This chapter presents the recent literature data data on the problems of etiopathogenesis, cardiac, and obstetric risk of mitral valve prolapsus, as well as the tactics of patients with this pathology. Modern views on the role of genomic, genetic disorders, and metabolomics in violations valvular structures in the etiology and pathogenesis of clinical manifestations of the mitral valve prolapsus. In addition, the data on the peculiarities of pregnancy and childbirth in women with mitral valve prolapsus (miscarriage, cervical incompetence, preeclampsia, fetal growth restriction, placental insufficiency, labor anomaly, and postpartum hemorrhage) are studied. However, ambiguous and sometimes conflicting data on the relationship and the incidence of these complications with mitral valve prolapsus require further research to determine the set of diagnostic and preventive measures.
ВведениеВо время беременности особенно актуален принцип персонализированной фармакотерапии. Подбор терапии в случаях экстрагенитальных заболеваний или возникающих осложнений беременности -особенно трудная и важная для клинициста задача, поскольку требует от него знания всевозможных неблагоприятных влияний назначаемых препаратов на органо-и гистогенез у плода. Особенности фармакокинетики лекарственных веществ во время беременности отличаются от таковых у небеременных женщин: изменение действия препаратов наблю-дается у антибиотиков, антигипертензивных и антиретровирусных препаратов [1−5], что обусловлено не только генетическими характеристиками женщины, но и многочисленными физиологическими изменениями, происходящими в ее организме, в том числе синтезом ряда белков-регуляторов метаболизма и гормонов фетоплацентарным комплексом [6]. Безусловно, систематизация действия лекарственных веществ во время беременности была необходима, в первую очередь по их тератогенному и эмбриотоксическому эффекту. В 1975 г. Беременность -состояние, для которого особенно актуален принцип персонализированной фармакотерапии, принимая во внимание физиологические изменения в организме женщины, а также развивающийся плод, в отношении которого возможны неблагоприятные действия, в том числе дозозависимые, от применяемых лекарственных препаратов. Вопрос персонифицированной фармакотерапии гипертензивных нарушений имеет тесную взаимосвязь с клинической фармакогенетикой, так как за особенностями изменения метаболизма препарата во время беременности стоит генетический код женщины, обусловливающий активность работы цитохромов печени, плодово-плацентарного барьера, почечного клиренса. Сбор дополнительной информации поможет сформировать терапевтические модели и определить пути персонификации фармакотерапии при беременности. Целью настоящего обзора является обобщение имеющихся данных по фармакогенетике антигипертензивных препаратов, применяемых во время беременности.
