Актуальность: Оценка состояния коагуляционного потенциала беременной является одним из основных компонентов в прогнозировании неблагоприятных репродуктивных исходов. В настоящее время наблюдается низкая прогностическая эффективность маркеров коагуляции (протромбиновый индекс (ПТИ), протромбиновое время (ПВ), активированное частичное тромбопластиновое время (АЧТВ), фибриноген, показатель тромбоцитов), используемых в скрининговом обследовании беременных в первом триместре для выявления коагулопатий различного генеза. В связи с этим назрела необходимость предложить более эффективные лабораторные маркеры нарушения свертывающей системы крови для проведения скрининговых обследований. Цель исследования – оценка прогностического значения маркеров коагуляции (ПТИ, ПВ, АЧТВ, фибриноген, показатель тромбоцитов), используемых в скрининговых обследованиях беременных в первом триместре для выявления риска неблагоприятных исходов беременности.Методы: Был проведен поиск литературы в базах данных PubMed, ELibrary, Cochrane, Scopus, Web of Science по ключевым словам: «pregnancy», «laboratory markers», «coagulation», «miscarriage». В поиск были включены результаты клинических исследований, метаанализов, рандомизированных контролируемых испытаний (РКИ), систематических обзоров, опубликованных на английском языке за последние 10 лет.Результаты: В настоящее время назрел вопрос о пересмотре и дополнении имеющихся скрининговых маркеров коагуляции в пользу представленных новейших маркеров: тромбомодулин (TM), тромбин-антитромбиновый комплекс (TAT), комплекс ингибиторов плазмина α2 (PIC) и тканевого активатора плазминогена (tPAI-С), учитывая их высокую значимость в определении коагуляционного потенциала женщин в репродуктивном возрасте как достоверных маркеров диагностики тромбоза с высокой чувствительностью. Это значительно улучшит процесс выявления лиц, предрасположенных к нарушению коагуляции и существенно снизит спонтанные репродуктивные потери. Заключение: Высокая прогностическая эффективность новых маркеров свертывания крови TM, TAT, PIC и tPAI-C в информировании о нарушении коагуляционного потенциала у беременных женщин, а также высокая чувствительность указанных маркеров в отношении тромбозов создают предпосылки для более длительных и глубоких исследований данных маркеров у женщин с негативными результатами беременности в первом триместре с целью диагностики и прогноза репродуктивных потерь.
