Введение. Аугментационная уретропластика с использованием буккальной слизистой в настоящее время является оптимальным методом лечения протяженных стриктур уретры. Тем не менее, данная хирургия сопряжена с довольно высокой частотой рецидивов и осложнений. Одной из причин, что может определять успех или негативных исход операции является морфологическая структура трансплантата, что требует более углубленных исследований. Цель исследования. Оценить исходное морфологическое состояние буккальной слизистой у мужчин и определить целесообразность ее фармакотерапевтической подготовки, как предиктора улучшения качества приживления аутотрансплантата при аугментационной уретропластике. Материалы и методы. У 20 пациентов с протяженными стриктурами спонгиозной уретры изучена морфологическая структура фрагментов буккальной слизистой. Объектами морфологического исследования являлись: интактная слизистая оболочка полости рта перед аугментационной уретропластики (1 биопсия) и фрагменты слизистой после 7 дневной обработки полости рта раствором антисептика «Листерин» (2 биопсия). Биопсийный материал исследовали микроскопически при стандартных окрасках (гематоксилин-эозин) и иммуногистохимическими методиками. Результаты. Исходная гистоструктура буккальной слизистой у каждого третьего больного сопровождалась воспалительной инфильтрацией на уровне субэпителиального слоя с десквамацией эпителия. после недельной обработки полости рта антисептиком гистологически и иммуногистохимически доказано выраженное увеличение и усиление межклеточных контактов в трансплантатах, характеризующееся выраженной экспрессией антитела во всей толщине эпителиального пласта. при этом происходит «самоорганизация» подслизистой основы за счет снижения отёка, усиления межклеточных и фокальных адгезионных контактов, с усилением компонентов сосудистого русла и количеством сосудистых «почек». Выводы. Таким образом, недельная предоперационная обработка полости рта позволяет купировать повреждения покровного эпителия и воспалительную инфильтрацию в подслизистой основе с улучшением ее васкуляризации, что может быть расценено как положительный фактор для последующего приживления трансплантата. Единственным радикальным методом лечения стриктурной болезни уретры у мужчин являются резекция уретры или уретропластика. Но до настоящего времени не в полной мере выработаны стандарты по реабилитации больных перенесших данную операцию. В первую очередь, это относится к восстановлению эректильной функции и купированию психоэмоциональных расстройств, которые всегда сопровождают эту болезнь, особенно у мужчин молодого возраста.Цель исследования. Изучить распространённость тревоги и депрессии на фоне симптомов нижних мочевых путей (СНМП) и эректильные расстройства при стриктурной болезни до и после хирургического лечения.Материалы и методы. Проведен клинико-статистический анализ хирургии 70 мужчин с различной этиологией стриктурной болезни уретры, а также их исходного андрогенного статуса. Определена динамика качества жизни у больных по симптомам СНМП, сексуальным расстройствам и эректильным дисфунциям (ЭД), в сочетании с психоэмоциональным статусом по шкалам тревоги и депрессии (HADS 1, 2).Результаты. Исходный андрогенный статус пациентов оказывает влияние на риски возникновения раневых осложнений в раннем послеоперационном периоде. Успешное хирургическое лечение стриктур уретры (СУ) характеризуется быстрым и достоверным купированием СНМП и меньшим купированием тяжести сексуальных расстройств и ЭД.Заключение. Совокупность оценок СНМП, ЭД и тревоги/депрессии в среднесрочном и отдаленном периоде после успешной хирургии СУ следует рассматривать в совокупности. Introduction. Currently, the most common method of treating extended urethral strictures is augmentation urethroplasty using oral mucosa grafts. Analysis of the long-term outcomes of this surgery type shows a high incidence of relapses and complications.Purpose of the study. To improve the outcomes of augmentation urethroplasty, in particular the dorsal inlay (Asopa) technique, in patients with extended spongy urethral strictures by minimizing the risk of recurrent strictures.Materials and methods. The study is based on an analysis of the surgery in 90 patients (aged 18-72 years) with extended spongy urethral strictures. Seventy patients (group I) underwent dorsal inlay augmentation urethroplasty according to the Asopa technique, and 20 patients (group II) — according to the author's modified technique. Statistical data analysis was carried out using the SPSS ver.26 software (SPSS Inc. Chicago, IL, USA).Results. A comparative analysis of the course of the early postoperative period showed a lower number of complications in group II patients compared to group I — 20.0% versus 34.3%, respectively. The recurrent strictures were registered for groups I and II in 18.8% and 5.6% of cases 6 months after surgery, respectively. The recurrent urethral narrowing was most often localized in the area of distal anastomosis between the buccal graft and the native urethra in patients from both groups.Conclusion. The modified dorsal inlay augmentation urethroplasty technique developed and implemented in clinical practice by increasing the internal urethral lumen in the areas of proximal and distal anastomosis between the buccal graft and the native spongy urethral body allows minimizing the risks of recurrent urethral narrowing after augmentation urethroplasty.
