Primary hyperparathyroidism (PHT) is the third most common endocrine disorder in men and women after diabetes mellitus and thyroid disease and one of the most frequent causes of osteoporosis and fractures among the secondary osteopathy. PHT refers to socially significant problems in connection with involvement in the pathological process of the majority of organs and system In recent years, the conception of epidemiology, clinic and tactics of management of patients with PHT has changed due to significant increase of morbidity at the expense of identification of mild forms of disease. The modern management concept tactics depends on clinical manifestations. Bearing in mind the importance of the problem under consideration, the working group was set up for the development of federal recommendations on the treatment of PHT based on the principles of evidence-based medicine. The experience accumulated by the domestic and international experts was summarized in the federal clinical guidelines on PHT.
The article provides an overview of the current literature integrating clinical data on the role of androgen deficiency in pathogenesis of metabolic malfunctions and diabetes mellitus. The combination of androgen deficiency and diabetes mellitus is a risk factor of cardiovascular diseases. Due to the fact that general physicians, endocrinologist dont have knowledge of this problem most of androgen deficiency cases are not only remained without treatment but also not revealed.
In this review are discussed experimental and clinical data about the role of gut microbiota and its changes associated with age and lifestyle. The large intestinal microbiota plays an important role in normal bowel function and the maintenance of host health through the formation of short chain fatty acids, modulation of immune system reactivity, and development of colonization resistance. The intestinal microflora is a peculiar indicator of the condition of a microorganism reacting to age, physiological, dietary, and geographical factors from change of qualitative and quantitative structure. Studies have demonstrated that obesity and metabolic syndrome may be associated with profound microbiotal changes. Changes in gut microbiota control metabolic endotoxemia - induced chronic inflammation, oxidative stress, and metabolic disorder which are connected with the increased risk of development of cardiovascular diseases and pathology associated with age, which leads to accelerated aging. It is obvious that maintenance of a homeostasis and a normal metabolism is impossible without restoration of a variety of normal associations of intestinal microorganisms.
При хронической болезни почек уменьшение количества действующих нефронов вызывает каскад осложнений, включая дефицит витамина D, гиперфосфатемию, изменение функции кальций-чувствительного рецептора, увеличе-ние синтеза паратиреоидного гормона и гипертрофию с гиперплазией клеток околощитовидных желез. На началь-ных стадиях хронической болезни почек основным патогенетическим звеном минерально-костных нарушений явля-ется снижение концентрации кальцитриола. По основным клиническим рекомендациям цели лечения вторичного гиперпаратиреоза у пациентов с хронической болезнью почек направлены на предотвращение прогрессирования заболевания и подавление активности околощитовидных желез с помощью модуляции рецепторов к витамину D. Основные эффекты кальцитриола осуществляются посредством взаимодействия c рецептором витамина D, обладаю-щим высокой аффинностью и специфичностью. Однако, наряду с неоспоримыми положительными эффектами, каль-цитриол повышает сывороточные концентрации кальция и фосфора. Парикальцитол, в отличие от неселективных активных метаболитов витамина D, реже приводит к гиперфосфатемии и гиперкальциемии. Применение парикаль-цитола ассоциировано также с ингибированием синтеза ренина, снижением протеинурии, артериального давления, что является важным положительным эффектом, особенно у пациентов с сахарным диабетом (CД).В статье представлен результат собственного наблюдения по коррекции вторичного гиперпаратиреоза у пациента с СД 2-го типа (СД2) и хронической болезнью почек 3-4 стадии. Обсуждаются эффекты применения селективного активатора рецепторов витамина D -парикальцитола на примере клинического случая. Продемонстрирована ком-пенсация вторичного гиперпаратиреоза без риска развития гиперкальциемии и гиперфосфатемии, с эффективным дополнительным нефропротективным действием препарата. Secondary hyperparathyroidism is an early complication of chronic kidney disease, with increasing severity as the glomerular filtration rate decreases and characterized by a progressive increase in parathyroid hormone and growth of the parathyroid glands. It is generally accepted that a deficiency in active form of vitamin D or calcitriol levels seems to play a relevant role in its development and progression of secondary hyperparathyroidism. A reduction in plasma calcitriol levels occurs early in renal disease. Major renal guidelines recommend use of vitamin D for secondary hyperparathyroidism in chronic kidney disease. In the treatment vitamin D receptor activation inhibit glandular hyperplasia; reduce parathyroid hormone levels impact on bone turnover and mineral density. Treatment with calcitriol can occasionally result in hypercalcemia and hyperphosphatemia in renal patients due promotes intestinal calcium and phosphorus absorption. This limits its suitability for the treatment. В статье представлен обзор современной литературы, обобщающий экспериментальные и клинические данные о связи ожирения с изменениями состава микробиоты кишечника, а также о влиянии бариатрических операций на массу тела и качественный состав микробиоты. Доказано, что