Цель. Оценить возможность электронного сбора данных о пациентах со ста-бильной стенокардией после планового чрескожного коронарного вмеша-тельства (ЧКВ) (ангиопластики со стентированием коронарных артерий) в ФГБУ "НМИЦ им. В. А. Алмазова" МЗ РФ. Провести изучение качества жизни и удовлетворенности пациентов результатами ЧКВ в условиях реальной кли-нической практики. Материал и методы. Разработан опросник, включающий валидизированные русскоязычные версии анкет Seattle Angina Questionnaire (SAQ) и EQ-5D-5L. Опросники заполняются пациентами при поступлении на плановое ЧКВ, перед выпиской из стационара, через 1 неделю, 1, 6 и 12 месяцев после выписки из стационара. После выписки опросники высылаются пациентам по элек-тронной почте или осуществляется сбор информации по телефону. Исследо-вание выполняется за счет гранта Российского научного фонда (проект № 17-15-01177). Результаты. С 01.01 по 30.04 2017г выполнено ЧКВ у 558 пациентов (70,7% мужчины) в возрасте 64,9±9,6 лет. Подписали Информированное согласие на участие в исследовании 59% пациентов, из них менее 1% используют элек-тронную почту. По данным EQ-5D-5L баллы составили: исходно 64,4±19,6, перед выпиской 67,0±19,4, через 1 неделю 67,7±17,6 и через 1 месяц 63,3±19,2. По данным SAQ через 1 месяц пациенты отметили повышение физической активности, стабильности стенокардии, качества жизни и умень-шение тяжести стенокардии. Через 1 неделю и 1 месяц не удовлетворены результатами операции были 19% и 13% пациентов, 68% и 69% отмечали высокую удовлетворенность результатами ЧКВ. Результаты ЧКВ соответство-вали ожиданиям у 78% и 79% пациентов, соответственно. Заключение. При сборе информации от пациентов частота ответа на сооб-щения по электронной почте крайне низка, что требует длительного сбора информации по телефону. Необходима разработка методов электронного сбора информации (сообщения, мобильные приложения) для упрощения сбора данных и организации обратной связи между пациентами и врачами. Результаты обработки анкет показали несоответствие данных разных опрос-ников друг другу, что требует разработки единого специфического опросника с его последующей валидизацией. Aim.To describe the characteristics of patients who were invited in observational, prospective study on a value-based approach to assess the outcomes of treatment of patients with stable coronary artery disease (CAD) by percutaneous coronary intervention (PCI).Material and methods.We analyzed the baseline characteristics of patients with stable CAD enrolled in a planned PCI with stenting at Almazov National Medical Research Center in 2017. Statistical processing of the results was carried out using Statistica 6.0 (Statsoft Russia) and Microsoft Excel 2017. All registered patients were included in the statistical analysis. Based on the initial clinical characteristics of the patients included in the study, descriptive statistical parameters were calculated. For continuous variables, the mean value and standard deviation were calculated, for the categorical variables — the frequency of characteristic occurrence.Results. From January 1, 2017 to December 31, 2017, were performed 1740 PCI operations with stenting in 1458 patients aged 63,8±9,6 years (men — 1008 (69,1%)). At the time of admission to the hospital, the body mass index was 29,4±5,2 kg/m2, BP=132,3±15,1/79,6±8,5 mm Hg; HR=69,4±8,3 beats per minute. PCI of one coronary artery was performed in 75,6%, multivascular — in 16,6%, in chronic occlusions — in 4,6% and in bifurcation lesions — in 3,2% of patients, respectively. Type 4a myocardial infarction as a complication of PCI was registered in 2,9% of patients. The average duration of inpatient treatment was 5,5±3,4 days.Conclusion.Patients with stable CAD who underwent PCI are a representative sample which allows the assessment of treatment outcomes, an objective assessment of the intervention and the calculation of pharmacoeconomic parameters. An observational study will help identifying the factors affecting the efficiency of surgery and getting knowledge of patients’ opinions about this method.
The aim of this study is to assess the life quality of patients with stable coronary artery disease after angioplasty and stenting of coronary arteries at the post-hospital stage.Materials and methods. Methods of the sociological analysis (questionnaire surveys) and methods of mathematical statistics (descriptive statistics, time series method, factor and variance analyses) were used at different stages of the prospective observational study. The research materials were as follows:1458 electronic patient records with a stable coronary heart disease (SCHD) after angioplasty and stenting of coronary arteries (ASCA); 620 questionnaires filled in by patients before the surgery, 1, 6, 12 months after discharge. The statistical analysis was performed using the IBM SPSS Statistics software.Results. The results of a comprehensive survey make it possible for us to assert that during the studied period, stable good healths of cardiac surgery patients with ASCA were maintained. Within the framework of the EQ-5D-5L questionnaire, it was revealed that more than 50% of patients have no physiological problems. The results of the SAQ analysis demonstrate that 58% of the patients feel better, and more than 34% of the patients do not have shortness of breath 1 year after the surgery. A statistically significant improvement in their healths was established according to a visual analogue scale relatively to the annual observation mark (62.82 ± 20.95), which corresponds to the high results assessment of the medical technology use. At the same time, 53% of the patients notify that the treatment results meet their own expectations.Conclusion. The proposed calculation of the integrated index of patients’ treatment efficiency demonstrated by the patients with stable coronary heart disease after angioplasty and stenting of the coronary arteries is based on the results of the factor analysis. This calculation can be used to assess the efficiency of pharmacotherapy in the framework of a value-oriented approach to the treatment of a number of other pathologies.
