This paper presents a clinical case describing hypoglycemic condition with atypical symptoms in a young patient in the early postpartum period. Abdominal ultrasound revealed a large formation in the hook region of the pancreas, which was subsequently confirmed according to endoscopic ultrasound, CT and MRI. The idea of insulinoma was formed according to the presence of pancreatic formation, documented Whipple’s triad, and data on hyperinsulinism at the outpatient stage. However, the 72-hour fasting test showed no significant increase in insulin and C-peptide levels. After excluding other causes of hypoglycemia, the patient was suspected of having proinsulinoma, which was confirmed by increased blood proinsulin level. Gastropancreatoduodenal resection was performed. We detected a highly differentiated neuroendocrine pancreatic tumor with the expression of CD56, NSE, synaptophysin and chromogranin A, with a Ki-67 index of about 1%. After surgical treatment, hypoglycemic conditions were not observed.
Обоснование. В мире около двух миллиардов человек имеют недостаточное потребление йода. Цель. Оценить обеспеченность йодом и эффективность профилактики йодного дефицита у взрослого населения Санкт-Петербурга. Методы. В ретроспективной части работы проведен анализ распространенности гипертиреотропинемии новорожденных Санкт-Петербурга за период 2013-2014 гг. В одномоментное поперечное когортное исследование были включены 542 жителя Санкт-Петербурга в возрасте 18-77 лет. Все участники были разделены на три группы согласно возрасту. Беременные женщины были выделены в отдельную группу. У всех участников проведено анкетирование и проанализи-рованы значения йодурии. Результаты. Количество новорожденных, имевших концентрацию тиреотропного гормона более 5 мЕД/л, составило 6,9%, отражая дефицит йода легкой степени тяжести у населения. Йодированную соль использовали в пищу только 41,1% всех обследованных и 52,2% беременных женщин. Препараты йода принимала половина беременных женщин. Медиана йодурии (МЙУ) всех участников составила 91,2 мкг/л и соответствовала дефициту йода легкой степени тяже-сти. Значение МЙУ у беременных женщин составило 112,4 мкг/л и соответствовало недостаточному потреблению йода. Заключение. Население Санкт-Петербурга проживает в регионе легкого дефицита йода. Профилактика дефицита йода, проводимая в Санкт-Петербурге в 2013-2015 гг., недостаточно эффективна. Лица репродуктивного возраста и беремен-ные женщины имеют дефицит йода легкой степени тяжести. Globally, it is estimated that 2 billion individuals have an insufficient iodine intake. Aim. To assess iodine status and the effectiveness of iodine deficiency prophylaxis in adult population of St. Petersburg. Methods. А retrospective analysis of the prevalence neonatal hyperthyrotropinemia in St. Petersburg for the period 2013-2014 was made. In a cross-sectional study were included 542 residents of St. Petersburg aged 18-77 years. All participants were divided into 3 groups according to age. Pregnant women were evaluated separately. All participants were questioned, the values of urinary iodine concentration (UIC) were analyzed. Results. The frequency of neonatal thyroid-stimulating hormone concentrations above 5 mU/L was 6.9%. Iodized salt was used by 41.1% of all surveyed persons and 52.2% of pregnant women. Iodine supplements were taken by half of pregnant women. Median UIС of all participants was 91.2 mcg/L and corresponded to mild iodine deficiency. Median UIC in pregnant women was 112.4 mcg/L and corresponded to insufficient iodine intake. Conclusions. The population of St. Petersburg lives in a region of mild iodine deficiency. Prophylaxis of iodine deficiency held in St. Petersburg in 2013-2015 was not effective enough. Persons of reproductive age and pregnant women had mild iodine deficiency.
