The objective of the study: detection of early preclinical manifestations of vision disorders in patients with TB/HIV co-infection using optical coherence tomography (OCT).Subjects and methods. A prospective open-label study was carried out enrolling patients with TB/HIV co-infection (Group 1, n = 19) and patients with pulmonary and extrapulmonary tuberculosis (Group 2, n = 18). In both groups, standard ophthalmological examination and OCT were performed with measurement of the thickness of the choroid and the diameter of upper and lower temporal and nasal arteries and veins, and assessment of arteriovenous coefficient (AVC).Results. In the patients of both groups, the thickness of the choroid in the fovea area decreased in comparison with healthy volunteers (control group, n = 20), however, the decrease in choroid thickness was statistically significantly more pronounced in the patients of Group 1, and this group was also characterized by change in the caliber of retina arteries and veins, which is expressed in a higher AVC.Conclusion. OCT allows early preclinical diagnosis of eye disorders (in the absence of eye complaints) in patients with tuberculosis, including those with concurrent HIV infection. Detection of choroid disorders and AVC changes in such patients allows timely prescription of therapy and prevention of pathological changes from progression.

Обследованы 90 больных туберкулезом легких, разделенных на 3 группы по интенсивно-сти и форме заболевания: первая -32 больных инфильтративным туберкулезом с поражением 2-х сегментов; вторая -31 пациента с туберкулемой легкого, третья -27 больных фиброзно-каверноз-ным туберкулезом с давностью заболевание не более двух лет после относительной стабилизации процесса. Четвертая -группа сравнения -30 здоровых добровольцев. Проведенный анализ пока-зал, что в целом при туберкулезе легких аминокислотный баланс моноцитов можно охарактеризовать как дефицитный по антиоксидантным ресурсам. В случае туберкулезного воспаления в большей сте-пени критерием специфической резистентности организма являются не столько процессы окисления глутатиона, а способность накапливать необходимые концентрации таурина в иммунокомпетентных клетках. Показатели соотношения таурина в плазме и моноцитах обнаруживают различные значения в зависимости от клинической формы туберкулеза легких. Перераспределение аминокислот плазмы в моноциты и, наоборот, «вымывание» из клеток некоторых аминокислот можно рассматривать в ка-честве фактора, отражающего тяжесть течения инфекционного процесса. Возрастание в популяции числа лиц с поздними стадиями ВИЧ-инфекции обусловливает особенности течения как впервые выявленного туберкулеза, так и его рецидивов и, соответственно, требует формирования особого подхода к наблюдению за этими пациентами в диспансерных группах� Цель исследования: определить особенности рецидивов туберкулеза у пациентов с ВИЧ-инфекцией� Материалы и методы. В исследование ретроспективно включено 205 пациентов из Свердловской области� Всем пациентам установлен диагноз рецидива туберкулеза органов дыхания (имелись единичные случаи сочетания с туберкулезом других локализаций)� Из них у 104 пациентов диагностирована ВИЧ-инфекция (I группа), у 101 пациента был ВИЧ-негативный статус (II группа)� Группы были сопоставимы по возрасту и гендерному составу больных� Результаты. Рецидивы туберкулеза у пациентов с ВИЧ-инфекцией возникают раньше, чем у ВИЧ-негативных (через 31,5 ± 3,0 мес� и 55,1 ± 5,7 мес� соответственно, p < 0,001)� К моменту диагностики рецидива туберкулеза у 49,0% (95%-ный ДИ 47,1-77,7) пациентов с ВИЧ-инфекцией уровень клеток CD4 + составлял < 200 кл�/мкл, наиболее часто это снижение было среди пациентов, имевших на момент клинического излечения туберкулеза CD4 + 200-349 кл�/мкл� К моменту диагностики рецидива туберкулеза среди пациентов с ВИЧ-инфекцией доля неэффективной антиретровирусной терапии (АРВТ) увеличилась и составляла 31,7%, а также наметилась тенденция (близкая к статистически значимой) к увеличению доли АРВТ сроком менее 6 мес� При рецидиве туберкулеза у пациентов с ВИЧ-инфекцией чаще бывает распространенный и генерализованный туберкулез, чем у ВИЧ-не-гативных� Наиболее частыми дефектами наблюдения в группе диспансерного учета (III ГДУ) за пациентами, клинически излеченными от туберкулеза, являются снижение частоты рентгенологического и бактериологического обследований и недооценка клинической сим-птоматики� Это приводит к отсроченной диагностике рецидива после появления первых его проявлений, снижению частоты диагностики рецидива при плановом диспансерном обследовании, особенно у больных ВИЧ-инфекцией� Возможно, следует пересмотреть тактику диспансерного фтизиатрического наблюдения для ВИЧ-позитивных пациентов, перенесших туберкулез, в сторону удлинения наблюдения в активных диспансерных группах� В качестве дополнительных критериев, увеличивающих сроки наблюдения в ГДУ, могут рассматриваться следующие: значение CD4 +-клеток/мкл 200 и менее и неэффективная АРВТ, подтвержденная лабораторно� Ключевые слова: рецидив туберкулеза, ВИЧ-инфекция, диспансерное наблюдение Для цитирования: Сенин А� М�, Медвинский И� Д� Особенности рецидивов туберкулеза у пациентов с ВИЧ-инфекцией // Туберкулёз и болезни лёгких�-2018�-Т� 96, № 10�-С� 41-47�
