Применение микропроточных TaqMan Array Cards (TAC карт) стало одним из инновационных направлений микробиологии последних двух десятилетий, позволяя одномоментно определять целый спектр энтеропатогенов (бактерий, вирусов, простейших, гельминтов). Цель. Идентификация возбудителей кишечных инфекций у детей с постдиарейным гемолитико-уремическим синдромом (ГУС) с использованием метода ПЦР в реальном времени на основе ТАС карт. Материалы и методы. В исследование включено 54 ребенка в возрасте от 9 месяцев до 10 лет с диагнозом ГУС, госпитализированных в УЗ «2-я городская детская клиническая больница» г. Минска в 2021 г. Результаты. Гены энтеропатогенов, способных вызывать диарейный синдром, были выявлены у 39 (72,2%) из 54 обследованных детей: гены бактериальных патогенов (диареегенная E. сoli, Campylobacter, Clostridium difficile, Bacteroides fragilis) – у 23 (42,6%), вирусных патогенов (рота-, норо-, адено-, саповирусов) – у 11 (20,4%), у 2 детей (3,7%) выявлены гены простейшего (Cryptosporidium), у 3 детей (5,6%) – гены и вирусных и бактериальных патогенов. Среди 25 детей с выявленной диареегенной E. coli у 20 (80,0%) присутствовала шигатоксин-продуцирующая E. сoli (STEC). Во всех 20 случаях наличие STEC сопровождалось выявлением шигатоксина 2-го типа, в 8 их них одновременно присутствовал шигатоксин 1-го и 2-го типов. В 6 случаях выявлялись только STEC, в 8 – обнаружена энтерогеморрагическая E. coli (STEC/EHEC), у 6 – гибридные варианты (STEC/EAEC и STEC/EHEC/EAEC). 92,6% детей получали антибактериальные или кишечные противомикробные препараты на момент поступления в нефрологический центр. STEC выявлялись через 7 (5; 9) дней, максимально – 13 дней от начала диареи до забора фекалий. Выводы. Полученные с использованием ТАС карт данные подтверждают роль STEC как основного этиологического агента ГУС. Роль других E. сoli (не шигатоксин-продуцирующих штаммов), а также других патогенов в развитии ГУС требует дальнейшего изучения. Целесообразно расширение алгоритма этиологической диагностики ГУС и генетического анализа обнаруженных гибридных вариантов диареегенных E. coli. Introduction. Applying of microfluidic TaqMan Array Cards (TAC cards) has become one of the innovative directions in microbiology of the last two decades, allowing simultaneous detection of a wide range of enteropathogens (bacteria, viruses, protozoa, helminths). Purpose. Identification of the aetiology of diarrhoea in children with postdiarrheal hemolytic uremic syndrome (HUS) using real-time PCR based on TAC cards. Materials and methods. A total of 54 patients aged 9 months to 10 years with HUS hospitalized at the 2nd City Children’s Clinical Hospital in Minsk in 2021 were included in the study. Results. Genes of enteropathogens responsible for diarrheal syndrome were identified in 39 (72.2%) of 54 examined children: genes of bacterial pathogens (diarrheal E. coli, Campylobacter, Clostridium difficile, Bacteroides fragilis) – in 23 (42.6%), viral pathogens (Rota-, Noro-, Adeno-, Sapoviruses) – in 11 (20.4%); in 2 children (3.7%) genes of the protozoan (Cryptosporidium) were detected, and in 3 children (5.6%) – genes of both viral and bacterial pathogens. Among 25 children diagnosed with diarrheagenic E. coli, 20 (80.0%) had shigatoxin-producing E. coli (STEC). In all 20 cases shigatoxin type 2 (stx2) was detected, in 8 of them both shigatoxin types 1 and 2 were present. In 6 cases, only STEC was detected, whereas enterohemorrhagic E. coli (STEC/EHEC) in 8 cases and hybrid variants in 6 cases (STEC/EAEC and STEC/EHEC/EAEC). 92.6% of children received antibacterial or intestinal antimicrobials at the time of admission to the nephrology center. STEC were detected after 7 (5; 9) days, with a maximum of 13 days from the onset of diarrhea to fecal collection. Conclusions. The data obtained using TAC cards confirm the role of STEC as the main etiological agent of HUS. The role of other E. coli (non-shiga toxin-producing strains), as well as other pathogens, in the development of HUS requires further study. It is advisable to expand the algorithm for HUS etiological diagnosis of and genetic analysis of detected hybrid variants of diarrheal E. coli.
