Введение. Пациентам, госпитализированным с COVID-19, часто назначают антибиотики широкого спектра действия, которые могут способствовать селекции и распространению антибиотикорезистентных штаммов.Цель. Выявить продукцию карбапенемаз и установить механизмы активного выведения антибиотиков различных групп у множественно- и экстремально-антибиотикорезистентных штаммов Klebsiella pneumoniae и Acinetobacter baumannii, выделенных от госпитализированных пациентов с инфекцией COVID-19.Материалы и методы. Для 51 множественно- и экстремально-антибиотикорезистентного штамма Klebsiella pneumoniae и 47 множественно-антибиотикорезистентных штаммов A. baumannii, выделенных от госпитализированных пациентов с инфекцией COVID-19, методом микроразведений в бульоне определены минимальные подавляющие концентрации (МПК). Дополнительно установлены МПК антибиотиков в присутствии протонного ингибитора эффлюксных насосов – карбонилцианид-3-хлорфенилгидразона (CCCP). Фенотипический скрининг продукции метало-β-лактамаз (МБЛ) у штаммов K. pneumoniae выполнен методом двойных дисков с ЭДТА. Детекция генов карбапенемаз осуществлена методом ПЦР-анализа в режиме реального времени.Результаты. Все штаммы проявляли множественную либо экстремальную устойчивость к антибиотикам. Все штаммы A. baumannii были устойчивы к карбапенемам, левофлоксацину, амикацину и являлись продуцентами OXA-карбапенемаз. Сохраняли чувствительность к карбапенемам только 2,0–3,9% штаммов K. pneumoniae, к амикацину – 7,8% штаммов. Продукция карбапенемаз выявлена у 68,6% штаммовK. pneumoniae (NDM – 31,4%, OXA-48 – 23,5%, KPC – 2,0%, копродукция двух карбапенемаз – 11,8%). Ингибирование эффлюксных насосов позволило снизить значения МПК карбапенемов, цефалоспоринов, левофлоксацина, амикацина, тигециклина для 2,1–25,5% штаммов A. baumannii и K. pneumoniae, но не восстанавливало их чувствительность к антибиотикам.Заключение. Устойчивость к карбапенемам и другим β-лактамам у штаммов A. baumannii и K. pneumoniae связана с продукцией карбапенемаз. Присутствие у части исследуемых штаммов механизмов эффлюкса является важным, но не единственным механизмом антибиотикорезистентности. Introduction. Patients hospitalized with COVID-19 are often prescribed broad-spectrum antibiotics that can contribute the selection and spread of antibiotic-resistant strains.Purpose. To identify carbapenemase production and the active efflux mechanisms for antibiotics of various classes in multidrug- and extensively drug-resistant Klebsiella pneumoniae and Acinetobacter baumannii strains isolated from patients with COVID-19 infection.Materials and methods. This study included 51 multidrug- and extensively drug-resistant Klebsiella pneumoniae strains and 47 multidrug-resistant strains of Acinetobacter baumannii isolated from patients with COVID-19 infection. The minimum inhibitory concentrations (MICs) for meropenem, imipenem, cefepime, ceftazidime, doxycycline, levofloxacin, tigecycline, rifampicin, chloramphenicol and amikacin were assessed by broth microdilution method for all investigated strains. Moreover, MICs of antibiotics in the presence of efflux pump inhibitor (carbonyl cyanide-chlorophenylhydrazone, CCCP) were determined. For phenotypic detection of metallo-β-lactamase (MBL) production the double-disk EDTA method was performed. Carbapenemase genes were detected by real- time PCR.Results. All of the isolated strains have demonstrated multidrug or extensively drug resistance. All strains of A.baumannii were resistant to carbapenems, levofloxacin, amikacin and were OXA-carbapenemase producers. Susceptibility to carbapenems remained only in 2.0–3.9% of K. pneumoniae strains, to amikacin – in 7.8% of strains. Carbapenemase production was detected in 68.6% of K. pneumoniae strains (NDM – 31.4%, OXA-48 – 23.5%, KPC – 2.0%, co-production of two carbapenemase – 11.8%). Inhibition of efflux pumps made it possible to reduce the MIC values for carbapenems, cephalosporins, levofloxacin, amikacin, tigecycline in 2.1–25.5% of A. baumannii and K. pneumoniae strains. On the other hand, it didn’t restore their susceptibility to antibiotics.Conclusion. Resistance to carbapenems and other β-lactams in A.baumannii and K. pneumoniae strains is associated with the production of carbapenemases. The presence of efflux mechanisms in some of the studied strains is an important, but not the only mechanism of antibiotic resistance.
