Представляем результаты собственного наблюдательного когортного исследования пациентов с дисциркуляторной энцефалопатией I-II стадии, получавших терапию комбинацией дигидроэргокриптина и кофеина (Вазобрал®). Интерес к этому вопросу обусловлен тем, что цереброспинальная венозная недостаточность играет роль в патогенезе нейродегенеративных и сосудистых заболеваний головного мозга, приводя к развитию когнитивных нарушений, формированию вторичных головных болей, снижая качество жизни пациентов. Цель исследования -оценка эффективности и безопасности препарата Вазобрал® у больных с дисциркуляторной энцефалопатией I-II стадии, обусловленной венозной дисциркуляцией на фоне хронической цереброспинальной венозной недостаточности. Материал и методы исследования: 102 амбулаторных пациента (средний возраст 63,5 ± 3,74 года, 25 (25,5%) мужчин и 77 (74,5%) женщин), страдающих хронической ишемией мозга и имеющих признаки хронической цереброспинальной венозной недостаточности. Дисциркуляторная энцефалопатия I стадии с легкими когнитивными нарушениями диагностирована у 58 (59,2%) пациентов, у 44 (40,8%) человек установлена дисциркуляторная энцефалопатия II стадии с умеренными когнитивными расстройствами. Всем наблюдаемым проводилось дуплексное сканирование ветвей дуги аорты, яремных и позвоночных вен с измерением линейной скорости кровотока; оценивалась интенсивность головных болей, когнитивного статуса, выраженность аффективного синдрома с помощью специальных опросников. В течение 3 мес. пациенты принимали Вазобрал® 8/80 мг/сут. Полученные данные были проанализированы с помощью компьютерных программ SPSS и Statistica. Достоверность различий -при ДИ 95% p<0,05. Результаты. Отмечена достоверная положительная динамика состояния больных на фоне приема Вазобрала®: интенсивность головной боли уменьшилась с 4,11 до 0,67 балла, оценка когнитивных функций увеличилась на 14% от исходного уровня, выраженность тревожно-депрессивного симптомокомплекса достоверно уменьшилась на 38-56%. Заключение. Эпилепсия относится к числу распространенных заболеваний, занимая в структуре неврологической патологии третье место, и является значимой медико-социальной проблемой. Лечение эпилепсии основано на подборе индивидуальной терапевтической дозы про-тивоэпилептических препаратов (ПЭП). Современные антиконвульсанты даже в среднетерапевтических дозах обладают рядом побочных эффектов, которые снижают качество жизни (КЖ) пациентов с эпилепсией.Исследование качества жизни все шире использует-ся в клинической медицине, так как позволяет, с одной стороны, глубже изучить влияние заболевания на раз-личные составляющие здоровья пациента, с другой -выявить дополнительные преимущества или недостат-ки проводимой терапии.По-прежнему актуальной остается проблема по-вышения качества жизни у пациентов с эпилепсией.Это объясняется тем, что эпилепсия является одним из наиболее значимых в медико-психологическом и со-циальном плане заболеванием. В мире насчитывает-ся около 50 млн больных эпилепсией, 5 % населения переносят не менее одного припадка в течение жизни, у 20-30 % пациентов заболевание является пожизнен-ным [1, 5,14]. По данным Всемирной организации здравоохранения (ВОЗ), более 75 %, а согласно рос-сийскому анализу -около 63 % зарегистрированных пациентов не получают рациональную противоэпи-лептическую терапию. Зарегистрирован более высокий коэффициент смертности лиц с данным заболеванием, чем в популяции в целом [3, 5].Эпилепсия -заболевание, требующее длительной, многолетней терапии, что имеет принципиальное зна-чение для здоровья и КЖ больного [8, 9, 22].
