Харківська медична академія післядипломної освіти, Харків, УкраїнаРезюме. Порушення системи гемостазу відіграють одну з ключових ролей в механізмі патогенезу коронавірусної хвороби. Поширеність тромбоемболічних ускладнень у хворих з важким перебігом COVID-19 досягає 20-30%. Ключові керівництва містять рекомендації щодо необхідності використання профілактичних доз еноксапарину. Проте результати інтенсивної терапії залишаються незадовільними і продовжується пошук оптимального режиму дозування еноксапарину. Мета дослідження -порівняти вплив різних режимів використання еноксипарину на результати інтенсивної терапії у хворих з гострим респіраторним дистрес-синдромом (ГРДС), викликаним SARS-nCoV. Матеріали і методи. Було проведено одноцентрове проспективне порівняльне дослідження, до якого за період травень-жовтень 2020 р. включено 91 пацієнта з тяжким перебігом коронавірусної хвороби з розвитком двобічної пневмонії та ГРДС. Залежно від режиму дозування еноксапарину, хворих було розподілено на три групи. Пацієнти групи 1 (n=14) отримували 4000 анти-Ха МО еноксапарину на добу, групи 2 (n=20) -8000-1200 анти-Ха МО, групи 3 (n=57) -1600-2000 анти-Ха МО /добу. Статистичний аналіз результатів проведено за допомогою програми «Statistica 10». Оцінювали медіану, мінімальні і максимальні значення, стандартне відхилення. Достовірність відмінностей показників оцінювали з використанням непараметричного критерію Вілкоксона. Результати вважались достовірними при значеннях р < 0,05. Дані представлені у вигляді М ± SD [min-max]. Проводилась оцінка відносного ризику (RR) і відношення шансів (OR) розвитку несприятливих подій (тромбоемболічних ускладнень і летальності).Результати. В групі 1 було зафіксовано 9 епізодів тромбоемболій легеневої артерії, що відповідає 64,3 %, в групі 2 -7 епізодів (35,0 %), а в групі 3 -13 (22,4 %), (р13=0,04).Загальний рівень летальності склав 69,23% і був достовірно нижчим в групі 3 -56,9 %, ніж в групі 1 -92,9 %, і в групі 2 -85,0%.Висновки. Використання лікувальних доз еноксапарину є одним з ключових напрямків терапії і позитивно впливає на виходи у хворих з коронавірусною хворобою, зменшуючи частоту розвитку тромбоемболічних ускладнень.Ключові слова: коронавірусна хвороба, дихальна недостатність, гострий респіраторний дистресс синдром, тромбоемболія легеневої артерії, тромбопрофілактика, низькомолекулярні гепарини.
