Цель исследования. Оценить взаимосвязь между показателями углеводного и липидного обмена матери и формированием фетальной макросомии. Материалы и методы. Проведен анализ показателей углеводного обмена (глюкоза плазмы венозной крови натощак, гликированный гемоглобин) и липидного обмена (общий холестерин, триглицериды, липопротеины высокой и низкой плотности (ЛПВП, ЛПНП), соотношение ЛПВП/ЛПНП, лептин, адипонектин, соотношение адипонектин/лептин) у 76 беременных женщин с сахарным диабетом и 113 беременных женщин без нарушений углеводного обмена. Пропорциональность развития макросомных новорожденных определяли с помощью масса-ростового индекса и его центильной оценки относительно срока гестации и пола новорожденного с использованием онлайн-калькулятора INTERGROWTH-21st. Результаты. Уровень гликированного гемоглобина 6,5% и более в III триместре у беременных с сахарным диабетом является высокочувствительным (85,0%) предиктором формирования непропорциональной формы фетальной макросомии. Уровень материнских сывороточных адипокинов является фактором, ассоциированным с метаболическими нарушениями беременной женщины, но не оказывает прямого влияния на формирование фетальной макросомии. Дислипидемия (повышение уровня триглицеридов и соотношения ЛПНП/ЛПВП за счет снижения ЛПВП) в III триместре беременности наряду с нарушениями углеводного обмена повышает риск формирования фетальной макросомии. Заключение. Углубленный контроль биомаркеров углеводного и липидного профиля является важной составляющей первичной профилактики фетальной макросомии. Purpose of the study. To evaluate the relationship between indicators of carbohydrate and lipid metabolism of the mother and the formation of fetal macrosomia. Materials and methods. The analysis of indicators of carbohydrate metabolism (fasting venous blood plasma glucose, glycated hemoglobin) and lipid metabolism (total cholesterol, triglycerides, high and low density lipoproteins, HDL/LDL ratio, leptin, adiponectin, adiponectin/leptin ratio) was carried out in 76 pregnant women with diabetes and 113 pregnant women without carbohydrate metabolism disorders. The proportionality of the development of macrosomal newborns was determined using the mass-height index and its centile assessment relative to the gestational age and sex of the newborn using the INTERGROWTH-21st online calculator. Results. The level of glycated hemoglobin of 6.5% or more in the III trimester in pregnant women with diabetes mellitus is a highly sensitive (85.0%) predictor of the formation of a disproportionate form of fetal macrosomia. The level of maternal serum adipokines is a factor associated with metabolic disorders of a pregnant woman, but does not directly affect the formation of fetal macrosomia. Dyslipidemia (increased triglyceride levels and LDL/HDL ratio due to a decrease in HDL) in the third trimester of pregnancy, along with carbohydrate metabolism disorders, increases the risk of fetal macrosomia. Conclusion. In-depth control of carbohydrate and lipid profile biomarkers is an important component of the primary prevention of fetal macrosomia.
Цель исследования. Оценить точность ультразвуковой фетометрии у беременных с нарушениями углеводного и/или жирового обмена.Материалы и методы. В государственном учреждении «Республиканский научно-практический центр «Мать и дитя» проведено ретроспективное исследование 677 историй родов за 2015–2019 годы. Для характеристики информативности ультразвуковой фетометрии проводили сравнение предполагаемой массы плода по данным ультразвукового исследования, выполненного не более чем за 7 дней до родоразрешения, с массой новорожденного. Рассчитывали абсолютную ошибку в граммах и абсолютную процентную ошибку. Под точностью ультразвуковой фетометрии понимали способность определить предполагаемую массу плода с абсолютной процентной ошибкой до 10%.Результаты. У беременных женщин без нарушений углеводного и жирового обмена с физиологически протекающей беременностью и массой тела новорожденного 3000–3999 г точность определения предполагаемой массы плода при ультразвуковой фетометрии составляет 85,1%. Точность ультразвуковой фетометрии снижается при наличии нарушений углеводного и/или жирового обмена у беременной с тенденцией к занижению предполагаемой массы плода у беременных женщин с сахарным диабетом. При наличии фетальной макросомии точность ультразвуковой фетометрии снижается независимо от наличия или отсутствия нарушений углеводного и/или жирового обмена у беременной. Однако у пациенток с сахарным диабетом при наличии диабетической фетопатии отмечаются наименьшие показатели точности ультразвуковой фетометрии – 44,8%.Выводы. Таким образом, точность ультразвуковой фетометрии снижается при наличии нарушений углеводного и/или жирового обмена, а также при увеличении массы тела новорожденного более 4000 г. В связи с этим является обоснованным поиск дополнительных ультразвуковых маркеров для увеличения точности определения предполагаемой массы плода. Purpose of the study. To evaluate the accuracy of ultrasound fetometry in pregnant women with diabetes mellitus and/or obesity.Materials and methods. In the state institution "Republican Scientific and Practical Center "Mother and Child" a retrospective study of 677 birth histories for 2015-2019 was conducted. To characterize the informative value of ultrasonic fetometry, we compared the estimated fetal weight according to the data of an ultrasound scan performed no more than 7 days before delivery with the weight of the newborn. The absolute error in grams and the absolute percentage error were calculated. The accuracy of ultrasonic fetometry was understood to mean the ability to determine the estimated fetal weight with an absolute percentage error of up to 10%.Results. In pregnant women without diabetes mellitus and obesity with a physiologically occurring pregnancy and a newborn’s body weight of 3000-3999, the accuracy of determining the estimated fetal weight with ultrasound fetometry is 85.1%. The accuracy of ultrasound fetometry is reduced in the presence of diabetes mellitus and obesity in a pregnant woman with a tendency to underestimate the estimated fetal weight in pregnant women with diabetes. In the presence of fetal macrosomia, the accuracy of ultrasound fetometry decreases regardless of the presence or absence of diabetes mellitus and obesity in a pregnant woman. However, in patients with diabetes mellitus in the presence of diabetic fetopathy, the lowest accuracy indicators of ultrasonic fetometry are observed – 44.8%.Conclusion. Thus, the accuracy of ultrasonic fetometry is reduced in the presence of diabetes mellitus and obesity, as well as with an increase in the body weight of the newborn over 4000 g. In this regard, it is reasonable to search for additional ultrasound markers to increase the accuracy of determining the estimated fetal weight.
