Прогресс в ревматологии, прежде всего, связан с внедрением генно-инженерных биологических препаратов (ГИБП), которые демонстрируют преимущества перед стандартными методами лечения в отношении как эффективности, так и безопасности. При артериите Такаясу вопросы применения ГИБП не разработаны. Среди перспективных направлений лечения рефрактерного артериита Такаясу рассматривают ингибирование интерлейкина (ИЛ)-6. Представлен собственный опыт эффективного применения тоцилизумаба (гуманизированные моноклональные антитела к ИЛ-6 рецептору) у пациентки 27 лет с артериитом Такаясу, резистентным к лечению глюкокортикоидами и цитостатиками (циклофосфан, метотрексат), неполным эффектом альтернативной терапии флударабином, непереносимостью циклофосфана и осложнениями лечения глюкокортикоидами. В результате лечения тоцилизумабом (600 мг внутривенно каждые 4 недели в течение 24-х месяцев) достигнута стойкая ремиссия артериита Такаясу, подтвержденная результатами позитронной эмиссионной томографии, и минимизирована доза глюкокортикоидов. Применение ГИБП, действие которых направлено на ингибирование ИЛ-6, можно рассматривать как перспективную инновационную стратегию лечения артериита Такаясу, в первую очередь при неэффективности стандартной терапии или ее плохой переносимости. В последние десятилетия в мире отмечается быстрый рост заболеваемости подагрой. Несмотря на хорошо изученные механизмы развития и особенности клинической картины заболевания, подагра диагностируется поздно. Традиционно считается, что подагра встречается у 1-3 % населения развитых стран, преимущественно у мужчин старше 45 лет. Однако в современной литературе имеются сведения о снижении возраста дебюта подагры и учащении случаев ее развития у женщин. Постоянно увеличивающаяся заболеваемость, ранняя инвалидизация, ухудшение качества жизни и высокий риск смерти обосновывают актуальность проблемы подагры и требуют совершенствования методов выявления, ранней диагностики и оптимизации тактики ведения больных. Цель данного исследования-выявление полового диморфизма клиники подагры (по материалам ревматологических отделений клиник г. Саранска). Материалы и методы В исследование было включено 195 больных подагрой (169 мужчин и 26 женщин), обследованных за 2011-2015 гг. в ревматологических отделениях ГБУЗ «МРКБ» и ГБУЗ «РКБ № 5» г. Саранска. Средний возраст женщин-52,3±12,3 года, мужчин-54,3±13,2 года, медиана длительности заболевания-5,2 (2,6-7,8) года у женщин и 9 (3,5-14,5) лет у мужчин. Результаты исследования Хронический артрит был выявлен у 58,8 % женщин и 67,5 % мужчин. В группе женщин продолжительность первого приступа артрита в среднем составила 13 суток, в группе мужчин-10 суток. Формирование тофусов у женщин наблюдалось раньше и в большем количестве, чем у мужчин. Среди коморбидных состояний у женщин чаще, чем у мужчин, выявлялись компоненты метаболического синдрома (МС): артериальная гипертензия (АГ), сахарный диабет (СД) 2 типа, дислипидемия. Обсуждение и заключения Клиническое течение подагры у женщин имеет свои особенности, поскольку данное заболевание протекает тяжелее: раньше, чем у мужчин, формируется хроническая тофусная подагра; чаще выявляются АГ, СД 2 типа, дислипидемия.
