Cannon [1], впервые описавшего гуморально-гормональные изменения при различных эмоциональных состояниях (страхе, гневе и т.д.), и H. Selye [2], сформулировавшего концепцию «стресса» (стресс-синдрома). Психогенно провоцированные ССЗ возникают в связи как с тяжелым неспецифическим стрессом (катастрофы и катаклизмы), так и с психотравмирующими ситуациями высокой личностной значимости. Adherence has a direct impact on reducing the effectiveness of atrial fibrillation therapy and increasing the risk of thromboembolic events. Among the factors involved in the decrease of adherence, the social and psychological characteristics of patients remain insufficiently studied. At the same time, the available publications allow us to conclude that there are markers of the risk of reduced adherence in patients with atrial fibrillation, which include age, cognitive impairment, psychoemotional disorders (including depression and anxiety) and specific behavioral patterns.
Цель. Изучение опыта применения Пантогама актив (ООО "ПИК-ФАРМА", Россия) в рамках персонализированного подхода к коррекции когнитив-ных и эмоциональных (тревожно-депрессивных) нарушений у пациентов с артериальной гипертензией (АГ) и хронической сердечной недостаточ-ностью (ХСН). Материал и методы. Проведен анализ трех современных исследований, в которых получены данные об эффективности D-,L-гопантеновой/рац-гопан-теновой кислоты (препарат Пантогам актив) при сердечной патологии (АГ и ХСН). Конечными точками оценки эффективности терапии были: редукция когнитивных нарушений, коррекция эмоционального статуса (тревожные и депрессивные расстройства), улучшение качества жизни. Также оценива-лось влияние препарата на состояние показателей сердечно-сосудистой системы. Результаты. В трех исследованиях приняли участие суммарно 232 пациента с сердечно-сосудистыми заболеваниями (ССЗ), из них 147 пациентов соста-вили группу контроля. Длительность приема Пантогама актив составляла от 4 до 12 недель. На фоне лечения доля респондеров в отношении когнитивных нарушений составила в среднем 78-100%, при эмоциональных расстрой-ствах -до 88%, улучшение качества жизни -86%. Переносимость терапии оценивается как хорошая, минимально выраженные побочные эффекты под-вержены редукции в первую неделю лечения. Заключение. Применение Пантогама актив в комплексной терапии пациен-тов с АГ и ХСН приводит к коррекции коморбидных когнитивных и эмоциональ-ных нарушений при отсутствии привыкания, гиперстимуляции и синдрома отмены, улучшает комплаентность пациентов с сердечно-сосудистой патоло-гией, повышает качество жизни и способствует повышению эффективности терапии основного ССЗ и улучшению кардиологических показателей (артери-ального давления, вариабельности ритма сердца, толерантности к физиче-ской нагрузке). Also, the influence of the drug was evaluated, on the parameters of cardiovascular system. Results. In the 3 studies, totally 232 patients participated, with cardiovascular diseases (CVD), of those 147 were controls. Duration of Pantogam active intake lasted for 4 to 12 weeks. Responders by the parameter of cognitive function were 78-100%, for emotional disorders -to 88%, for life quality improvement -86%. Tolerance of the therapy was evaluated as good, and minimal side effects were reduced within the first week of treatment. Conclusion.Application of Pantogam active for complex treatment of AH and CHF patients leads to correction of comorbid cognitive and emotional disorders, with the absence of tolerance, hyperstimulation and withdrawal syndrome, improves life quality, contributes to therapy effectiveness of main CVD and improvement of cardiovascular parameters (blood pressure, heart rate variability, exercise tolerance).Russ J Cardiol 2017, 7 (147): 152-161 http://dx
Objective: to analyze available scientific publications on the problem of clinical and genetic aspects of depressive disorders in patients with coronary heart disease (CHD). Material and methods: by keywords “coronary heart disease”, “mental disorders”, “depression”, “genetics”, “genetic markers”, “polymorphism”, “personality” articles in English and Russian were searched in the MEDLINE/PubMed, Scopus, Web of science, eLibrary databases in the period from 2020 to 2021. Conclusion: depression comorbid CHD is considered as a clinical problem due to its high prevalence, as well as its influence on the features of the course and prognosis of CHD, as well as on the compliance of patients. Heterogeneity of the clinical structure of depressive disorders in CHD (nosogenic, endogenomorphic, endogenous, somatogenic depression) causes difficulties in differential diagnosis and complex therapeutic measures. There are several factors contributing to the onset of depression in CHD patients: biological, clinical, personal characteristics. Genetic markers of depression in CHD, which have prognostic value, require further clarification.
