To study the structure of risk factors and associated cardiovascular and concomitant non-cardiac diseases in patients with atrial fibrillation (AF) on the base of the data of registry, and to estimate diagnostics and treatment quality in real outpatient practice in the Ryazan region with high cardiovascular mortality rate. Material and methods. A total of 3690 patients with AF, arterial hypertension (HT), ischemic heart disease (IHD) and chronic heart failure (CHF) consulted by general practitioners and cardiologists in three outpatient clinics of Ryazan, were enrolled into the outpatient REgistry of CardioVAScular diseAses (RECVASA). 530 of 3690 (14.4%) patients had the "atrial fibrillation" diagnosis in their outpatient charts: 175 (33.0%) men, 355 (67.0%) women, and mean age -72.3±10.1 years. Results. Permanent, paroxysmal or persistent forms of AF were indicated in outpatient charts of 43.2%, 26.4% and 24.7% of 530 AF patients, respectively, and 5.7% of outpatient charts did not specify AF form. All the AF patients had associated cardiovascular diseases (HT and/or IHD and/or CHF). The history of myocardial infarction, stroke and diabetes was revealed in 20.2%, 14.9% and 21.1% of patients, respectively. Revealed level of diagnostics did not satisfy adequate standard of examination with such cardiac pathology. Prescription of necessary drugs of some groups was insufficient, although indications existed. At the time of being included into the registry only 15.2% of AF patients received cardio-vascular drugs at a privileged price compared to 39.2% in the preceding years (p<0.05). AF diagnosis was confirmed in 84.2% of 76 randomly selected patients at its validation. Conclusion. The RECVASA research revealed high incidence of cardiac comorbidity in patients with the AF, underestimation of cardiovascular risk factors, insufficient use of diagnostic tests and prescription of some drug groups recommended by national and international guidelines. Изучить структуру факторов риска, сочетанной сердечно-сосудистой и сопутствующей патологии у больных с фибрилляцией предсердий (ФП), а также оценить качество диагностики и лечения в реальной амбулаторно-поликлинической практике с помощью регистра на территории Рязанской области -субъекта РФ с высоким уровнем сердечно-сосудистой смертности. Aim.To study the structure of concomitant cardiovascular and comobid pathology, risk factors in patients with arterial hypertension (AH), coronary heart disease (CHD), with congestive heart failure (CHF) and atrial fibrillation (AF), and to evaluate the quality of diagnostics and treatment in the conditions of real outpatient-polyclinic practice by the Registry in Ryazanskaya region – a RF region with high level of cardiovascular mortality.Material and methods.Into the outpatient-polyclinic registry RECVAZA (Registry of cardiovascular diseases) totally 3690 patients included with AH, CHD, with CHF, AF and their comorbidity, who admitted physicians’ offices in 3 outpatient institutions in Ryazan city: 1047 (28%) males and 2643 (72%) females, mean age 66,1±12,9 y.o.Results.The AHG diagnosis was mentioned in 3648 (98,9%) patients: CHD – in 2548 (69,1%), CHF – 2726 (73,9%), AF – 530 (14,4%). In 79,5% cases there was comorbidity. In general each one patient had 2,6 diagnoses of 4 analyzed. Myocardial infarction and brain stroke were in 11,4% and 9,5% patients’ anamnesis; diabetes – in 19,1%. The level of investigation of patients did not match the expected level with cardiologic pathology. There was non-sufficient amount of investigations and drugs prescribed, especially ACE inhibitors and angiotensine receptor blockers in CHF patients, statins in CHD, beta-blockers in patients with myocardial infarction, anticoagulants in AF, though they had indications. At the moment of Registry, a discount rate of drug price had 16,7% patinets vs. 33,1% in previous years (p<0,0001).Conclusion.RECVAZA results revealed in AH, CHD, CHF and AF patients high prevalence of cardiovascular comorbidity, non-sufficient risk factors evelauation nad accordance to national and international guidelines for treatment – important and real gap for diagnostics and treatment quality improvement in AH, CHD, CHF, AF anf their concomitance.

