Инфаркт миокарда – это состояние, при котором происходит ряд гемодинамических, воспалительных и метаболических нарушений. После перенесенного инфаркта миокарда многие пациенты имеют неблагоприятный прогноз. В этой связи существует острая необходимость в биомаркере количественного анализа крови, который эффективно оценивал бы состояние и резерв сердечной мышцы, а также прогноз в раннем постинфарктном периоде. В последнее время уделяется большое внимание исследованию роли адипонектина в прогнозе неблагоприятных сердечно-сосудистых событий в постинфарктном периоде. Множество исследований посвящено роли адипонектина при сердечно-сосудистых заболеваниях, но его роль в постинфарктном периоде противоречива. Целью данного исследования было изучить гендерные особенности уровня адипонектина в раннем постинфарктном периоде (30-40 сутки) у пациентов, перенесших коронарное стентирование, а также определить взаимосвязь данного биомаркера с толщиной эпикардиального жира и стенозом коронарных артерий 4-й степени. В исследование включили 93 пациентов (55 мужчин и 38 женщин) с впервые возникшим инфарктом миокарда, перенесших коронарную реваскуляризацию. Пациенты были рандомизированы в группы. Больные первой группы (37 мужчин и 18 женщин) не получали терапию c целью профилактики сердечно-сосудистых заболеваний до впервые возникшего инфаркта миокарда. Больные второй группы (18 мужчин и 20 женщин) получали терапию для профилактики сердечно-сосудистых заболеваний. Терапия, направленная на профилактику сердечно-сосудистых заболеваний (артериальная гипертензия, стабильная стенокардия), предполагала регулярный прием бета-адреноблокаторов, ингибиторов ангиотензинпревращающего фермента, статинов и антиагрегантов. В ходе исследования обнаружено, что стеноз 4-й степени одного коронарного сосуда достоверно чаще встречается у мужчин, не получающих профилактическую терапию в отношении сердечно-сосудистых заболеваний до впервые возникшего инфаркта миокарда; стеноз 4-й степени двух коронарных сосудов достоверно чаще встречается у мужчин, получающих терапию с целью профилактики сердечно-сосудистых заболеваний до впервые возникшего инфаркта миокарда; высокие уровни адипонектина в раннем постинфарктном периоде ассоциируются с многососудистым поражением коронарных артерий вне зависимости от получаемой терапии для профилактики сердечно-сосудистых заболеваний до впервые возникшего инфаркта миокарда. Myocardial infarction is a condition in which a number of hemodynamic, inflammatory and metabolic disorders occur. After myocardial infarction, many patients have a poor prognosis. In this regard, there is an urgent need for a quantitative blood test biomarker that would effectively assess the state and reserve of the heart muscle, as well as the prognosis in the early post-infarction period. Recent studies have paid great attention to the role of adiponectin in the prognosis of adverse cardiovascular events in the post-infarction period. Many studies have focused on the role of adiponectin in cardiovascular disease, but its role in the post-infarction period is controversial.The aim of the study was to study the gender characteristics of the adiponectin level in the early post-infarction period (30-40 days), in patients who underwent coronary stenting, as well as to determine the relationship of this biomarker with epicardial fat thickness and coronary artery stenosis of the 4th degree. The study included 93 patients (55 men and 38 women) with a first-time myocardial infarction who underwent coronary revascularization. The patients were randomized into groups. Patients of the first group (37 men and 18 women) did not receive therapy for the prevention of cardiovascular diseases before the first myocardial infarction. Patients of the second group (18 men and 20 women) received therapy for the prevention of cardiovascular diseases. Therapy aimed at the prevention of cardiovascular diseases (arterial hypertension, stable angina pectoris) included regular intake of beta-blockers, angiotensin-converting enzyme inhibitors, statins and antiplatelet agents. The study revealed: stenosis of the 4th degree of the 1st coronary vessel is significantly more common in men who do not receive therapy for the prevention of cardiovascular diseases before the first myocardial infarction; stenosis of the 4th degree of the 2nd coronary vessels was significantly more common in men receiving therapy for the prevention of cardiovascular diseases before the first myocardial infarction; high levels of adiponectin in the early post-infarction period are associated with multivessel damage to the coronary arteries, regardless of the received therapy for the prevention of cardiovascular diseases before the first myocardial infarction.
