В статье представлен нарративный несистематический обзор литературы на тему причин и структуры преждевременной смертности среди психически больных. Смертность среди людей с психическими заболеваниями стабильно выше, чем в среднем по популяции. Исследования указывают на более высокие риски смертности у данной категории пациентов, чем у населения в целом. Сейчас наибольшее число случаев смерти связаны с сердечно-сосудистыми и респираторными заболеваниями. С увеличением ожидаемой продолжительности жизни среди населения в целом разница в показателях смертности среди людей с психическими заболеваниями и в общей популяции будет увеличиваться. Без разработки альтернативных подходов к укреплению физического здоровья людей с психическими заболеваниями и к их лечению повышенная смертность среди психически больных будет сохраняться. Цель исследования. Изучить и обобщить данные о причинах и показателях смертности пациентов, имеющих психические заболевания, от основных групп соматических заболеваний. Материалы и методы. Проведен несистематический поиск иностранных источников в базе данных PubMed. Поиск статей осуществлен по ключевым словам: «mental disorders», «mortality rate», «mortality causes of death», «mortality statistics». Включались полнотекстовые статьи, находящиеся в открытом доступе, либо без открытого доступа при наличии абстракта. The article provides a narrative, nonsystematic review of the literature on the causes and structure of premature mortality among the psychiatric population. Mortality rate among people with mental illness is consistently higher than in the population average Research indicates higher mortality risks in these patients if compared to general population. Currently, the largest numbers of mortalities are related to cardiovascular and respiratory diseases. As life expectancy in the general population increases, the difference in mortality rates between individuals with mental illness and the general population is going to widen. Unless alternative approaches to improving physical health and treatment of individuals with mental illnesses are elaborated, the increased mortality rate among psychiatric populations will persist. Purpose of the study. To study and summarize data on reasons and mortality rates in patients with mental illnesses died due to the main groups of somatic diseases. Materials and methods. A non-systematic search for foreign sources in the database PubMed was performed. Articles were searched using such keywords as: "mental disorders," "mortality rate," "mortality causes of death," and "mortality statistics". Full-text articles were included either available in free access or without free access if abstracts were present.
В настоящее время самоповреждающее поведение становится все более распространенным среди подростков, все больше привлекает внимание со стороны исследователей, работников службы здравоохранения в области психического здоровья и всего общества в целом, что подчеркивает актуальность изучения этой проблемы. Цель. На основе рассмотрения реального клинического случая провести анализ результатов экспериментально-психологического исследования девочки-подростка с акцентуацией характера и самоповреждающим поведением. Материалы и методы. Для подготовки оригинального исследования несуицидального самоповреждающего поведения был отобран данный клинический случай. Анализ проводился в нескольких направлениях: клиническом и анамнестическом на основе выписки из истории болезни, а также специально разработанной для исследования статистической карты. Для патопсихологического исследования использовался расширенный набор психодиагностических методик и шкала для исследования несуицидального самоповреждения. Результаты. В статье описывается клинический случай исследования подростка 14 лет с диагнозом F92.8 – Смешанное расстройство поведения и эмоций, сопровождающееся акцентуацией характера и самоповреждающим поведением. Представлены результаты исследования, включая основные этапы и клинический поиск при диагностике, а также дизайн проводимого исследования. Особое внимание уделяется триггерным факторам и клиническим характеристикам, предрасполагающим к развитию несуицидального самоповреждающего поведения. Currently, self-injurious behavior is becoming more common among adolescents, attracting more and more attention from researchers, mental health professionals and society as a whole, which emphasizes the relevance of studying this problem. Purpose. Based on the consideration of a real clinical case, to analyze the results of an experimental psychological study of a teenager girl with character accentuation and self- injurious behavior. Materials and methods. To prepare an original study of non-suicidal self-injurious behavior, this clinical case was selected. The analysis was carried out in several directions: clinical and anamnestic – based on an extract from the medical history, as well as a statistical map specially designed for the study. For the pathopsychological study, an expanded set of psychodiagnostic methods and a scale for the study of NSSP were used. Results. The article describes a clinical case of a 14-year-old teenager with F92.8 Mixed disorder of behavior and emotions, accompanied by character accentuation and self- injurious behavior. The results of the study are presented in the article, including the main stages and clinical search for diagnosis, as well as the design of the study. Particular attention is paid to trigger factors and clinical characteristics that predispose to the development of non-suicidal self-injurious behavior.
