Цель исследования. Оценка прогностической значимости раково-эмбрионального антигена (РЭА) у больных раком прямой кишки и корреляции его исходного уровня со степенью лечебного патоморфоза после неоадъювантной химиолучевой терапии (ХЛТ). Материалы и методы. Проведена оценка информативности показателей РЭА у 179 больных раком прямой кишки (РПК), определяемого до и после предоперационной ХЛТ в суммарной очаговой дозе (СОД)50 Гр. Результаты. Анализ результатов, представленных в работе, показал, что у всех пациентов ХЛТ вызвала достоверно значимое снижение уровня РЭА (-71%) через 10 нед после ее окончания (р < 0,001). В ходе патоморфологического исследования после неоадъювантного лечения I степень патоморфоза опухоли зафиксирована у 4,5% больных, II-38,5%, III-45%, IV-12% (степень патоморфоза не связана с клинической стадией и степенью дифференцировки РПК). Выявлено, что пациенты с III и IV степенью лечебного патоморфоза исходно имели уровень РЭА ниже, по сравнению с больными со степенью I-II. Клиническое прогрессирование заболевания диагностировано в 24% случаев (43/179). Отмечено, что у больных с IV степенью лечебного патоморфоза опухоли ни в одном случае не было выявлено рецидива РПК. Заключение. Результаты проведенного исследования показали, что проблема индивидуального прогнозирования эффективности комбинированного лечения РПК остается весьма актуальной, достаточно сложной и еще не совсем решенной. Однако можно полагать, что использование такого показателя как РЭА в мониторировании больных РПК после проведенного лечения может служить критерием чувствительности РПК к ХЛТ. Исходно низкий уровень антигена может рассматриваться как позитивный фактор ответной реакции опухоли на проводимое лечение и безрецидивную выживаемость больных местно-распространенным РПК.
