Рост числа пациентов с психическими расстройствами в стационарах общего профиля в условиях сокращения длительности госпитализации требует проведения диагностических и лечебных мероприятий в максимально сжатые сроки. Эффективным методом психотерапевтического лечения, способным охватить большое число больных за короткий срок, является групповая гипносуггестивная психотерапия (ГСПТ). Цель исследования-оценка эффективности короткого курса групповой ГСПТ у пациентов многопрофильного стационара с сопутствующими основному заболеванию пограничными психическими расстройствами, а также определение влияния однократного сеанса такой терапии на состояние больных. Пациенты и методы. Основную группу составили 78 больных, которым проводили ГСПТ, контрольную группу-37 больных, которые ГСПТ не подвергались. Динамику психического состояния в процессе лечения оценивали с помощью перечня симптомов опросника SCL-90-R (Simptom Check List-90-Revised); текущее состояние пациентов-с использованием опросника САН (самочувствие, активность, настроение), субтеста ситуационной тревоги интегративного теста тревожности (ИТТ-ст). Результаты и обсуждение. У пациентов основной группы отмечена более выраженная степень редукции психических расстройств, особенно симптомов тревоги и депрессии, по сравнению с таковой в контрольной группе. Показано, что однократный сеанс ГСПТ положительно влияет на текущее состояние пациентов, значительно улучшая их самочувствие и уменьшая проявления тревоги. Улучшение как соматического, так и психического состояния пациентов непосредственно перед выпиской из стационара представляется важным терапевтическим и социальным фактором, в том числе для успешности амбулаторного лечения. Заключение. Для уточнения уровня сохранности эффекта короткой интервенции с использованием ГСПТ необходимо проведение катамнестического исследования. Применение ГСПТ может быть эффективно у пациентов многопрофильного стационара.
Drug-induced parkinsonism (DIP) is a frequent parkinsonian syndrome that appears as a result of pharmacotherapy for the management of psychosis. It could substantially hamper treatment and therefore its diagnosis has a direct influence on treatment effectiveness. Although of such high importance, there is a lack of systematic research for developing neuroimaging-based criteria for DIP diagnostics for such patients. Therefore, the current study was aimed at applying a metabolic brain imaging approach using the 18F-FDG positron emission tomography and spatial covariance analysis to reveal possible candidates for DIP markers. As a result, we demonstrated, to our knowledge, the first attempt at the application of the Parkinson’s Disease-Related Pattern (PDRP) as a metabolic signature of parkinsonism for the assessment of PDRP expression for schizophrenia patients with DIP. As a result, we observed significant differences in PDRP expression between the control group and the groups with PD and DIP patients. Similar differences in PDRP expression were also found when the non-DIP schizophrenia patients were compared with the PD group. Therefore, our findings made it possible to conclude that PDRP is a promising tool for the development of clinically relevant criteria for the estimation of the risk of developing DIP.
Having measurable physiological correlates, hypnosis should be measurable generally itself. The precise, continual, quantitative assessment (versus phenomenological one) of a current trance level (i.e., “depth”) is possible only instrumentally. We’ve shown that electrophysiological patterns of a trance are stable from session to session, but significantly vary among subjects. Hence, to measure the trance level individually we proposed the following Brain-Computer interface approach and tested it on the 27 video-EEG recordings of 8 outpatients with anxiety and depressive disorders: on the data of the first session using Common Spatial Pattern filtering and Linear Discriminant Analysis classification, we trained the predictive models to discriminate conditions of “a wakefulness” and “a deep trance” and applied them to the subsequent sessions to predict the deep trance probability (in fact, to measure the trance level). We obtained integrative individualized continuously changing parameter reflecting the hypnosis level graphically online, providing the trance dynamics control. The classification accuracy was high, especially while filtering the signal in 1.5-14 and 4-15 Hz. The applications and perspectives are being discussed.
