Objective: To study the structural and immunohistochemical features of placentas in women after assisted reproductive technology (ART) with allogeneic eggs (oocyte donation and surrogate motherhood). Study Design: The study involved 89 women whose pregnancy occurred as a result of in vitro fertilization (IVF) with a donor egg in a surrogate motherhood program (IVF-SM, n = 47 patients) or oocyte donation (IVF-DO, n = 42). The comparison group consisted of 21 patients in whom pregnancy occurred as a result of IVF with their own egg (IVF-OE). A clinical and anamnestic analysis of the pregnant women was carried out. Morphological and immunohistochemical studies were performed on placental material. Immunohistochemical analysis of CD8, CD56, CD138, and CD25/CD4 markers indicating the processes of impaired tolerance in placenta was carried out. В популяции, по данным ВОЗ [2], около 5% женщин бесплодны в связи с анатомическими, генетиче-скими, эндокринными и иммунологическими фактора-ми.В совместном докладе ВОЗ и Всемирного банка об инвалидности было показано, что бесплодие является восьмым по распространенности серьезным нарушением здоровья, при этом оно занимает пятое место после де-прессии, нарушения зрения, непредумышленных травм и алкоголизма среди групп населения в возрасте моложе 60 лет [3]. Указано, что паци-енток программы экстракорпорального оплодотворения относят к группе высокого риска невынашивания, осложненного течения беременности и родов. Большое внимание уделено особенностям течения беременности и родов у суррогатных матерей.Авторы информируют об отсутствии конфликта интересов. Ключевые слова: бесплодие, вспомогательные репродуктивные технологии, история развития, осложнения беременности, высокий процент кесарева сечения, суррогатное материнство, особенности течения беременности у суррогатных матерей.The review gives some historical stages in the development of assisted reproductive technologies and analyzes the results of recent researches dealing with obstetric problems in pregnant women after in vitro fertilization of both their and donor oocytes. The in vitro fertilization patients are mentioned to be assigned to a group at high risk for miscarriage, complicated pregnancy, labor, and delivery. Much attention is given to the specific features of the course of pregnancy, labor, and delivery in surrogate mothers. The authors declare no conflicts of interest.

There was executed an analysis of the literature data on fast track surgery in foreign and domestic publications for the last 7 years, included in the database PubMed, Medline, Scientific electronic library (elibrary.ru). The review considers in details the principles of fast track surgery and the rationale for their use. The results of application of accelerated rehabilitation programs in surgical specialties are presented and analyzed. The use such programs allow shorten the duration of hospitalization, restore the patient’s ability to work in a more shorter time, without reducing the effectiveness of the treatment, which would have a positive economic effect. The implementation of the fast track surgery program requires a multidisciplinary approach of all specialists involved in perioperative management of the patient.

Objective. To study the efficacy and to assess the safety of the use of titanium ribbon-like implants for surgical correction of various forms of apical prolapse. Patients and methods. 23 patients with apical prolapse (stage III–IV) aged 29 to 74 years were examined. Surgical correction of various forms of apical prolapse was performed using titanium ribbon-like mesh implants, which are more inert to the surrounding tissues than polypropylene scaffold systems. Dynamic outpatient observation of the patients was carried out after 3, 6, 12, 15 months after surgery. Results. The duration of surgery varied from 55 to 80 minutes, on average 74 ± 11.5 minutes. Intraoperative blood loss ranged from 50 to 150 mL (100 ± 35.7 mL). Of the early postoperative complications, only hematomas in the projection of the posterior vaginal wall of small sizes (up to 10 ml) – 2 (8.7%) were noted. Comparison of the questionnaire data from patients in the preand postoperative period showed an improvement in the quality of life of patients, an increase in their social and sexual activity. There was no significant displacement of the pelvic organs, and there were no mesh-associated complications. Conclusion. Laparoscopic bilateral subperitoneal hystero-, cervico-, colposuspension to the aponeurosis using titanium mesh ribbon-like implants improves the efficiency of surgical treatment of various forms and severity of apical prolapse.