Введение. Вспышки гемолитико-уремического синдрома (ГУС) описаны во многих странах мира, однако такая информация отсутствует по странам Восточной Европы.Цель. Определить клинические и лабораторные различия, оценить заболеваемость и исходы во время и вне вспышки ГУС у детей в Беларуси.Материалы и методы. В исследование включены 80 детей с установленным диагнозом ГУС в 2021 году в Республике Беларусь, 64 из них поступили в детский диализный центр г. Минска и были разделены на 2 группы: 1-я (29 детей) – вне вспышки ГУС, 2-я (35 детей) – во время вспышки. Кроме стандартных лабораторных исследований, у 52 пациентов проведен анализ кала на шигатоксинпродуцирующие E. coli методом ПЦР в реальном времени (TaqMan Array Card).Результаты. Заболеваемость ГУС в 2021 году составила 5,0 на 100 000 детей <15 лет (80 случаев) и 10,6 на 100 000 детей <5 лет (59 случаев). Доля мальчиков – 52%, детей <5 лет – 74%, пациентов с атипичным ГУС – 2,5%. 27.09.2021–29.10.2021 зафиксирована вспышка ГУС – 45 детей преимущественно из 3 регионов страны: город Минск – 17, Минская область – 16, Витебская область – 11. Группы пациентов не различались по возрасту – 2,5 (1,6; 5,1) и 3,6 (2,2; 5,1) года, доле пациентов с гемоколитом – 62% и 69%, исходным уровням гемоглобина – 85 (77; 99) и 102 (90; 105) г/л и тромбоцитов – 45 (25; 71) и 53 (29; 78) ×109/л, потребности в диализе – 79% и 57%, длительности анурии – 13 (7; 16) и 12 (8; 15) дней, летальности – 3,4% (аГУС) и 2,9% (на фоне COVID-19), частоте выявления шигатоксинпродуцирующей E. coli в образцах фекалий – 33% и 37%. Причину вспышки ГУС установить не удалось.Выводы. Заболеваемость ГУС у детей в Беларуси остается одной из самых высоких в Европе. Требуется разработка национального алгоритма действий по выявлению источника заражения, индикации и идентификации возбудителя при STEC-инфекциях. Introduction. Outbreaks of hemolytic uremic syndrome (HUS) have been described in many countries around the world, but no such information is available for Eastern European countries.Purpose. To determine clinical and laboratory differences, and to evaluate morbidity and outcomes during and outside the outbreak of HUS in children in Belarus.Materials and methods. A total of 80 children diagnosed with HUS in 2021 in Belarus were included in the study; 64 of them were admitted to the pediatric dialysis center in Minsk and divided into 2 groups: group 1 (29 children) – outside the HUS outbreak, and group 2 (35 children) – during the outbreak. In addition to standard laboratory tests, 52 children underwent stool analysis for Shiga toxin-producing Escherichia coli by real- time PCR (TaqMan Array Card).Results. The incidence of HUS in 2021 was 5.0/100 000 children <15 years (80 cases) and 10.6/100 000 children <5 years (59 cases). Boys were 52%, children <5 years old were 74%, patients with atypical HUS were 2.5%. Between 27.09.2021 and 29.10.2021 an outbreak of HUS was registered in 45 children, mostly from three country’s regions: Minsk city – 17, Minsk region – 16 and Vitebsk region – 11. Patients of groups 1 and 2 did not differ in terms of age, 2.5 (1.6; 5.1) and 3.6 (2.2; 5.1) years, respectively, incidence of hemocolitis:62% and 69%, respectively, baseline hemoglobin levels, 85 (77; 99) and 102 (90; 105) g/L, respectively, and platelet counts, 45 (25; 71)×109/L and 53 (29; 78) ×109/L, respectively, need for dialysis, 79% and 57%, respectively, duration of anuria, 13 (7; 16) and 12 (8; 15) days, respectively, mortality, 3.4% (aHUS) and 2.9% (on the background of COVID-19), respectively, incidence of Shigatoxin-producing E. coli in faecal samples, 33% and 37%, respectively. The cause of HUS outbreak remained undetermined.Conclusions. Thus, the incidence of HUS in children in Belarus remains one of the highest in Europe. A national algorithm should be developed to detect the source of infection and to indicate and identify the pathogen in STEC infections.
Целью исследования являлся анализ заболеваемости эпидемическим паротитом в Республике Беларусь за период 2013–2022 гг. на основании результатов лабораторного обследования пациентов и филогенетического исследования возбудителя. За десятилетний период в стране официально зарегистрировано 38 случаев эпидемического паротита, заболеваемость составила менее 1 случая на 1 млн населения. С использованием серологических и молекулярно-генетических методов за это время обследовано 1013 пациентов с подозрением на эпидемический паротит. Диагноз подтвержден у 29 (2,7%) из них, в том числе у 19 жителей Республики Беларусь и 10 иностранных граждан, которые не включаются в национальную регистрацию. Лабораторно подтвержденные случаи составили 50% из официально зарегистрированных. При расширенном обследовании 103 пациентов с исключенным диагнозом эпидемического паротита у 13 (12,6%) обнаружен генетический материал других вирусов: вируса Эпштейна – Барр (5,8%), цитомегаловируса (4,9%), энтеровируса (1,9%). Из 5 генотипированных вирусов паротита 4 принадлежали к разным генетическим вариантам генотипа G и 1 – к генотипу F, что свидетельствует об их завозном характере. Проведенное исследование подтверждает спорадический характер заболеваемости эпидемическим паротитом в Республике Беларусь благодаря успешной вакцинопрофилактике и отсутствие эндемичной циркуляции вируса паротита. The purpose of the study was to analyze the incidence of epidemic parotiditis in the Republic of Belarus for the period 2013-2022 based on the results of laboratory examination of patients and phylogenetic study of the pathogen. Over a ten-year period, 38 cases of epidemic parotiditis were officially registered in the country, the incidence was less than 1 case per 1 million population. During this time, 1013 patients with suspected epidemic parotiditis were examined using serological and molecular genetic methods. The diagnosis was confirmed in 29 (2.7%) of them, including 19 residents of the Republic of Belarus and 10 foreign citizens who are not included in the national registration. Laboratory-confirmed cases accounted for 50% of the officially registered cases. In an extended examination of 103 patients with an excluded diagnosis of epidemic parotiditis, 13 (12.6%) revealed genetic material of other viruses: Epstein–Barr virus (5.8%), cytomegalovirus (4.9%), enterovirus (1.9%). Of the 5 genotyped parotiditis viruses, 4 belonged to different genetic variants of genotype G and 1 to genotype F, which indicates their imported nature. The study confirms the sporadic nature of the incidence of epidemic parotiditis in the Republic of Belarus due to successful vaccination and the absence of endemic circulation of the parotiditis virus.
