Согласно последним данным, представленным Междуна-родной федерацией диабета, 8,5% населения в мире страдают са-харным диабетом 2-го типа (СД-2). При этом в развитии СД-2 ключевая роль отводится взаимодействию факторов окружаю-щей среды и генетически наследуемых компонентов. Выявлены многочисленные модифицируемые факторы риска развития СД-2, наиболее важным из которых является патогенетический вклад висцерального ожирения. В настоящее время в качестве перспек-тивного направления терапии СД-2 рассматривается профилак-тический подход, позволяющий предотвратить и/или замедлить прогрессирование диабетического процесса и его грозных макро-и микрососудистых осложнений.Между тем стремительное развитие молекулярной генетики уже позволяет и в клинической диабетологической практике оце-нить сопряженный с СД-2 генетический риск -в зависимости от носительства однонуклеотидных полиморфизмов (SNP -single nucleotide polymorfism) генов, опосредующих развитие разноо-бразных метаболических нарушений и состояния глюкозолипо-токсичности. The knowledge of genetic determinants can refine our understanding of the pathogenesis of this disease and introduce pharmacogenetic and nutrigenetic approaches to its effective therapy and prevention.
The article considers genetic susceptibility to development of diabetes mellitus type II, its micro-vascular complications and intensity of response under application of incretin therapy depending on carriage of polymorphism of gene TCF7L2 involved into WNT-path and incretin axis.
The purpose of the study was to analyze the association of gene polymorphisms of leptin, adiponectin, tumor necrosis factor alpha (TNF-α), insulin-induced gene, and the parameters of carbohydrate and lipid metabolism, level of C-reactive protein (CRP) in patients with type 2 diabetes mellitus (T2DM) with anthropometric data. A total of 95 patients with T2DM of Azerbaijan population. The average age at the time of the survey was 50,4±0,51 years. Duration of the disease was 5,8±1,5 years. We found that patients with T2DM and obesity in the Azerbaijan population have most common polymorphism of insulin-induced gene in the homozygous form (GG), a mutant version of the gene in heterozygous form of leptin, adiponectin gene mutant variant in homozygous form and the normal version of the gene TNF-α in homozygous form. In 47.3% of cases there was a high level of CRP, a linear body mass index with normal homozygous GG allele of the TNF-a gene , insulin-induced gene, and a mutant version of the adiponectin and leptin genes.