Изучить структуру сочетанной сердечно-сосудистой и сопутствующей патологии, факторов риска у больных артериальной гипертонией (АГ), ишемической болезнью сердца (ИБС), с хронической сердечной недостаточностью (ХСН) и с фибрилляцией предсердий (ФП), а также оценить качество диагностики и лечения в условиях реальной амбулаторно-поликлинической практики с помощью Регистра на территории Рязанской области-субъекта РФ с высоким уровнем сердечно-сосудистой смертности. Материал и методы. В амбулаторно-поликлинический регистр РЕКВАЗА (РЕгистр КардиоВАскулярных ЗАболеваний) включены 3690 больных АГ, ИБС, с ХСН, с ФП и их сочетаниями, обратившихся к терапевтам и кардиологам 3 поликлиник г. Рязани: 1047 (28%) мужчин и 2643 (72%) женщин, средний возраст пациентов 66,1±12,9 лет. Результаты. Выявленный уровень обследования пациентов с сердечно-сосудистыми заболеваниями (ССЗ) не соответствовал должному объему обследования при имеющейся кардиальной патологии. Отмечена недостаточная частота необходимого назначения препаратов ряда классов, в частности ингибиторов ангиотен-зин-превращающего фермента и блокаторов рецепторов ангиотензина у больных с ХСН, статинов у больных ИБС, β-адреноблокаторов у лиц, перенесших инфаркт миокарда, антикоагулянтов у больных с ФП, несмотря на наличие показаний. На момент включения в Регистр льготными лекарствами по поводу ССЗ пользовались 16,7% больных vs 33,1% в предыдущие годы (p<0,0001). Заключение. Результаты исследования РЕКВАЗА выявили у больных АГ, ИБС с ХСН и ФП высокую частоту сочетанной сердечнососудистой патологии и сопутствующих заболеваний, не полную оценку факторов кардиоваскулярного риска, недостаточное соответствие национальным и международным рекомендациям методов обследования и лечения. Повышение соответствия клиническим рекомендациям-важный и реальный резерв улучшения качества диагностики и лечения больных АГ, ИБС, с ХСН, с ФП и их сочетаниями в амбулаторно-поликлинической практике. Оценить качество диагностики и лечения пациентов с выраженной ГХС (уровень общего холестерина >6,2 ммоль/л) на амбулаторно-поликлиническом уровне. Материал и методы. В группу исследования (n=1642) включены все лица с диагнозом артериальная гипертензия, ишемическая болезнь сердца, хроническая сер-дечная недостаточность, фибрилляция предсердий, обратившиеся к участковым терапевтам или кардиологам одной из случайно отобранных поликлиник г. Рязани в марте-мае 2012 г. и включенные в регистр РЕКВАЗА. Результаты. Группу лиц с выраженной ГХС составил 561 человек (44%) в возрасте 67 (59-75) [Me (25%-75%)] лет. При этом диагноз гиперлипидемии указан толь-ко в 9% амбулаторных карт. Данные одного и более анализов крови с указанием уровня холестерина липопротеидов низкой плотности (ХС ЛПНП) были представле-ны только в 7% амбулаторных карт. К высокому и очень высокому сердечно-сосудистому риску были отнесены 83,7% больных с выраженной ГХС, при этом статины были рекомендованы только 17,8%. Статины были рекомендованы преимущественно в умеренных дозах, лишь только одному пациенту был назначен аторвастатин в дозе 40 мг/сут. Только у 5% больных, принимающих статины, был исследован уровень ХС ЛПНП на фоне терапии, целевые значения ХС ЛПНП достигнуты у них не были. Заключение. Данные исследования позволили выявить высокую частоту наличия выраженной ГХС среди больных с сердечно-сосудистыми заболеваниями и низкое качество диагностики и лечения больных с выраженной ГХС на амбулаторно-поликлиническом уровне. To assess the quality of diagnosis and treatment of patients with severe hypercholesterolemia (total cholesterol >6.2 mmol/L) in the real outpatient practice. Material and methods. All patients with a diagnosis of arterial hypertension, ischemic heart disease, chronic heart failure, atrial fibrillation applied to primary care physicians or cardiologists in one of the randomly selected out-patient clinic of Ryazan in March-May 2012 and included into the RECVASA registry were enrolled into the study group (n=1642). Results. The group of patients with severe hypercholesterolemia consisted of 561 (44%) patients at the age of 67 (59-75) years [Me (25% -75%)]. At that, diagnosis of hyperlipidemia was indicated only in 9% of outpatient cards. Aim. To evaluate the structure of combined cardiovascular diseases, drug treatment and observation of patients with a history of stroke in the framework of prospective outpatient registries. Material and methods. The study was conducted based on 3 outpatient clinics of Ryazan city. Patients with a history of acute cerebrovascular accident (ACVA) of any remoteness (AR) were included into ACVA-AR outpatient registry (n=511). Patients who had visited the outpatient clinics for the first time (FT) after cerebral stroke (n=475) were included into the ACVA-FT outpatient registry. The structure of the cardiovascular diseases (CVD), compliance with the clinical recommendations of the prescribed and received drug therapy were evaluated. The proportion of patients with dispensary observation for CVD, using preferential drug provision was determined. Results. A combination of 2 or more CVDs was found in 84.4% and 82.5% of cases, and severe cardiovascular multimorbidity (3-4 CVDs) – in 69% and 64% of cases, respectively, in ACVA-AR and ACVA-FT registers. Compliance with the clinical guidelines prescribed and received drug therapy was insufficient at the outpatient stage. Necessary prescription of drugs with a proven beneficial effect on the prognosis were observed significantly more frequent in the ACVA-FT registry, compared to the ACVA-AR registry at the enrolling stage of the study (p<0.05): statins for stroke – 50.1% vs 25.2%; statins for coronary heart disease (CHD) – 47.2% vs 27.9%; antiplatelet agents for CHD without atrial fibrillation – 65.6% vs 54.3%; anticoagulants for atrial fibrillation – 17.7% vs 9.3%; beta-blockers for heart failure 43.5% vs 33.1%, respectively. After 2-3 years of the follow-up frequency of prognostically significant prescriptions in patients of the compared registries were not significantly different, except prescriptions for statin therapy (47.6% vs 21.3%, respectively). The prognostically significant prescriptions during the enrolling stage in ACVA-AR and ACVA-FT registries occurred in 44.4% and 54% of the total number of proper prescriptions, and in the long-term follow-up period – in 55% and 57%, respectively; and the dispensary observation coverage was only 35.0% and 31.8%, respectively. According to patient contact only 21-24% of patients used the system of preferential drug provision at the stage of inclusion into the registers, and after 2-3 years of follow-up – 1.5-2 times less (12-14%). Conclusion The results of the study REGION found the presence of cardiovascular multimorbidity in 83% of patients with a history of stroke, insufficient quality of prescribed drug therapy in the out-patient clinic, especially in the ACVA-AR registry. The quality of medical treatment of patients improved within 2-3-year follow-up after the reference visit to out-patient clinic, but not sufficiently. Increase in dispensary observation coverage and optimization of the system of preferential drug provision are also important reserves for improving the quality of treatment of patients with a history of stroke, as well as prevention of cardiovascular complications.

To study a pattern of concomitant cardiovascular diseases (CVDs) and to estimate particularities and quality of medical antihypertensive therapy in hypertensive patients in real outpatient practice with a help of the Registry in Ryazan region. Material and methods. A total of 3690 patients with hypertension, ischemic heart disease, chronic heart failure and atrial fibrillation, who had attended general practitioners and cardiologists of 3 outpatient clinics in Ryazan city, were enrolled in the outpatient Registry of cardiovascular diseases (RECVASA). The diagnosis of hypertension was recorded in 3648 of 3690 (98.9%) outpatient charts, 28.1% of the subjects were men and 71.9% -women. Results. A total of 2907 (79.7%) of 3648 patients had combination of hypertension with other CVDs. Combination of 3-4 cardiovascular diagnoses was registered in 63.8% of the cases. 11.5% and 9.5% of the patients had a history of myocardial infarction and cerebral stroke, respectively. Diagnosis of hypertension was verified in 448 of 450 randomized hypertensive patients (99.6%). The incidence of prescription of one and two antihypertensive drugs (AHDs) was 25% and 39%, respectively, of 3 AHDs -21%, 4 and more -2%. AHDs were not prescribed in 13% of hypertensive patients. The mean number of prescribed AHDs was 1.73. The mean incidence rate of target blood pressure achievement was 26.1%. We have noted insufficient ACE inhibitors/angiotensin receptor blockers (ARB) and beta-blockers prescription in different concomitant CVDs. Patients with 3-4 cardiovascular diagnoses were more often prescribed combined antihypertensive treatment. Prescription of ACE inhibitors/ARB, beta-blockers and thiazide diuretics combination was preferable in 74.1% of the cases, when taking into account absolute and relative contraindications for betablockers use -in 64.0%. 15.2% of the hypertensive patients used reimbursed drugs for CVDs at the moment of the Registry enrollment as compared with 39.2% in previous years (p<0.05). Conclusion. The RECVASA study data allowed revealing high incidence of concomitant CVDs in hypertensive patients, insufficient use of combined antihypertensive treatment, including AHDs with proved favorable influence on prognosis. Achievement of concordance of medical treatment to national and international guidelines, taking into account concomitant CVDs, and optimization of patients' coverage with reimbursed drugs are the main reserves for antihypertensive treatment quality improvement. Patients on βblockers had 1,8 times lower risk of MI. Conclusion. Prospective followup of cardiovascular patients under the frame of RECVASA registry showed significant negative influence on prognosis the following: age, male gender, anamnesis of MI and S, DM, COPD, CHF (34 FC), angina (34 FC), permanent AF, BP <110/75 and ≥180/110 mmHg, GFR <45 ml/min, HR >90/min, combination of AH, CHD, CHF and AH, CHD, CHF, AF. Positive influence on prognosis with CVD had treatment with βadrenoblockers, iACE, ARB.

