The increase in the incidence of colorectal cancer cannot go unnoticed. The number of patients with its complications in general surgical hospitals is growing steadily. The main ones include acute intestinal obstruction, bleeding, tumor destruction with paracancerous abscess formation or perforation. This limits the possibilities of carrying out the radical treatment of oncoproctological patients significantly, and reduces the number of resection interventions considerably. The incidence of postoperative complications in emergency patients is statistically higher than after elective surgeries. The most fatal is the failure of the enterocolonic anastomosis, usually requiring relaparotomy and often colostomy. With regional tumors, combined interventions are necessary, a prerequisite for which is the availability of trained specialists. The article presents a clinical case of performing a major surgery with a positive outcome for complicated colon cancer in a hospital on duty.
Introduction. The prevalence of colorectal cancer in the world is increasing every year. In more than 40% of patients, the disease debuts with a clinical picture of acute intestinal obstruction, while in one third of cases the tumor is located in the right half of the colon. This leads to the necessity to perform colon resection or bypass surgery, the main stage of which is making entero- colic anastomosis. However, up to 15% of these anastomoses has a leakage. Development of methods of forming a reliable anastomosis is thus critical.Materials and methods. 37 patients with acute intestinal obstruction, divided into 2 groups (17 and 20 people), are included into the study. All of them, as a surgical stage of treatment, underwent a right hemicolectomy with the formation of an entero- colic anastomosis or a bypass surgery. A double-row side-to-side anastomosis was used in the comparison group, and a single-row “end-to-side” anastomosis in the main group (patent No. 2709253). The incidence of anastomotic leakage and mortality were assessed. Comparison was performed using Fisher’s exact test.Results. There were no cases of anastomotic leakage and mortality in the main group. In the comparison group, there were 5 leakages and 1 death.Discussion. When a modified entero-colic anastomosis is formed, adequate blood flow is maintained in the walls of the anastomosed intestinal loops, which contributes to adequate regeneration.Conclusion. The first clinical experience with the modified end-to-side entero- colic anastomosis is successful. It is planned to further recruit patients and study the features of the postoperative period.

При формировании артериовенозной фистулы (АВФ) пациентам оперативное пособие дополняли интраоперационным приемом «гидравлического бужирования». При мор-фологическом исследовании сегментов подкожных вен в случае дисфункции АВФ в сосудистой стенке опре-делялась структурная дезорганизация и замещение участками склероза. На основе полученных данных разра-ботан алгоритм оптимизации хирургического пособия по формированию АВФ.

There were formation of arteriovenous fistula (AVF) by using optimizing technique of hydraulic dilatation. Morphologic study of superficial veins segments in case dysfunction AVF shows structural disorganization and some portions of multiple sclerosis. We developed the algorithm of optimizing surgical planning of AVF creation.
ВВЕДЕНИЕПостоянный сосудистый доступ (ПСД) яв-ляется необходимым условием длительной жиз-ни пациентов с хронической болезнью почек (ХБП) 5-й стадии [2,4]. На настоящий момент наилучшим видом ПСД является нативная арте-риовенозная фистула (АВФ) [4, 8, 9]. Несмотря на то что метод формирования АВФ известен около 50 лет [7], результаты функционирования нативной АВФ не всегда оправдывают ожидания хирургов: первичная проходимость артериове-нозной фистулы в течение 1 года составляет до 65% [5, 15, 17]. В настоящее время проводится множество исследований по уточнению факторов, оказывающих влияние на функционирование сформированной АВФ, в частности, большой интерес представляет изначальное состояние подкожной вены. Это внимание к венозному сосуду обусловлено в большей степени тем, что причинами дисфункции чаще являются патоло-гические изменения в венозном сегменте [16], а именно стеноз и тромбоз [6, 16].Цель исследования -уточнить морфофунк-циональное состояние подкожных вен пациен-тов с ХБП 5-й стадии.
МАТЕРИАЛ И МЕТОДЫВ настоящем исследовании проводилось фор-мирование постоянного сосудистого доступа методом АВФ на предплечье пациентам с ХБП 5-й стадии. В исследовании приняли участие 85 пациентов, в том числе 42 женщины и 43 мужчины. Средний возраст пациентов составил (53,1 ± 11,9) года.
