Київ Мета дослідження: вивчення особливостей клінічного перебігу остеоартрозу залежно від генотипу пацієнтів за геном фарнезілдифосфат синтази з метою удосконалення діагностики та прогнозування. У комплексному дослідженні 96 хворих на остеоартроз (ОА) виявлено, що переважають гомозиготи АА, частота яких достовірно більш висока, ніж гетерозиготів та гомозиготів з генотипом СС (53,1±5,1%, 41,7±5,0% та 5,2±2,3% відповідно; р<0,05). Частота гомозиготів з генотипом АА достовірно (практично у 8-10 разів; р<0,001) переважає частоту гомозиготів СС. Узагальнений індекс WOMAC у групах пацієнтів становив 48,8±1,8% та був більшим у гомозиготів АА з тенденцією до переваги у структурі тяжкості проявів скутості. Серед гомозиготів АА мають надмірну масу тіла або ожиріння 83,3±4,8%, серед гетерозиготів-75,0±6,8%, серед гомозиготів СС-80,0±19,1% осіб. Вміст апеліну у сироватці крові хворих на ОА залежно від індексу мТ та варіанта поліморфізму гена FDPS характеризуються тим, що при гомозиготності СС, збільшенні маси тіла вміст апеліну зменшується, тоді як при гомозиготності АА-достовірно не змінюється. Взаємозв'язок між поліморфізмом гена FDPS та стадією ОА характеризувався превалюванням у гомозиготних по алелі АА більш тяжких рентгенологічних проявів деградації суглобового хряща. Київ Мета дослідження: вивчення впливу рівня вмісту плазматичного апеліну у взаємозв'язку з поліморфізмом гена фарнезіл-дифосфат синтази та структурно-функціональним станом кісткової тканини (СФСКТ) у хворих на остеоартроз (ОА). Матеріали та методи. У комплексному дослідженні 96 хворих на ОА визначено, що серед усіх пацієнтів, які брали участь у дослідженні, переважають пацієнти з гомозиготним генотипом АА (по FDPS), частота яких виявлена достовірно більш високою, ніж у пацієнтів з гетерозиготним генотипом АС та гомозиготним СС: 53,1±5,1%, 41,7±5,0% та 5,2±2,3% відповідно (р<0,05). Результати. Частота пацієнтів з гомозиготним генотипом АА достовірно (практично у 8-10 разів; р<0,001) переважає частоту зустрічальності пацієнтів з гомозиготним генотипом СС. В осіб із гомозиготним генотипом СС серед хворих на ОА, коморбідний з остеопорозом, рівень вмісту плазматичного апеліну був достовірно вище, ніж у хворих на остеоартроз, поєднаний з остеопенією: 72,49±3,84 пг/см 3 та 42,97±2,15 пг/см 3 відповідно (р<0,05). У результаті дослідження доведено вплив поліморфізму гена FDPS на рівні варіативності концентрації плазматичного апеліну. Слід зазначити, що у хворих на ОА з генотипом АА без порушень СФСКТ рівні плазматичного апеліну відрізнялись від пацієнтів з остеопенією та достовірно перевищували аналогічний показник у пацієнтів цієї генотипової групи з остеопорозом (р<0,05). Заключення. доведено вплив гена на частоту та характер порушень структурно-функціонального стану кісткової тканини у хворих на остеоартроз (ОА): прогностично найбільш несприятливим є гомозиготний АА варіант, за наявності якого у хворих на ОА формування поєднаної остеопенії асоціюється зі збільшенням рівня плазматичного апеліну, а остеопорозу-його зниженням. Водночас підвищення концентрації апеліну при остеопенії можна оцінювати як транзиторне та використовувати в якості прогностичного маркера. Background:it is known that some genes (FDPS, LCT, VDR) which determine the calcium, vitamin D and lactate metabolism may have impact on osteoarthritis (OA) development and course, and thus, has a possible influence on OA treatment efficacy.Objectives:To determine the influence of genetic factors (genotyping of FDPS, LCT, VDR) on the efficacy of standard and modified treatment (with use of the platelet autologous plasma (PAP) in the early stages of knee OA.Methods:WOMAC index and the frequency of genotype variants for the FDPS, LCT, and VDR genes were studied in 96 patients (57 women, 38 men, 41.7 ± 1.2 years old) with primary knee OA (X-ray stage I-II). All patients had OA exacerbation with no clinical evidence of synovitis. Enrolled patients were divided into 2 groups: the first group consisted of 49 patients (27 women, 22 men, mean age 41.7 ± 1.2 years) who agreed to receive standard OA treatment (NSAID, exercises, orthopedic devises – as needed) and 3 intra-articular PAP injections (2 courses