ель исследования. Установить показания к применению контрлатерального контрастирования при антеградной реканализации хронических окклюзий коронарных артерий. Материалы и методы. В исследование были включены 128 пациентов с хро-ническими окклюзиями коронарных артерий, которым выполнялась антеградная река-нализация. Были проанализированы варианты коллатерального кровотока в посток-клюзионных сегментах за счѐт ипси-и контрлатерального контрастирования. Оценива-лось влияние визуализации постокклюзионного сегмента на результат антеградной ре-канализации.Результаты. Предложенная классификация коллатерального кровотока RAI поз-воляет более точно охарактеризовать варианты контрастирования постокклюзионного сегмента, так как в отличие от существующих классификаций содержит информацию о визуализации дистальной культи. Была установлена взаимосвязь эффективности анте-градной реканализации от визуализации типов коллатерального кровотока RAI-Ia, RAI-III, критериями которых является контрастирование дистальной культи. Также было отмечено более частое возникновение осложнений при определении типов коллатераль-ного кровотока RAI-0, RAI-Ib, RAI-II. Использование контрлатерального контрастирова-ния потребовалось при выполнении антеградной реканализации у 27% больных и было связано с недостаточным контрастированием постокклюзионного сегмента за счѐт ип-силатерального введения контрастного вещества. Обратило на себя внимание, что опытные операторы достоверно чаще использовали контрлатеральное контрастирова-ние при визуализации менее ½ постокклюзионного сегмента, а также при отсутствии визуализации дистальной культи.Выводы. Успех антеградной реканализации хронических окклюзий коронарных артерий достоверно чаще был достигнут при визуализации типов коллатерального кро-вотока RAI-Ia, RAI-III.
