Актуальность: Различные методы экстракорпорального оплодотворения (ЭКО) позволяют достичь желанную беременность, но одновременно создают проблему эффективного и персонализированного подхода в ведении беременности и родоразрешения ВРТ-индуцированных пациенток, обеспечивающего минимизацию акушерско-перинатальных потерь. Несмотря на то, что различные методы ВРТ помогают достичь желаемой беременности, существует проблема эффективного персонализированного подхода к минимизации акушерских и перинатальных потерь при ведении беременности и родов у пациенток, прошедших программу ЭКО. Особенным отличием второго триместра при ЭКО-индуцированной беременности является риск развитие истмико-цервикальной̆ недостаточности (ИЦН). Встречаемость ИЦН в популяции составляет 1-9% по данным различных авторов. Встречаемость ИЦН в случаях невынашивании беременности – 15-42%.Цель исследования – изучить эффективность применения акушерского пессария при ИЦН у беременных после ВРТ, оценить возможные факторы риска возникновения ИЦН при беременности в результате ВРТ.Материалы и методы: Были созданы две исследовательские группы: основная группа (n=84) – беременные после ВРТ с установленным диагнозом ИЦН и группа сравнения (n=116) – беременные после ВРТ без ИЦН.Проведен ретроспективный анализ 200 амбулаторных карт женщин после проведения программы ЭКО.Результаты: ИЦН достоверно чаще встречается у пациенток с эндокринным бесплодием. Применение акушерского пессария способствует пролонгированию беременности до 29-33 (33%) и 34-36 недель (62%) беременности, снижению очень ранних преждевременных родов и рождения детей с экстремально низкой массой тела. Применение акушерского пессария способствовало пролонгированию беременности до срока родов в 71% наблюдений. Достоверных различий в методах родоразрешения в исследуемых группах не выявлено. У пациенток с ИЦН достоверно чаще регистрируется количество послеродовый кровотечений.Заключение: Учитывая полученные результаты можно полагать, что при диагностике ИЦН до 20 недель гестации предпочтительно применение хирургической коррекции или сочетания хирургических методов и введения акушерского пессария, что позволит снизить число преждевременных родов.
Актуальность: Во всем мире неуклонно растет частота применения операции кесарева сечения. Применяются инновационные органосохраняющие хирургические техники - донное кесарево сечение с проведением метропластики (иссечения участка матки с врастанием плаценты). Необходимость новых подходов к ведению таких рожениц была обусловлена в первую очередь стремлением минимизировать кровопотерю, неизбежную при данном состоянии и достигающую порой 5000-7000 мл. Цель исследования – обосновать эффективность и преимущество проведения донного кесарева сечения при предлежании плаценты в сравнении с классическим методом оперативного родоразрешения.Методы: Был проведен ретроспективный анализ 27 случаев с предлежанием плаценты в Перинатальном центре и детской кардиохирургии г. Алматы. Результаты: Проведенный анализ донного кесарева сечения у беременных с предлежанием приращенной плаценты показал, что при донном кесаревом сечении интраоперационная кровопотеря уменьшилась в 2,5 раза, объем заместительной терапии в виде гемо-, плазмотрансфузии и реинфузии сократился с 4800,0 до 700,0 мл (р<0,05), экономическая затратность случаев снизилась в 2 раза.Заключение: Таким образом, операция «донное кесарево сечение» может быть рекомендована как метод выбора у беременных с предлежанием приращенной плаценты.