микробиота кишечника является своеобразным индикатором состояния макроорганизма, реагируя на возрастные, физиологические, диетические, климатогеографические факторы изменением своего качественного и количественного состава. Эти изменения приводят к повышению ее метаболической активности с образованием дополнительной энергии от съеденной пищи, способствуя усвоению питательных веществ и удлинению времени хранения в жировой ткани через разнообразные механизмы. Обнаружено, что микробиота кишечника влияет на энергобаланс не только за счет эффективного извлечения энергии из питательных веществ, но и за счет воздействия на гены, регулирующие ее расход и запасание. При всей сложности вопроса о причинно-следственной связи изменений состава микробиоты и ожирения, совокупность полученных данных обосновывает положение, что качественные и количественные изменения микробиоты кишечника могут способствовать развитию ожирения. Обнаружено, что индуцированные бариатрическими операциями метаболические и микробные модификации могут способствовать снижению веса, улучшению метаболических процессов и, как следствие, увеличению здорового периода и продолжительности жизни. Остеопороз и остеопатии / Osteoporosis and Bone Diseases | 102 CASE REPORT АКТУАЛЬНОСТЬПод термином «гастроэзофагельная рефлюксная бо-лезнь» (ГЭРБ) подразумевают развитие характерных сим-птомов и (или) воспалительное поражение дистальной части пищевода вследствие повторяющегося заброса в пищевод желудочного и (или) дуоденального содержи-мого [1]. Тенденция к увеличению заболеваемости ГЭРБ явилась основанием провозгласить на 6-й Европейской гастроэнтерологической неделе лозунг «XX век -век язвенной болезни, XXI -век ГЭРБ» [2]. Проведенный в Рос-сии ретроспективный анализ первичных заключений Гастроэзофагельная рефлюксная болезнь (ГЭРБ) -воспалительное поражение дистальной части пищевода вследствие повторяющегося заброса в пищевод желудочного и (или) дуоденального содержимого. В настоящее время наблюдает-ся тенденция к увеличению заболеваемости ГЭРБ в мире, особенно в развивающихся странах. Лечение ГЭРБ включа-ет рекомендации по изменению образа жизни пациента и медикаментозную терапию -первоначально ингибиторов протонной помпы (ИПП). Длительное применение ИПП связано с различными осложнениями, такими как дефицит железа, гипомагниемия, гипокальциемия, гипокалиемия, остеопороз и переломы костей, хронические заболевания почек, острая почечная недостаточность, пневмония и так далее. В литературе все чаще появляются описания клини-ческих случаев развития гипомагниемии. В феврале 2011 г. управление по санитарному надзору за качеством пище-вых продуктов и медикаментов FDA (Food and Drug Administration) опубликовало сообщение о новом нежелательном явлении при длительном приеме ИПП -гипомагниемии. В статье представлен результат собственного наблюдения 56-летнего пациента, обратившегося за консультацией с жалобами на судороги, тремор, боли в мышцах, нарушение ритма сердечной деятельности, общую слабость, вызванные гипомагниемией, гипокальциемией и гипокалиемией при низком уровне паратиреоидного гормона на фоне длительного применения ИПП по поводу ГЭРБ. На фоне отме-ны ИПП и назначения препаратов магния отмечено улучшение состояния, восстановление электролитных нарушений и функции околощитовидных желез. Причинно-следственная связь электролитных нарушений с ИПП подтверждена при повторном назначении ИПП и возобновлении гипомагниемии. Background: secondary hyperparathyroidism (SHPT) is an early complication of chronic kidney disease (CKD). Maintaining the level of 25(OH)D and parathyroid hormone concentrations in the target range reduce its associated complications (fractures and cardiovascular calcification). Aims: to examine the effectiveness of vitamin D supplementation and selective vitamin D receptor agonists treatment on SHPT in CKD. Material and methods: prospective observational study to evaluate the efficacy and safety of vitamin D therapy SHPT in 54 in patients with CKD. The first phase (24 weeks) – treatment of suboptimal 25-hydroxycalciferol (25(OH)D) levels. The second (16 weeks) – treatment colecalciferol-resistant SHPT by combination of cholecalciferol with paricalcitol. Blood samples were taken to assess parathyroid hormone (PTH), 25(OH)D, creatinine, calcium, phosphorus levels and calcium excretion. Results: After 8 weeks of cholecalciferol treatment all patients achieved 25(OH)D levels above 20 ng/ml, however 78% of patients still had SHPT. After 16 weeks, the decrease of PTH was achieved in all patients, but significantly only in patients with CKD 2 (19.2%, p< 0.01) and 3 (31%, p <0.05), compared with CKD 4 (17%, p >0.05). After 24 weeks of therapy, PTH normalized in all patients with CKD 2, in 15 (79%) with CKD 3 and in 9 (50%) patients with CKD 4. Cholecalciferol treatment resulted in a substantial increase in 25(OH)D levels with minimal or no impact on calcium, phosphorus levels and kidney function. After 24 weeks we initiated combination therapy (cholecalciferol and paricalcitol) for patients with colecalciferol-resistant SHPT (n=13). PTH levels decreased from 149.1±13.4 to 118.2±14.1 pg/ml at 8 weeks, and to 93.1±9.7 pg/ml (p <0.05) at 16 weeks of treatment. No significant differences in serum calcium, phosphorus or urinary calcium levels. Normalization of PTH was achieved in all patients with CKD 3 and in 8 patients with stage 4. One patient with CKD 4 needed an increase in paricalcitol dose. Conclusion: Cholecalciferol can be used in correcting vitamin D deficiency in patients with all stages of CKD, however, its effectiveness in reducing PTH in stage 4 is limited. Selective analogs, such as paricalcitol, were well-tolerated and effectively decreased PTH levels.
