Background Low back pain (LBP) is second only to common cold as the most frequent reason for seeking medical care and is the leading cause of Years Lived with Disability (YLD) worldwide (1). LBP often creates a therapeutic challenge due to its unclear etiology and multiple interventions of uncertain efficacy. Significant proportion of LBP may be attributed to osteoarthritis (OA) and degenerative changes in the spine. Glucosamine-chondroitin sulfate (GCS) combination is widely used in the treatment of OA, despite conflicting evidence of its efficacy; however there are few prospective scientific investigations of its therapeutic merits in the management of LBP. Objectives To study the efficacy and safety of GCS in the community management of LBP in a large-scale open pilot prospective interventional study. Methods We enrolled patients between 40 and 65 years of age who had LBP for at least 12 weeks with a pain intensity >3 on a 0-10 point visual analogue scale (VAS). Major exclusion criteria were the presence of fibromyalgia, degenerative spondylolisthesis, and alcohol and/or drug abuse. All patients were treated with ARTRA (combination of glucosamine hydrochloride 500 mg and chondroitin sulfate 500 mg in tablet form; Unipharm Inc.) at a dose of 1 tab bid for the first month and then 1 tab daily for the next two months. The primary endpoint was pain intensity (at rest and movement) as measured on a 0-10 point VAS. Secondary endpoints included the Oswestry Disability Index, patient global assessment of efficacy (0-5 scale) and NSAID consumption. Results A total of 8,598 subjects (mean age 52.1 years, 67.3% women, mean BMI 27.4) were enrolled in the study, and formed the intent-to-treat (ITT) population. All but 95 subjects (1.1%) completed the study. ITT analysis with worst observation carried forward (WOCF) showed an improvement in pain at rest from mean (± SD) of 5.2 ± 1.9 at study entry to 1.4±1.6 at 3 months (p<0.0001). Pain at movement decreased from 6.8±1.6 to 2.2 ±1.8 (p<0.0001). The Oswestry disability index improved by almost 75%, from 21.1±9.7 to 5.3±6.0 (p<0.0001) at 3 months. NSAIDs were used by 63.4% of patients at study entry; at 3 months of treatment, only 7.4% of patients required NSAIDs for pain control (p<0.0001). An adverse event (AE) was reported by 604 (7.0%) patients (mostly gastrointestinal in origin, such as nausea, abdominal pain and dry mouth) but only 85 (1.0%) patients deemed it severe enough to discontinue therapy. Conclusions Although open and uncontrolled, this large community-based study shows dramatic reductions in pain and disability, and in particular, a 88% reduction in NSAID consumption in patients with LBP treated with GCS. With its benign safety profile, GCS therapy deserves serious evaluation in the management of LBP in a prospective randomized double-blinded clinical trial. Представлены результаты ретроспективного анализа исследований биоэквивалентности воспроизведенных препаратов антагонистов рецепторов ангиотензина II. Показано, что препараты лозартана, валсартана и телмисартана могут быть отнесены к высоко вариабельным по такому фармакокинетическому параметру как максимальная концентрация в плазме крови. Препараты кандесартана, ирбесартана и олмесартана не демонстрируют высокую внутрииндивидуальную вариабельность в исследованиях биоэквивалентности. Описаны существующие в настоящее время регуляторные рекомендации и подходы к изучению биоэквивалентности высоковариабельных лекарственных препаратов, сформулированы рекомендации в отношении дизайна и оценки результатов исследований антагонистов рецепторов ангиотензина II.

Проведен анализ нормативных документов и регуляторных требований к статистическим принципам планирования и оценки результатов исследований биоэквивалентности лекарственных препаратов. Описаны существующие в настоящее время актуальные статистические подходы к оценке биоэквивалентности и соответствующие рекомендации к планированию исследований лекарственных препаратов, в том числе, высоковариабельных препаратов, препаратов с узким терапевтическим диапазоном и препаратов -аналогов эндогенных соединений. В статье рассмотрены такие статистические подходы, как биоэквивалентность в среднем (average Bioequivalence (aBe)), биоэквивалентность в среднем с расширением границ признания (average Bioequivalence with expanding limits (aBel)), биоэквивалентность в среднем с масштабированием границ (reference-Scaled average Bioequivalence (rSaBe)). Изложены особенности статистического анализа в отношении малоизученных лекарственных препаратов. Описаны приемлемые с регуляторной точки зрения алгоритмы планирования и проведения двухэтапных дизайнов исследований биоэквивалентности, так как в подобных исследованиях присутствует необходимость множественного тестирования гипотезы биоэквивалентности, что приводит к повышению вероятности ошибки первого рода (риска потребителя). Сформулированы рекомендации по выбору статистических подходов и описаны некоторые особенности статистических методов анализа в зависимости от дизайна исследования и типа воспроизведенного лекарственного средства.

Качество и результативность доклинических исследований лекарственных средств гарантируются соблюдением правил надлежащей лабораторной практики (НЛП). Вместе с тем данные Всемирной организации здравоохранения и анализ нормативно-правовой базы свидетельствуют о том, что в настоящее время весьма актуальными проблемами разработки лекарственных средств являются достоверность и воспроизводимость результатов базовых биомедицинских исследований. Вследствие поискового характера исследований, связанных с подтверждением научных гипотез, и разнообразия используемых методологических подходов отсутствует возможность рационального применения жестких критериев НЛП для всех видов фармакологических исследований. Согласно международным актам, правила НЛП имеют статус требований и регламентируют качество доклинических исследований безопасности лекарственных средств в хорошо стандартизованных батареях тестов токсикологических исследований и исследований фармакологической безопасности, но не распространяются на исследования первичной фармакодинамики, определяющей потенциальную терапевтическую эффективность лекарственного средства. Зарубежные регуляторные органы рекомендуют нормировать фармакологические исследования на основе принципов НЛП также и при изучении вторичной фармакодинамики, особенно в том случае, если данный вид исследований вносит важный вклад в оценку безопасности препарата. Исследования фармакологической активности лекарственных средств, имеющие решающее значение в оценке перспективности кандидата на доклиническом этапе разработки лекарственного препарата, как правило, представляют собой «нерегулируемые исследования», результаты которых могут оказаться некорректными. В работе рассмотрена система регламентации фармакологических исследований, включающая организационные принципы их планирования и проведения, изложенные в рекомендациях по качественной практике базовых биомедицинских исследований. Применение этих принципов позволит обеспечить достоверность и воспроизводимость результатов доклинических фармакологических исследований и повысить их научную и практическую ценность при разработке новых лекарственных средств. Рациональное применение лекарственных средств учитывает медико-биологические и фармацевтические факторы, обеспечивающие высокую биодоступность и эффективность лекарственного препарата. Вместе с тем представляет интерес взаимодействие лекарственного препарата с системами организма с учетом особенностей хронофармакологии, изучающей действие лекарственных средств в зависимости от биоритмов человека. С позиции хронопатологии различные патологические процессы и заболевания, в том числе сахарный диабет, является результатом нарушения биоритмов. В патогенезе сахарного диабета возникает десинхроноз, связанный не только с нарушением секреции эндогенного инсулина, но и с индуцированием искусственного ритма при инсулинотерапии. Представлена информация о влиянии биоритмов углеводного обмена на эффективность гипогликемической терапии. Обсуждается целесообразность изучения хроносхем для применяемых гипогликемических препаратов.
