РЕЗЮМЕ. У роботі на основі аналізу результатів хірургічного лікування пацієнтів з гострою непрохідністю тонкої кишки (ГНТК) встановлено особливості перебігу даного патологічного стану на фоні защемлення абдомінальних гриж. Мета – покращити безпосередні результати хірургічного лікування хворих на ГНТК, причиною якої є защемлення абдомінальної грижі, шляхом розробки діагностично-лікувального комплексу, спрямованого на запобігання можливим ускладненням. Матеріал і методи. Обстежено 221 хворого на ГНТК, в 10 (4,52 %) з них встановлено защемлення абдомінальної грижі. Ретроспективно проаналізовано 32 медичних карти стаціонарного хворого на це захворювання з несприятливим результатом лікування. Визначали індекс коморбідності (Charlson M. E. et аl., 1987), рівень інтоксикації досліджували за допомогою лейкоцитарного індексу інтоксикації (ЛІІ), за шкалою ASA визначали анестезіологічно-операційний ризик. Результати. Тривалість захворювання у хворих на ГНТК, причиною якої була защемлена грижа, складала в групі з несприятливим перебігом в середньому (3,73±0,36) доби і у пацієнтів, які видужали – (3,56±0,41) доби. У 70,0 % випадків пацієнти всіх вікових груп поступили в хірургічне відділення після 24 годин та в основному це хворі у віці 60 років і більше. На першу добу захворювання було госпіталізовано тільки 30,0 % пацієнтів. Середній час проведення операції в групі цій хворих на ГНТК склав (2,05±0,37) годин, однак, в 46,43 % випадків перевищував 2 години. В 7 (23,33 %) випадках виконана релапаротомія з тривалістю операції 2,15±0,42 годин і у 3 (10,0 %) хворих проведена операція втретє тривалістю 2,06±0,32 годин. У пацієнтів з ГНТК передопераційну підготовку проводили в середньому протягом (40,29±7,28) годин, до 24 годин операція виконана лише у 17 (60,72 %) хворих. У 41,67 % пацієнтів були тяжкі компенсовані захворювання, які становили постійну загрозу для їх життя. У 33,33 % хворих згідно з шкалою ASA анестезіологічно-операційний ризик відповідав V класу, що зумовлювало летальний наслідок у цих пацієнтів. Висновки. Перебіг гострої непрохідності тонкої кишки, причиною якої була защемлена абдомінальна грижа, більш тяжкий і несприятливий, ніж при інших етіологічних причинах.
The use of organic fertilizers creates optimal conditions for obtaining standard planting material. The technology of obtaining organomineral compost via storing bunker method using sawdust, poultry excreta and waste of mushroom production is presented. It was established that at a 60 -65% humidity of organomineral compost, the degree of its readiness is 3-5 months. The use of agricultural and forestry wastes contributes to their more efficient use in forest nursery production. The rational use of non-traditional organic fertilizers in the form of mushroom waste and sawdust will help reduce the burden on the environmental condition. To grow standard forest species seedlings and increase soil fertility, it is necessary to use both pure organic fertilizers and compost as they contain nutrients in plant-accessible form.
According to the literature, fMRI analysis at resting state (RS) is an informative methodological approach to the study of the basic level of a healthy and diseased brain's functional activity. Averaging data over observation groups for various forms of cerebral pathology is often unacceptable. Previously, we mastered and applied the Independent Component Algorithm (ICA) in FSL software to visualize and analyze individual fMRI resting networks of healthy people.Objective: to analyze individual fMRI resting networks associated with the state of motor activity and consciousness in patients with severe traumatic brain injury (STBI).Materials and methods. Observation groups: 23 patients with SТBI (main) and 17 healthy volunteers (control). 3T fMRI recorded at rest. Individual (norm and STBI) and group (norm) analysis of RS networks was carried out by FSL software (ICA algorithm) and SPM8 in MATLAB.For the DMN and Sensorimotor networks, topography and total volume and intensity of their activation of their activation were determined.Results. The topography features reproduced in the group and individual analysis of fMRI of healthy people, as well as the averaged quantitative indicators of the rest networks were used as reference for pathology.In the context of motor activity, the RS Sensorimotor network was considered. Its topography is close to normal in most patients without or with mild hemiparesis. The growth of this defect is accompanied by a decrease in the integral quantitative indicators of the network, combined with asymmetric reduction (lack of activation in the contralateral motor cortex) in rough hemiparesis.In the context of consciousness, the expression and characteristics of the DMN network were compared in healthy people and in patients with STBI at its various levels: from clear to chronic vegetative state. It was revealed that a decrease in the level of consciousness is accompanied by a reduction in the cortical components of DMN, primarily the frontal (anterior DMN), not pronounced in the vegetative state. Activation of the caudal component of DMN (in particular, the posterior cingular cortex) persists in patients with depressed consciousness: distinct and even somewhat enhanced compared to the norm with its reversible form, less pronounced with chronic Conclusion. The data obtained indicate the informative value of fMRI analysis of individual resting networks in the context of studying cerebral structural and functional foundations of consciousness and motor activity, as well as diagnosing the state of these functions in STBI.
