A rapidly advancing epidemic of HIV-1 infection has been documented among injecting drug users (IDUs) in Russia. The Northwestern Federal District was the first of the seven Russian Federal Districts involved in a drug-related HIV epidemic through an outbreak in Kaliningrad in 1996. The Northwestern Federal District has a high HIV prevalence rate having reached 252 per 100,000 by the end of 2003, exceeding the Russian average (180) by 1.4 times. The epidemic peaked in 2001. Since then the annual number of new cases has decreased, probably reflecting saturation among at least some IDU populations. However, at the same time, the heterosexual spread of HIV has become more prominent. To study the genetic epidemiology of HIV-1, samples were collected from 150 individuals covering a wide geographical area and different transmission groups in the Northwestern Federal District. Phylogenetic analysis revealed that an Eastern European subtype A HIV-1 strain similar to those reported earlier among IDUs in other regions of Russia accounted for 80% of HIV-1 infections and was the predominant subtype in six out of seven administrative territories studied both among IDUs and heterosexually infected persons. As an exception to the dominant role of the Eastern European subtype A strain, the CRF03-AB strain was found to be dominant in the city of Cherepovets located in the north central European Russian territory of Vologda Oblast. This is the first report of the CRF03-AB strain causing an outbreak outside the Kaliningrad region.
Aim. To study the advantages of ultrasound versus anthropometric assessment of muscle mass for early diagnosis of sarcopenia in patients >45 years of age.Material and methods. The study included 79 patients aged 4589 years with coronary artery disease (CAD), hypertension, type 2 diabetes, heart failure, class 1-3 obesity. Diagnosis of hypertension was carried out according to the European (2018) and Russian (2019) guidelines; CAD — according to Russian Society of Cardiology (2020) guidelines. Sarcopenia was diagnosed according to 2010 European Working Group on Sarcopenia in Older People (EWGSOP) criteria and 2018 EWGSOP2 guidelines. Muscle mass (MM) was determined by two methods: 1 — by measuring the rectus abdominis muscle (RAM) thickness using the ultrasound; 2 — by measuring the arm and lower leg circumference. Muscle strength was determined by wrist dynamometer. Muscle function was assessed using the 4-m gait speed test.Results. The study showed that in patients aged >45 years admitted to the emergency cardiology department, the body mass index exceeded 25 kg/m2 in 88,6% of cases. The incidence of sarcopenia of varying severity was 55,7% (n=44). The differences in RAM thickness, arm and lower leg circumference between the groups of patients with/without sarcopenia were significant (p<0,001), but were less than the threshold only for RAM thickness. RAM thickness levels progressively decreased with increasing severity of sarcopenia and significantly differed at all stages compared to patients without sarcopenia (p<0,001), regardless of body mass index. A decrease in lower leg circumference below the threshold values determining a MM decrease was observed only in severe sarcopenia, and arm circumference — in both men and women only in severe sarcopenia. There were no significant differences for arm and lower leg circumference depending on sarcopenia stages in overweight and obese patients.Conclusion. MM assessment by measuring RAM thickness with ultrasound in comparison with the anthropometric method makes it possible to diagnose sarcopenia in patients >45 years of age with cardiovascular diseases and obesity at earlier stages and to promptly recommend preventive measures.

