Представлен обзор литературы о связи хронической боли и суицидального поведения. Показано, что от хронической боли страдает от 8 до 45% населения, а доля пациентов, испытывающих сильную боль, достигает 54%. Общим негативным фактором является недостаточный болеутоляющий эффект у 60% больных, что отрицательно сказывается на качестве их жизни и социальном функционировании. Хроническая боль относится к независимым факторам суицидального риска; суицидальные мысли выявляются в среднем в 28%; в 4 раза повышается риск совершения однократной и в 6 раз-многократных попыток самоубийства, в 2-3 раза-завершённых суицидов. «Сильная боль» сопряжена с 5-кратным ростом риска суицида. Среди самоубийств, обусловленных соматическим (нераковым) заболеванием 28,7% связаны с хронической болью. Хроническая боль редко выступает в качестве единственного просуицидального фактора. Сочетание с депрессией, видимо, делает боль непереносимой. Боль и рак. Хроническая боль регистрируется у 80-95% больных распространённым раком. Суицидальные мысли выявляются у 10%, а повышенный суицидальный риск, связанный с болью у 30-85% больных. Ведущий фактор риска-недостаточный болеутоляющий эффект и отсутствие системного применения опиоидных анальгетиков. Развитие депрессии у этих больных повышает риск суицида не в 2-4, но в 25 раз. Хронические головные боли имеют достоверную связь с суицидальными идеями (OR 1,9) и попытками самоубийства (OR 2,3) и определяют 1% от уровня самоубийств от всех причин. Попытки суицида происходят в 10 раз чаще. До 70-80% самоубийств при первичных головных болях приходится на кластерные (пучковые) боли и мигрень. При мигрени с аурой частота суицидальных мыслей-47,2%, попыток-13,9%. Наличие депрессии может повышать суицидальный риск в 15 раз. При кластерной (пучковой) головной боли суицидальная готовность тесно связана с фазой клинических проявлений.
