Методы наружной хирургии глазных мышц различны и включают использование как регулируемых, так и нерегулируемых швов, что влияет на результат в отдаленном периоде наблюдения. Хирургическое вмешательство с использованием метода регулируемых швов применяют в течение ряда лет для улучшения функциональных результатов лечения косоглазия. Основной принцип метода регулируемых швов заключается в обеспечении безопасности экстраокулярных мышц с использованием временного или скользящего узла. Цель работы-разработка способа хирургического лечения неоднократно оперированного косоглазия с гиперэффектом и анализ клинических случаев. Нами предложен способ коррекции неоднократно оперированного косоглазия с гиперэффектом методом регулируемых швов. Описаны три клинических случая. Пациентка 34 года, сходящееся оперированное косоглазие, угол косоглазия 15 градусов. Произведена рецессия внутренней прямой мышцы 4 мм и резекция наружной прямой мышцы 5 мм. На следующий день после операции, в связи с выявлением гипоэффекта, проведена регулировка швов. Через 1 месяц угол девиации 0 градусов. Пациентка 29 лет, расходящееся косоглазие, угол девиации 35 градусов, ранее дважды оперирована по поводу сходящегося косоглазия на левом глазу. Проведена операция на левом глазу. На следующий день после операции выявлено наличие гиперэффекта, для коррекции выполнено ослабление репозированной мышцы и усиление рецессированной мышцы в соответствии с заявляемым способом. Достигнут желаемый косметический результат. Через 1 месяц угол девиации 0 градусов. Пациентка 24 года, ранее оперированное расходящееся косоглазие, угол девиации 15-20 градусов. Проведена репозиция внутренней прямой мышцы на 4 мм, рецессия наружной прямой на 4 мм. На следующий день после операции достигнут желаемый косметический результат. Через 1 месяц угол девиации 0 градусов. Представленный способ хирургического вмешательства и клинические случаи свидетельствуют о том, что подход к лечению косоглазия с использованием регулируемых швов представляется перспективным направлением в офтальмохирургии.