Изучить особенности частоты распространения различных факторов риска (ФР) сердечно-сосудистых заболеваний (ССЗ) у долгожителей Смоленской области. Материал и методы. Проведено анкетирование 112 пациентов мужского (n=58) и женского (n=54) пола в возрасте ≥90 лет, находившихся на стационарном лечении в Смоленском областном клиническом госпитале для Ветеранов Войн в период 2013-2015гг. Анкета включала следующие блоки (модули): паспортную часть, антропометрические показатели, модуль вредных привычек, семейный анамнез, клиническое и лабораторное исследования. Всем пациентам ежедневно измеряли уровень артериального давления (АД). Критериями артериальной гипертонии (АГ) считали уровень АД >140/90 мм рт.ст. или постоянный прием антигипертензивных препаратов. Всем пациентам выполнено исследование общего холестерина, триглицеридов, глюкозы, фибриногена, С-реактивного белка, общего билирубина, креатинина, мочевины в сыворотке крови (анализатор STAT FAX "USA"). У 30 долгожителей исследована жесткость сосудов мышечно-эластического типа с помощью определения скорости распространения пульсовой волны. Результаты. Продемонстрированы низкая частота у долгожителей курения (2,7%) и потребления алкоголя (16,9%), развитие АГ в позднем онтогенезе с преимущественным поражением периферических сосудов нижних конечностей. Для долгожителей Смоленского региона характерно наличие таких положительных психосоциальных факторов как высокий образовательный уровень и сохраненная способность к самообслуживанию. 34,6-41,9% опрошенных указали на наличие родственников-долгожителей 1 степени родства. Подавляющее большинство обследованных (64,2%) имели достаточную физическую активность, у них отсутствовали дислипидемия и метаболические нарушения. Заключение. Сочетание минимального количества ФР ССЗ с хорошей наследственностью и психосоциальными установками на развитие и образование в совокупности способствует достижению активного долголетия. Материал и методы. Проведено анкетирование 112 пациентов мужского (n=58) и женского (n=54) пола в возрасте ≥90 лет, находившихся на стационарном лечении и реабилитации в Смоленском областном клиническом госпитале для Ветеранов Войн в период 2013-2015гг. Изучали: перенесенные заболевания, индекс полиморбидности. Всем пациентам ежедневно измеряли уровень артериального давления. Критериями артериальной гипертонии считали величину артериального давления >140/90 мм рт.ст. или постоянный прием антигипертензивных препаратов. Всем пациентам делали общий анализ крови, проводились электрокардиография в стандартных отведениях, холтеровское мониторирование электрокардиограммы по показаниям, а также определяли общий холестерин, триглицериды, глюкозу, аланинаминотрансферазу, аспартатаминотрансферазу, щелочную фосфатазу, общий белок, билирубин, креатинин, мочевину в сыворотке крови (анализатор STAT FAX "USA"). Результаты. Результаты исследования продемонстрировали: выход на инвалидность в возрасте >60 лет, низкий индекс полиморбидности (<4 заболеваний на пациента) с преобладанием смешанного варианта полиморбидности и преимущественным поражением сердечно-сосудистой системы: артериальная гипертония диагностирована у 71,4%, ишемическая болезнь сердца у 98,9%, хроническая сердечная недостаточность у 87,5% обследованных, постинфарктный кардиосклероз у 16,7%, фибрилляция предсердий у 14,3% обследованных, долгожителей-ветеранов Великой Отечественной войны Смоленской области. Отмечена низкая частота распространения онкопатологии (2,7%), представленной преимущественно поражением кожи и ее придатков; эндокринологических заболеваний (2,7%), заболеваний системы крови (<1,8%), анамнестически отсутствовали психические заболевания. Медикаментозная терапия была представлена в основном антигипертензивными (их употребляли 72% опрошенных), сосудистыми (69,7%) препаратами, дезагрегантами и антикоагулянтами (67,6%). 27,7% опрошенных употребляли антибиотики, 38,4% -нестероидные противовоспалительные средства, 41,9% -диуретики. Заключение. Полиморбидный "фон" долгожителей-ветеранов Великой Отечественной войны Смоленской области представлен преимущественно заболеваниями сердечно-сосудистой системы с поздним выходом на инвалидность. Отмечается высокая приверженность антигипертензивной терапии, приему дезагрегантов и диуретиков. Ключевые слова: полиморбидность, заболеваемость, долгожительство.Конфликт интересов: не заявлен.
Аim. To study the clinical course of chronic gastritis with erosions in the elderly, depending on the presence of Helicobacter pylori in the gastric mucosa.Materials and methods. 75 patients (50 women and 25 men) aged 60 to 82 years with chronic erosive gastritis were examined. Тhey complained of pain in the epigastric region, pain of other localization, heaviness in the epigastric region, nausea, heartburn, constipation, having comorbidities. Patients were divided into 4 groups depending on the degree of contamination of the gastric mucosa with Helicobacter pylori: 1 group consisted of 20 people, which on the background of erosive lesions of the coolant, HP was not detected; in group 2 (25 man), it was observed a light contamination with coolant; in group 3 (20 patients) and in group 4 (10 patients) — high — were detected in more than 50 microbial cells in field of view.Result. The clinical picture of chronic erosive gastritis varies depending on the degree of contamination of the gastric mucosa Helicobacter pylori.Conclusion. This microorganism aggravates the course of the disease. The course of chronic erosive gastritis is influenced by concomitant pathology.