in 12 month, plasma volume 12-15ml/course, total platelet count per injection 1260,24 ± 22,1x109). The second group - 47 patients with OA who received standard treatment. Both groups were comparable by age, gender, body mass index and initial WOMAC. Genetic parameters and its influence on OA course and treatment efficacy was analyzed during 12 months.Results:The earliest age (37.2 ± 2.01 years) of clinical manifestation of knee OA was connected to homozygous genotype variants: LCT (relative risk 6.3:1), FDPS (relative risk 6.5:1) and VDR (relative risk 6.8:1). The best positive WOMAC changes was determined in patients with the CC genotype of LCT both in first and second groups. The WOMAC changes showed lower treatment efficacy in patients with CC genotype of FDPS and VDR in both groups, but results of patients who received PAP were better and their remission was longer (in 1.7 times) than in the standard treatment group.Conclusion: age of knee OA clinical manifestation and the treatment efficacy (both standard and with the use of platelet autologous plasma) has genetic predisposition.Disclosure of interests:None declared
BackgroundThe results of the PRP use in knee OA treatment are discussed ib the article.ObjectivesTo study the efficacy and safety of the PRP use in early stages of the knee OA.MethodsThe study was conducted at the Department of Family Medicine of the NMA of Postgraduate Education (Kyiv, Ukraine) and included 68 patients with diagnosed knee OA (I-II radiological stages).The patients with severe co-morbidities,trauma or after knee surgery were not included at the study.68 patients (27 men (39.7%) and 41 women (60.3%), mean age 41,3±1,2 years) were divided into 2 groups.Gr.1 included 34 patients who consented to receive standard OA treatment and 3 intra-articular injections of PRP (total volume 24–30ml,mean platelets number 869,24±32,1x109/μl)weekly;gr.2 consisted of 34 patients of comparable age with the same diagnosis who received only standard OA treatment.The CRP-level,WOMAC scale and Lequesne index was analyzed before treatment and in 1,3,6,12 months after course of treatment in both groups.ResultsCRP level was modestly abnormal in the 43,5% patients of group 1 and 38,7% patients of group 2; during the treatment period CRP got to normal in all patients, but during the follow-up period-after 6 and 12 months of treatment in the study group it was significantly lower (6,44±0,52) and (4,34±0,22) mg/L, than in the control group (7,96±0,60) and (6,72±0,51)mg/L (p<0,005). In addition in 5 patients of group 2 CRP level had increased again after 6 months which was the sign of OA exacerbation.WOMAC and Lequesne index had positive dynamic during treatment in both groups.At month 12 functional status of the patients in gr.1 improved by 33% in stiffness,pain during movement and after passing the distance decreased after treatment by 38.5% in gr.1,pain and discomfort when climbing and descending the stairs decreased by 38.9%. At the end of follow up period patients functional activity increased by 47.4% in gr.1 which was better then positive changes in gr.2 and was accompanied by a significant reduction of restrictions in daily activities.There were no adverse events due to use of PRP injections in gr.1.ConclusionsThe course of 3 intra-artricular injections added to the standard treatment of knee OA improves functional activity,reduces pain and probably can prolong remission in patients with the early stages of disease.The further long-term studies are needed with ultrasound/MRI monitoring of the articular cartilage to obtain more accurate information and determine the most effective methods of PRP use in OA treatment.ReferencesClinical and MRI outcomes after platelet-rich plasma treatment for knee osteoarthritis/B. . OA incidence and progression were predicted by HNs located on the 3rd digit (HR: 1.26(1.068e1.487) and 1.18(1.019e1.361), respectively). HN symmetry was predictive of OA incidence (Model-1, HR: 1.09(0.997e1.185)) and progression (Model-2, HR: 1.13(1.035e1.234)). Conclusions: HNs number, their locations, and symmetry were predictors of knee OA incidence and progression over 8-years.
