Purpose.To evaluate the role of ozonized erythrocytic mass transfusion in the restoration of damages in the architectonics of myocardium microvasculature and cardiomyocytes in case of a severe blood loss.Materials and Methods. Two batches of experiments were conducted, 17 white outbred rats in each. The animals were anesthetized with thiopental sodium (25 mg/kg). Blood loss was caused by taking 3ml of blood from the tail artery of rats, which is 35% of the circulating blood volume. One hour after the blood loss, transfusion of autoerythrocytes with normal saline solution and Ringer’s solution infusion in 1:1 ratio was performed in the control batch. In the experimental batch, 3 ml of autoerythrocytes treated with ozonized saline solution with ozone concentration of 2 mg/l and 3 ml of Ringer’s solution (1:1 ratio) were administered to rats. Erythrocyte mass was prepared from 3 ml of the autoblood harvested from the animals 3 days before the experiment. On a post-transfusion day5, in both batches and in five intact animals, hearts were removed following the intraperitoneal thiopental sodium injection (100 mg/kg). The left ventricle wall specimen from each heart was removed for examination. Histologic sections were stained with hematoxylin and eosin. The preparations were visually examined and morphometric studies were performed using microvisor Vizo-103.Results.It has been established that administration of ozonized erythrocyte for severe blood loss correction limited the decrease in numbers of capillary profiles and their diameters, formation of mixed and hyaline thrombi fully or partially occluding microvascular lumens, major hemorrhages and reduction of variation of nuclear profiles, decreased perivascular, pericellular, perinuclear, and endonuclear edema of myocardial tissue, cardiomyocyte overcontruction zone, and their ruptures.Conclusion.The positive trends for microcirculation indices, vascularization density and myocardial edematization as prognostic markers in assessing potential posthypoxic rehabilitation of damaged tissues upon blood loss correction with ozonized erythrocyte mass might be explained by the antihypoxic, antioxidant, and detoxifying actions of ozone on erythrocytes and/or its metabolites (ozonides) in the body post-transfusion.

Во многих случаях летальные исходы предопределяет развитие полиорганной недостаточ ности, обусловленной развитием необратимых мор фофункциональных изменений в жизненно важных органах [1][2][3][4].Возникающие при геморрагическом шоке мор фологические изменения связаны с неадекватной ми кроциркуляцией и нарушениями метаболизма во вре мя гиповолемии и последующей за инфузионной терапией реперфузией внутренних органов, временно выключенных из системы кровообращения централи зацией кровообращения [5, 6].Некупированные нарушения неизбежно про грессируют и последовательно приводят к клиничес ким проявлениям полиорганной недостаточности, по этому инфузионная терапия с использованием универсальных препаратов, направленных на поддер Objective: to perform an experimental study of the effect of isotonic sterofundin on the morphology of the myocardium, kid ney, lung, pancreas, liver, and brain in the late period after hemorrhagic shock. Material and methods. Hemorrhagic shock was induced in 36 male Wistar rats weighing 230-250 g by acute massive blood loss (AMBL) of 2.5 ml/100 g at a rate of 2 ml/min. An hour after AMBL, hypovolemia was replenished to 200% volume for 60 min with Ringer solution and isotonic sterofundin in control and experimental groups, respectively. Then the blood was reinfused at 70% volume of its loss. The magnitude of morphological changes was estimated 3 days following AMBL. Specimens harvested from organs of euthanized animals were examined histologically and images were recorded using a μVizo 103 microvisor. Results. The experimental studies demon strate significant structural damage of tissues within target organs following traditional infusion therapy with polyionic crys talloid solutions that may cause development of multiple organ dysfunction. Malate containing solution of sterofundin reduces the degree of organ structural damages that may prevent multiple organ dysfunction in the posthemorrhagic period.
В экспериментальном исследовании проведен анализ действия цитофлавина (0,2 мл/кг, вводимого внутрибрюшинно в течение 10 сут) на показатели свертывающей системы крови, гематологические показатели крови, а также электрокинетические и агрегационные свойства эритроцитов у крыс после черепно-мозговой травмы. Показано, что на 3 сут после черепно-мозговой травмы развивалась гиперкоагуляция, умеренная гипофибриногенемия, уменьшение количества тромбоцитов и эритроцитов в среднем на 30 % (p = 0,001) и содержания гемоглобина на 29 % (p = 0,001), увеличение среднего объема эритроцитов на 10 % (p = 0,019), повышенная их агрегация. Использование цитофлавина с 1 сут после травмы способствовало увеличению количества эритроцитов на 17 % (p = 0,048), гемоглобинана 7 % (p = 0,020), повышало электроотрицательность мембран эритроцитов и снижало их агрегационные показатели, что сочеталось с увеличением количества тромбоцитов на 7 % (p = 0,024), уменьшением коагуляционного гемостаза и стабилизацией фибринолитической активности крови крыс.
