Aim. To estimate the impact of glycemia recorded during myocardial infarction (MI) in-patient care on short-term prognosis of patients without 2nd type diabetes mellitus (2DM).Material and methods. Totally 296 patients were prospectively investigated. According to glucose levels patients were divided into three groups: 1st with ≤4,0 mM/l (7,4%); 2nd with 4,01–7,79 mM/l (69,9%); 3rd with ≥7,8 mM/l (22,6%). The rate of glucose metabolism disorders and complications during in-hospital care were studied.Results. In 2/3 of patients with glycemia ≥7,8 mM/l at hospitalization, later the changes of glucose metabolism were found by glucosetolerance test: prediabetes (36,9%), 2DM (32,3%). In the patients of 3rd group significantly higher was the rate of 3-vessel disease (41,8%) anf MI complications: congestive left-ventricular failure — 52,2% vs 27,3% in the 1st group and 34,1% in the second (p=0,017), cardiogenic shock — 26,9% vs 4,5% and 6,8% (p<0,001), conduction disorders — 27,3% vs 9,1% and 11,7% (p=0,006), in-hospital mortality — 13,8% vs 4,5% and 4,4% (p=0,025). The risk of death in subjects with glycemia ≥7,8 mM/l was 3,48 (95% CI: 1,41–8,60) times higher than in normoglycemic (p=0,007). The glycemia was independently linked with complications of MI during in-hospital period: OR = 1,128; 95% CI: 1,005–1,266 (p=0,042), — as also with the age, severity of myocardial damage and systolic pressure at admittance.Conclusion. There was higher prevalence of MI complications and 3 times higher risk of death in patients without 2DM, but having ≥7,8 mM/l glucose (22,6% of patients) at admittance. The glycemia parameter was an independent predictor for unfavorable prognosis of MI without previous 2DM diagnosis and should be used as part of secondary prevention care.
ЦЕЛЬ: оценить связь между N-концевым пропептидом мозгового натрийуретического пептида (NT-proBNP), эхокардиографическим (ЭхоКГ) показателем давления наполнения левого желудочка (E/é) и окружностью талии (ОТ). МАТЕРИАЛЫ И МЕТОДЫ: использованы данные международного проекта «Узнай свое сердце» (2015–2018 гг.). В данный анализ включены 2352 участника в возрасте 35–69 лет, 41,8% мужчин (N=982). Для оценки систолической и диастолической функции левого желудочка (ЛЖ) были использованы данные трансторакальной ЭхоКГ, проводившейся в одно- и двухмерном допплеровском режимах. Фракция выбро- са (ФВ) ЛЖ определена по методу Симпсон. Давление наполнения ЛЖ оценено по показателю отношения Е/é ср. (скорость раннего наполнения по трансмитральному Допплеру / скорость ранней релаксации по тканевому Допплеру), при этом é ср. = ((é lateral (скорость латеральной части) + é septal (скорость сеп- тальной части))/2. За критерий диастолической дисфункции принят уровень E/é >9. Уровень NT-proBNP >125 пг/мл считался повышенным. Метаболический синдром (МС) устанавливался с использованием крите- риев AHA/NHBLI 2009 г. Использован линейный регрессионный анализ с зависимой переменной NT-proBNP (в ln-трансформированном виде) и независимыми переменными ОТ, E/é, пол и возраст. Результаты: в анали- зируемой выборке 33,1% участников (N=778) имели МС; 14,8 % человек имели абдоминальное ожирение без метаболических нарушений. 90% лиц имели сохраненную ФВ ЛЖ (≥50%). 730 участникам с МС про- ведена оценка давления наполнения ЛЖ и уровня NT-proBNP: 31,6% лиц с МС (N=231) имели умеренное повышение давления наполнения ЛЖ (E/é в диапазоне 9–14) и 1,5% (N=11) – выраженное повышение (E/é ≥15). Среди лиц c МС и умеренным повышением давления наполнения ЛЖ у 52,8% (N=122) было нор- мальное значение NT-proBNP (<125 пг/мл) и у 47,2% (N=109) – повышенное (≥125 пг/мл). По результатам регрессионного анализа на всей выборке независимую положительную связь с уровнем NT-proBNP имели возраст (β = 0,378, р<0,001), пол (β = 0,118, р<0,001), давление наполнения ЛЖ (β = 0,196, р<0,001), отрица- тельное – окружность талии (β = -0,110, р<0,001). ВЫВОДЫ: в популяции жителей Архангельска частота МС, определенного по критериям AHA/NHBLI 2009 г, составила 33%. Повышение давления наполнения ЛЖ, основного показателя диастолической дис- функции у лиц с ожирением, выявлено у 33,1% участников с МС. Половина лиц с МС и показателем E/é в диапазоне 9–14 имели нормальное значение NT-proBNP. В целом в анализируемой выборке уровень NT-proBNP был положительно ассоциирован с давлением наполнения ЛЖ, и отрицательно – с окружностью талии, что необходимо учитывать при выявлении лиц с начальной стадией сердечной недостаточности с сохраненной фракцией выброса и абдоминальным ожирением. Полученные нами данные согласуются с имеющими зарубежными публикациями по проблеме NT-proBNP и ожирения
A total of 153 men with obesity of different degree and 51 men with normal body weight aged 40-60 years were examined in order to detect a relationship between anthropometric parameters and computer tomography data characterizing the accumulation of abdominal fat at different sites. Body weight, height, waist and hips circumferences, and cutaneo-fatty folds were measured. The area of total abdominal fatty tissue, vis- ceral and subcutaneous fat were evaluated by computer tomography; scanning of the abdominal cavity showed images of two sections (at the levels of the 2-3 and 4-5 lumbar verte brae). The results indicate that abdominal obesity in men is characterized by predominant accumulation of fatty tissue in visceral fat depots. The most informative anthropometric markers of the degree of visceral fat accumulation are sagittal diameter and waist circumference. Sagittal diameter 21.1 cm and waist circumference 95.9 cm and more indicate pronounced visceral obesity (93.2 and 88.9% examinees, respectively). These anthropometric parameters are recommended for the diagnosis of visceral obesity in clinical practice.
