThe Omegaven™ trial is a multi-centric, double-blind, randomized, placebo- controlled trial applying two single doses of Omegaven™ or placebo on 258 patients undergoing major liver resection. Primary endpoints are morbidity and mortality one month after hospital discharge, defined by the Clavien- Dindo classification of surgical complications (Ann Surg 240(2):205–13, 2004) as well as the Comprehensive Complication Index (CCI) (Ann Surg 258(1):1–7, 2013). Secondary outcome variables include length of Intensive Care Unit (ICU) and hospital stay, postoperative liver function tests, fatty acid and eicosanoid concentration, inflammatory markers in serum and in liver tissue. An interim analysis is scheduled after the first 30 patients per randomization group.DiscussionLong-term administration of n3-PUFA have a beneficial effect on metabolism and hepatic injury. Patients often require surgery without much delay, thus long-term n3-PUFA uptake is not possible. Also, lack of compliance may lead to incomplete n3-PUFA substitution. Ñîâðåìåííûå òåõíîëîãèè ëå÷åíèÿ ðóáöîâûõ ñòðèêòóð aeåë÷íûõ ïðîòîêîâ Цель. Оценить современные тенденции в хирургии рубцовых стриктур желчных протоков на основе анализа отдаленных результатов различных вариантов вмешательств на желчных протоках, выполненных в крупном федеральном хирургическом центре за длительный период времени. Материал и методы. За период более 30 лет рассмотрен опыт хирургического лечения 349 пациентов с доброкачественными стриктурами желчных протоков. Восстановление желчеоттока посредством открытых операций произведено 301 (86%) пациенту. Антеградные чрескожные эндобилиарные вмешательства (чрескожная чреспеченочная холангиостомия, баллонная дилатация стриктур) выполнены 48 (14%) пациентам. Отдаленные результаты прослежены у 296 (85%) из 349 больных, из них у 258 больных с открытыми операциями и у 38 пациентов с эндобилиарными вмешательствами. Результаты. У 214 (83%) из 258 больных в отдаленном периоде получены хорошие результаты, заключавшиеся в отсутствии атак холангита. У 44 (17%) больных имел место рецидив стриктуры в сроки наблюдения более 20 лет. Эндобилиарная коррекция желчеоттока проводилась в сроки от 6 до 28 мес (в среднем 17 ± 10 мес). У 12 (25%) пациентов лечение закончено, признаков рецидива стриктуры нет (сроки наблюдения 4-24 мес). Заключение. Для разработки полноценной доктрины хирургического лечения рубцовых стриктур желчных протоков с учетом возможностей современных методов миниинвазивной коррекции желчеоттока необходимо дальнейшее накопление опыта применения эндобилиарных вмешательств, в том числе проведение контролируемых исследований. The management of biliary decompression in malignant hilar carcinoma remains controversial. This review shows the most relevant aspects of endoprosthetics for proximal biliary obstruction, including necessity of stenting and morphological verification before radical surgery, selection of approach to drain etc. The main contradictions and ways to solve them are presented in this article, based on evidence researches, international and expert consensus conferences.
Scintigraphic volumetry data are similar to those of CT volumetry in evaluation of future remnant liver volume. Scintigraphic volumetry may be used as an alternative in assessment of future remnant liver volume after advanced hepatic resections.