Больные с сочетанием хронической сердечной недостаточности, артериальной гипертонии и перенесенного ранее инфаркта миокарда: клинико-анамнестические характеристики и практика назначения ингибиторов ангиотензин-превращающего фермента, блокаторов рецепторов ангиотензина и β-адреноблокаторов, приверженность лечению (данные амбулаторного регистра РЕКВАЗА) Цель. Изучить клинико-анамнестические характеристики, частоту назначения в амбулаторной практике ингибиторов ангиотензин-превра-щающего фермента (ИАПФ)/блокаторов рецепторов ангиотензина (БРА) и β-адреноблокаторов (β-АБ), приверженность медикаментозной терапии у больных с сочетанием хронической сердечной недостаточности (ХСН), артериальной гипертонии (АГ) и инфаркта миокарда (ИМ) в анамнезе в РЕгистре кардиоВАскулярных ЗАболеваний (РЕКВАЗА). Материал и методы. В рамках регистра РЕКВАЗА выполнен анализ данных в группах пациентов с сочетанием ХСН, АГ и перенесенного ИМ (n=406) и пациентов с сочетанием ХСН, АГ и ишемической болезни сердца (ИБС) без анамнеза ИМ (n=1897). В этих группах сопоставле-ны структура сочетанной сердечно-сосудистой патологии и сопутствующих некардиальных заболеваний, выраженность клинических проявлений ХСН, ИБС и АГ, частота назначения ИАПФ/БРА и β-АБ, приверженность медикаментозной терапии (по опроснику Мориски-Грина). Результаты. Больные с сочетанием ХСН, АГ и ИБС с и без ИМ в анамнезе значимо различались по доле лиц мужского пола (47,8% против 24,9%, соответственно), частоте наличия фибрилляции предсердий (25,9% против 20,5%, соответственно), сахарного диабета (27,3% про-тив 15,7%, соответственно) и острого нарушения мозгового кровообращения (ОНМК) в анамнезе (17,2% против 10,7%б соответственно). При этом в сравниваемых группах не было значимых различий среднего возраста (69,9±11,0 против 70,3±11,0 лет, соответственно), а так-же частоты наличия в анамнезе болезней органов дыхания, хронической болезни почек, заболеваний органов пищеварения, ожирения и ане-мии. У больных с сочетанием ХСН, АГ и постинфарктного кардиосклероза (ПИКС) по сравнению с пациентами без ПИКС значимо чаще ди-агностировались 3-4 функциональный класс ХСН (62% против 47,9%, соответственно), АГ 3 степени (92,5% против 84,2%, соответственно), стенокардия напряжения 3-4 функционального класса (84,4% против 66,4%, соответственно). У больных с ПИКС чаще (p<0,05) назнача-лись β-АБ (56,7% против 42,2%, соответственно), комбинация ИАПФ/БРА с β-АБ (44,6% против 35,1%, соответственно), но реже -ИАПФ/БРА (73,7% против 77,6%, соответственно). Доля лиц, приверженных лечению (4 балла по шкале Мориски-Грина) была большей у больных с ПИКС (37,2% против 30,6%, соответственно; р<0,05). Заключение. Больные с ХСН в сочетании с АГ и ПИКС по сравнению с пациентами без ПИКС характеризовались большей частотой наличия фибрилляции предсердий, сахарного диабета и перенесенного ОНМК, более тяжелым клиническим течением ХСН, АГ и ИБС, большей ча-стотой назначения β-АБ, комбинации ИАПФ/БРА и β-АБ, меньшей частотой назначения ИАПФ/БРА, а также более высокой приверженностью медикаментозной терапии.