Цель исследования. С использованием созданной системы надзора за антибиотикорезистентностью определить генетические детерминанты антибиотикорезистентности грамотрицательных бактерий – возбудителей инфекций кровотока и оценить их этиологическое значение в период пандемии COVID-19. Материалы и методы. Из выделенных положительных гемокультур отобрано 249 штаммов грамотрицательных бактерий с множественной или экстремальной антибиотикорезистентностью. Критерием включения в исследование являлось наличие устойчивости к карбапенемам (имипенему и/или меропенему). Детекция генов карбапенемаз выполнена методом ПЦР в режиме реального времени для 135 множественно- и экстремально-антибиотикорезистентных штаммов Klebsiella рneumoniae, 91 штамма Acinetobacter baumannii и 23 штаммов Pseudomonas aeruginosa. Проведен сравнительный анализ этиологической структуры и профилей резистентности 176 инвазивных изолятов, выделенных при исследовании крови на стерильность в условиях пандемии COVID-19. Контрольную группу составил 171 изолят положительных гемокультур, изученных до пандемии COVID-19 в 2019 году. Результаты и обсуждение. В этиологической структуре инфекций кровотока в период пандемии COVID-19 отмечено увеличение частоты выделения грамотрицательных бактерий. Выделенные в период пандемии из крови штаммы K. pneumoniae, A. baumannii, P. аeruginosa демонстрировали рост резистентности к основным антимикробным препаратам. Продукция карбапенемаз выявлена у 21,7% штаммов P. аeruginosa (МБЛ VIM), всех протестированных штаммов A. baumannii (OXA-23 у 3,3% изолятов, OXA-40 – у 96,7%), у 48,2% штаммов K. рneumoniae OXA-48, у 14,8% изолятов карбапенемаза KPC, у 32,6% металло-β-лактамаза NDM, еще 4,4% являлись копродуцентами β-лактамаз OXA-48 и NDM. Разработанная лабораторная информационная система АРМ врача-бактериолога «Клиническая микробиология» позволила увеличить оперативность и надежность обработки микробиологической информации. Заключение. Показано преобладание в этиологической структуре инфекций кровотока в период пандемии COVID-19 штаммов K. pneumoniae (67,3%) и значительное увеличение у грамотрицательных бактерий устойчивости к большинству противомикробных лекарственных средств. Purpose of the study. To determine the genetic determinants of antibiotic resistance in Gram-negative bacteria that cause bloodstream infections and to assess their etiological significance during the COVID-19 pandemic using the established antibiotic resistance surveillance system. Materials and methods. 249 strains of gram-negative bacteria with multi-drug and extensively-drug resistance were selected from isolated positive blood cultures. The criterion for inclusion in the study was the presence of carbapenems resistance (imipenem and/or meropenem). Detection of carbapenemase genes was performed by real-time PCR for 135 multi- and extensively-drug resistant K. pneumoniae strains, 91 A. baumannii strains and 23 P. aeruginosa strains. A comparative analysis of the etiological structure and resistance profiles of 176 invasive strains isolated in blood sterility testing during the COVID-19 pandemic was carried out. The control group consisted of 171 isolates of positive hemocultures studied before the COVID-19 pandemic in 2019. Results and discussion. In the etiological structure of bloodstream infections during the COVID-19 pandemic, an increase in the frequency of isolation of gram-negative bacteria was noted. The K. pneumoniae, A. baumannii, P. aeruginosa strains isolated from the blood during the pandemic showed an increase in resistance to the main antibiotics. Carbapenemase production was detected in 21.7% P. aeruginosa strains (MBL VIM), all tested A. baumannii strains (OXA-23 in 3.3% of isolates, OXA-40 in 96.7%). Among K. pneumoniae strains were: 48.2% OXA-48-producers, 14.8% KPC-producers, 32.6% of strains metallo-β-lactamase NDM producers. Another 4.4% were co-producers of β-lactamases OXA-48 and NDM. The developed laboratory information system AW of a bacteriologist "Clinical Microbiology" made it possible to increase the efficiency and reliability of processing microbiological information. Conclusion. The predominance of K. pneumoniae strains in the etiological structure of bloodstream infections during the COVID-19 pandemic (67.3%) and a significant magnification in resistance to most antibiotics in gram-negative bacteria.
