Цель исследования-изучение в сравнительном аспекте параметров качества жизни пациентов после пластики паховых грыж с применением эндоскопических методов. Материал и методы. В работе проанализированы параметры качества жизни, полученные с помощью русской версии международного опросника EuroQol Index (EQ-5D) и визуальной аналоговой шкалы болевого синдрома у 41 пациента (мужчины) после лечения первичных односторонних неосложненных паховых грыж в хирургическом отделении Сургутской городской клинической больницы способами трансабдоминальной предбрюшинной герниопластики (ТАПП) (25 пациентов) и тотальной экстраперитонеальной герниопластики (ТЭП) (16 пациентов). Оценку проводили в ранний (1-е, 3-и сутки) период и через 12 мес после герниопластики. Результаты. Анализ результатов лечения 41 пациента с паховыми грыжами методами ТАПП и ТЭП показал незначительность, немногочисленность осложнений и отсутствие рецидивов заболевания после проведенных герниопластик. В то же время в раннем послеоперационном периоде (1-й, 3-й день после операции) разной выраженности проблемы со здоровьем, оцениваемым по шкалам опросника EQ-5D, имели все пациенты. Наличие боли в первый послеоперационный день сопровождалось ограничением подвижности, самообслуживания, невозможностью выполнения повседневных дел, выражалось в низкой оценке состояния своего здоровья в этот период. Отмечаемый болевой синдром у прооперированных методом ТАПП был более значительным. Уже к 3-м суткам интенсивность боли снижалась и регистрировалась соответственно как умеренная и легкая. EQ-5Dиндекс, являющийся интегральным показателем опросника у пациентов, перенесших ТАПП, также были ниже, чем после ТЭП, отражая, таким образом, более высокую субъективную оценку своего качества жизни в этот период у последних (значения медиан, верхних и нижних квартилей для показателей качества жизни пациентов после герниопластики методами ТАПП и ТЭП соответственно 0,37 (0,3; 0,42) и 0,46 (0,35; 0,6)). Определяемые через 12 мес показатели свидетельствовали о повышении качества жизни и отсутствии различий их величин у прооперированных методами ТАПП и ТЭП (0,52 (0,4; 0,58) и 0,58 (0,45; 0,65) соответственно). Заключение. Ограничения подвижности, самообслуживания, выполнения повседневных дел, тревога, боль и дискомфорт в области вмешательства отразились в низкой оценке состояния своего здоровья пациентами по шкалам опросника EQ-5D прежде всего в ранний послеоперационный период. Сравнительная оценка показала, на этом этапе более позитивный характер компонентов, отражающих качество жизни и интенсивность болевого синдрома, имеют пациенты после ТЭП. К 12-му месяцу после операции определяемые различия нивелируются и оптимизируются.
Research is devoted to topical issue of vascular surgery – to treatment of patients with multilevel arterial occlusions of the lower extremities in a stage of critical ischemia. 80 patients with a combination of critical ischemia and coronary heart disease were randomized in 2 groups. The concept of the combined treatment of patients with multilevel defeat of the arterial course in combination with the coronary heart disease combining complex conservative therapy with perioperation application of a vazaprostan and sulodexide and multicomponent reconstructive (including endovascular) operations, is applied at 40 patients with critical ische-mia. Improvement of diastolic function of the left ventricle against reduction of heart rate is noted. In the first three months after the beginning of reception of a vazaprostan and a sulodexide improvement of a coronary blood-groove in the first for 23% was observed. Total number of attacks of ischemia per day in the main group decreased by the end of 3 months by 68% in the main group. Recovery of normal geometry of heart in group of the patients receiving vazaprostan and sulodexide is revealed at 56% of patients. In 3 years after operation the frequency of preservation of an extremity at them was 30% higher, than at patients to whom therapy wasn´t carried out by these drugs. Use in strategy of complex conservative therapy of a vazaprostan and sulodeksid allows to reduce at them the frequency of amputations and to improve quality of life due to stabilization of a course of coronary heart disease.
