С целью оценки диагностического потенциала фактора дифференцировки роста-15 (GDF-15) как прогностического биомаркера рецидива фибрилляции предсердий (ФП) после электрической кардиоверсии обследовано 46 пациентов в возрасте 57,2±6,4 года, страдающих персистирующей формой ФП: 35 (76,1%) мужчин и 11 (23,9%) женщин. С учетом наличия синдрома обструктивного апноэ во сне (СОАС) были сформированы 2 клинические группы: группа F -11 (23,91%) пациентов без СОАС; группа FA -35 (76,09%) пациентов с СОАС. Исходно у всех пациентов определяли уровень GDF-15 в сыворотке крови методом иммуноферментного анализа. Всем обследуемым была выполнена плановая эффективная электрическая кардиоверсия. Период наблюдения составил 12 (11-13) месяцев, по истечении которого оценивалась конечная точка в виде наличия рецидива ФП. За период наблюдения у 29 (63,04%) пациентов выборки развился рецидив ФП, 17 (36,96%) пациентам сохранить синусовый ритм удалось. При наличии СОАС у пациентов с ФП уровень GDF-15 выше, чем у пациентов без данного синдрома (1014,27 (885,0; 1302,0) пг/мл и 661,07 (625,71; 835,02) пг/мл соответственно; р=0,001), и изменения содержания биомаркера ассоциированы со сроком наступления рецидива (R=-0,71; p<0,0001), с индексом апноэ/гипопноэ (R=0,47; p=0,001), с индексом массы тела (R=0,45; p=0,002), с фракцией выброса левого желудочка (R=-0,29; p=0,04). По результатам многофакторного регрессионного анализа Кокса установлено, что независимыми предикторами рецидива ФП являются уровень GDF-15 (ОШ 1,004; 95% ДИ 1,002-1,006; p<0,0001) и размер левого предсердия (ОШ 1,108 (95% ДИ 1,007-1,218; р=0,03). Методом ROC-анализа было определено пороговое значение уровня GDF-15 более 967,78 пг/мл (р<0,001), позволяющее с чувствительностью 75,9% и специфичностью 88,2% предсказать развитие рецидива ФП в течение года после электрической кардиоверсии у пациентов с ФП в сочетании с СОАС.
