В Российской Федерации в структуре онкологической смертности рак желудка также занимает 2-е ранговое место (ежегодно заболе-вают более 37 000 человек, умирают более 20 000 человек); летальность в течение первого года после установления диагноза достигает 50 % [2]. Материал и методы. В МРНЦ им. А.Ф Цыба -филиале ФГБУ «НМИЦ радиологии» МЗ РФ проведена II фаза клинического исследования эффективности комбинированного лечения больных местнораспространенным раком желудка с неоадъювантной химиолучевой терапией и последующей гастрэктомией с лимфодиссекцией в объеме D2. Основными критериями включения были: морфологически подтвержденный рак желудка, клиническая стадия сT3-4N0, сT2-4N1-3; М0. Всем больным до начала неоадъювантной терапии выполнялось комплексное обследование, включая СКТ органов грудной и брюшной полоcтей и лапароскопию для исключения перитонеального канцеро-матоза. Лучевая терапия проводилась в СОД 45 Гр по методике дневного дробления дозы (1 + 1,5 Гр) на фоне модифицированной схемы CAPOX. Хирургическое лечение в объеме гастрэктомии/субтотальной резекции желудка планировали через 4-6 нед после завершения курса химиолучевой терапии. Оценка токсичности неоадъювантной химиолучевой терапии проводилась с помощью шкалы NCI CTC, версия 3.0. Itroduction. Gastric cancer incidence and mortality rates remain very high worldwide, including the Russian Federation. More than 50 % of gastric cancers are locally advanced at presentation [1]. Perioperative or adjuvant chemotherapy is a standard treatment for gastric cancer patients. The use of neoadjuvant chemoradiotherapy is considered very promising.Material and Methods. We present the analysis of the results of surgical treatment of cancer of the stomach and gastrooesophageal junction after various options of neoadjuvant therapy: chemotherapy, chemoradiotherapy, and their combinations. The experience of the Medical Radiological Research Centre (MRRC), including 5 clinical prospective studies and one randomized multicenter clinical trial, was analyzed. A total of 237 patients with histologically proven locally advanced cancer of the stomach and gastro-oesophageal junction were included into the study. Of these patients, 202 received neoadjuvant therapy. Our treatment outcomes were compared with those of randomized trials published over the last 15 years.Results. Of 202 patients who received neoadjuvant therapy, 190 (94 %) underwent surgery (R0 resection: 184 patients). In the early postoperative period, complications were observed in 62 patients (32.6 %). Re-surgery was performed in 11 (5.8 %) patients. Postoperative mortality was 1.6 % (3 patients). Multicenter randomized studies and meta-analyses, as well as the long-term experience of MRRC, have shown that neoadjuvant therapy does not increase the number of postoperative complications and mortality, increases the rate of R0 resections and improves long-term treatment outcomes in patients with resectable locally advanced cancer of the stomach and gastro-oesophageal junction.Conclusion. Analysis of long-term clinical studies conducted at the MRRC as well as the treatment results published by other authors show that in some cases neoadjuvant therapy can lead to soft tissue changes in a surgical area and may complicate surgical management; however, it does not have a negative impact on the rates of postoperative complications and mortality.
Background. Gastric cancer is the 5th most common cancer and the 3rd leading cause of cancer death globally. In Russia, gastric cancer is the 2-nd leading cause of death (10.7 %). Surgery in combination with perioperative/adjuvant chemotherapy is the standard treatment for locally advanced gastric cancer. Four cycles of neoadjuvant chemotherapy and 4 cycles of adjuvant chemotherapy with the FLOT regimen is the most widely used treatment modality. The basis for the widespread use of this chemotherapy regimen was the results of a large randomized FLOT4 trial, according to which the perioperative FLOT regimen showed greater efficacy compared to the ECF regimen.The aim of the study was to demonstrate the feasibility of achieving patholopgical complete response and long-term remission after one course of neoadjuvant chemotherapy with FLOT regimen in a patient with locally advanced gastric cancer.Case description. A 69-year-old patient presented to A. Tsyb Medical Radiological Research Centre with complaints of general weakness, moderate epigastric pain for 2 months. The examination revealed stage T3N0M0 IIA gastric cancer. The patient received one cycle of neoadjuvant chemotherapy with the FLOT regimen. Given the fact, that the patient developed massive bleeding from the tumor, it was decided to stop neoadjuvant treatment. Subtotal gastrectomy with D2 lymph node dissection was performed. Histological examination of the surgical specimen revealed pathological complete response (grade 1a according to the classification of K.Becker et al., 2003). The patient is alive with no signs of disease progression for more than 36 months after starting the treatment.Conclusion. This clinical case demonstrated personalized approach to the treatment of patients with locally advanced gastric cancer. Pathological complete response was achieved after one cycle of neoadjuvant chemotherapy.
