УО «Гродненский государственный медицинский университет», Гродно, Беларусь В настоящем обзоре представлен анализ литературных сведений о токсических воздействиях желчных кислот на ацинарные клетки поджелудочной железы как возможного патогенетического фактора острого билиарного панкреатита. Показано, что на начальной стадии развития острого билиарного панкреатита повреждение ацинарных клеток выражается в нарушении секреторного процесса, активности пищеварительных ферментов, строении и функции клеточных мембран и органелл. Предполагается, что общим механизмом, который опосредует финальную гибель ацинарных клеток, может быть нарушение кальциевых сигнальных путей, индуцируемое желчными кислотами. Желчные кислоты проникают в ацинарные клетки с помощью Na +-зависимых и Na +-независимых транспортеров плазматической мембраны, или связываются с G-белковым рецептором желчных кислот. Цитозольные желчные кислоты вызывают устойчивое увеличение концентрации Ca 2+ через механизмы, способствующие поступлению его в цитозоль из внутриклеточных депо и внеклеточного пространства. Перегрузка Ca 2+ приводит к внутриклеточной активации трипсиногена, деполяризации митохондрий, истощению АТФ и к последующей гибели ацинарных клеток.
УО «Гродненский государственный медицинский университет», Гродно, Беларусь В настоящем обзоре проанализированы литературные сведения о взаимоотношениях звездчатых и раковых клеток поджелудочной железы и их возможной роли в прогрессировании опухолевого процесса. В присутствии раковых клеток панкреатические звездчатые клетки активируются, что выражается в усилении их пролиферации и миграции, стимуляции синтеза компонентов внеклеточного матрикса и формировании плотной фиброзной ткани вокруг опухоли. В свою очередь панкреатические звездчатые клетки способствуют росту опухоли, её химио-и радиорезистентности. Они индуцируют пролиферацию и распространение раковых клеток, образование отдаленных метастазов.
The review presents an analysis of current scientific data on the role of ductal secretion disorders in the pathogenesis of chronic pancreatitis. It is shown that under the influence of alcohol or other provoking factors, the CFTR transport mechanism of duct cells undergoes damage, which leads to the decrease in the bicarbonate and fluid secretion. Dehydration and acidification contribute to the hyperconcentration of protein and mucous components of pancreatic secretion and to the change in its rheological properties, which complicates further transport through the ductal system. Current conditions are favorable for the formation of mucoprotein plugs and the ductal obstruction that may have an important role in the pathogenesis of chronic pancreatitis at the early stage of development of the disease.