У крыс, перенесших черепно-мозговую травму (ЧМТ), изучали динамику электрофоретической подвижности эритроцитов (ЭФПЭ), агрегацию и концентрацию АТФ и 2,3-дифосфоглицериновой кислоты (2,3-ДФГ) в эритроцитах, состояние микроциркуляции головного мозга и двигательную активность крыс при действии цитофлавина (0,2 мл/кг в сутки в течение 7 сут). Показано, что цитофлафин увеличивал ЭФПЭ к 3 сут от начала применения, к 7 сут регистрировали снижение степени агрегации эритроцитов, увеличение концентрации АТФ и 2,3-ДФГ в эритроцитах крыс с ЧМТ. Изменение функциональных показателей эритроцитов под влиянием цитофлавина сочеталось с улучшением состояния микроциркуляции головного мозга и восстановлением двигательной активности животных. В контрольной группе крыс частичное восстановление исследуемых параметров наблюдалось к 12 сут посттравматического периода. Выявленные эффекты цитофлавина позволяют расширить возможности применения препарата в качестве средства коррекции функциональных показателей эритроцитов и микроциркуляции головного мозга при черепно-мозговой травме.
Цель исследования -изучить влияние натрия оксибутирата на микроциркуляцию в брыжейке тонкой кишки и метабо лизм печени собак при геморрагическом шоке. Материал и методы. Работа основана на изучении материала от 72 со бак обоего пола, массой 15,5±1,5 кг. Гиповолемическую гипотензию вызывали свободным кровопусканием из бедрен ной артерии. Систолическое артериальное давление снижали до 40 мм рт. ст. и поддерживали на этом уровне в течение 1 часа по методике Wiggers. Величина кровопотери составила 31-33 мл/кг. Собаки были разделены на 4 серии: 1 я се рия -интактные (5), 2 я серия -гиповолемическая гипотензия в течение 1 часа (8), 3 я контрольная серия (31) -че рез 1 час гиповолемической гипотензии внутривенно вводили физиологический раствор из расчета 0,9-1,1 мл/кг, 4 я опытная серия (28) -через 1 час гиповолемической гипотензии внутривенно вводили 10% раствор натрия оксибутира та из расчета 180-200 мг/кг. У собак 1 й и 2 й серии, а также в контрольной и опытной сериях, разделенных на две под группы -спустя час после введения препарата и 1 час после реинфузии крови; под местной анестезией 0,25% раство ром новокаина в сочетании с внутривенным введением тиопентала натрия (15-20 мг/кг) производили лапаротомию, иссекали кусочки правой доли печени для гистохимического и биохимического исследования. В 3 й и 4 й сериях, спу стя час после введения препарата, производили реинфузию гепаринизированой крови. Животных наблюдали в течение 1 часа. У собак последних двух серий на этапах контрольного времени оценивали состояние микроциркуляции в сосу дах брыжейки тонкой кишки методом биомикроскопии на установке, смонтированной на основе микроскопа МБР 1. Результаты. Применение натрия оксибутирата при геморрагическом шоке, несмотря на невосполненную кровопоте рю, улучшает микроциркуляцию в сосудах брыжейки тонкой кишки. В печени -повышает интенсивность реакций окислительного фосфорилирования и пентозофосфатного пути, активизирует процессы использования глюкозы, пре дупреждает накопление лактата, сохраняет запасы гликогена и повышает содержание высокоэнергетических фосфа тов. Заключение. Введение 10% раствора оксибутирата натрия из расчета 180-200 мг/кг через 1 час гиповолемической гипотензии предотвращает нарастание нарушений микроциркуляции в брыжейке тонкой кишки и метаболических из менений в печени в течение последующего часа геморрагического шока. Восполнение кровопотери на фоне предшест вующего введения натрия оксибутирата способствует более полному и быстрому восстановлению микроциркуляции в брыжейке тонкой кишки и метаболических процессов в печени в раннем постреанимационном периоде.
