One month after treatment, there was a slight decrease in uncorrected and best corrected visual acuity (UCVA and BCVA) - from 0.08±0.03 and 0.4±0.15 preoperatively down to 0.06±0.02 and 0.3±0.07, respectively, caused by pseudohaze of the cornea. Keratometric parameters (the average refractive power of the cornea, corneal astigmatism, and corneal thickness) did not change significantly. The demarcation line was identified in 56% of cases. By the 3-month follow-up, UCVA and BCVA improved up to 0.1±0.07 and 0.52±0.1, respectively. The refractive power of the cornea decreased by 2.0 diopters and corneal astigmatism - by 0.7 diopters reaching 46.8±2.7 and 5.1±1.3 diopters, respectively (p≤0.04). Central corneal thickness decreased by an average of 29 microns. The demarcation line remained visible in 25% of cases. At 6 months, BCVA averaged 0.58±0.13, at that, 56% of eyes gained 1 line and 31% - 2 lines. The refractive power of the cornea decreased down to 45.7±1.6 diopters, corneal astigmatism - down to 4.8±1.5 diopters. The demarcation line was not detected. At 1 year, there were no significant changes in the average values of optometric indices as compared to the 6-month period. Сonclusion. Pellucid marginal corneal degeneration should be regarded as a kind of primary ectasia, often bilateral and notable for its characteristic clinical picture and late onset. In progressive disease, photochemical crosslinking of the cornea contributes to the improvement of optometric parameters and stabilization of the process.
Сахарный диабет (СД)-самое распространенное из эндокринных заболеваний, поэтому является приоритетной медикосоциальной проблемой. Часто при диабете именно глазные проявления в первую очередь приводят к инвалидизации больных. В обзоре представлен анализ морфологических, экспериментальных и клинических исследований конъюнктивы и роговицы у пациентов с СД-оригинальные статьи, обзоры и монографии отечественных и зарубежных авторов преимущественно последних лет. Рассмотрены современные методы исследований конъюнктивы и роговицы при СД. Обсуждены клинико-морфологические изменения этих тканей, описаны ранние диабетические изменения их структурных слоев. Представлена важная роль изменений сосудов конъюнктивы и нервных волокон роговицы (НВР) при диабете, показана корреляция между плотностью (НВР) и эпидермальных нервных волокон.
The main factor in the pathogenesis oftraamatic and burn damage to eye tissues is the disruption of regeneration processes, which may lead to detrimental consequences such as rough cicatrization [1]. Clinical practice (in particular, ophthalmologic) constantly suffers from a lack of medicinals capable of accelerating the reparative processes in tissues. A promising direction in the development of drugs possessing these properties consists in the use of natural biostimulators, Glycosaminoglycans (GAGs) are the components of connective tissues that take an active part both in the tissue metabolism and in the stimulation of reparative processes during traumatic and bum damage [2]. Histochemical investigations showed a sharp drop in the corneal GAG content upon damage to the eye. As is known, the main GAG fractions entering into the composition of eye cornea are chondroitin sulfate (CS) and keratan sulfate (KS) necessary for recovery of the transparency of eye cornea) [3 -6].The drug keracol, developed in the Ufa Research Institute of Eye Diseases and produced by the "Immunopreparat" corporation (Ufa Department), provides clarification of the cornea following various kinds of damage including bums, ulcers, fistulas, decementocytosis, and mechanical injury. However, the keracol substance, obtained from the bovine cornea by a process involving special treatment and comminution, exhibits a number of disadvantages. The drug is insoluble in water, rapidly washed out as a result of lacrimation, and can be used for therapy only under stationary hospital conditions. All this stimulated development of a new drug, free of the above disadvantages and exceeding keracol in its therapeutic efficacy. Below we present the results of investigation of the process of GAG extraction from bovine cornea and the pharmacological properties of eye drops prepared on the basis of the new drug.
ОфтальмологияOphthalmology ахарный диабет (СД) -самое распространенное эндокринное заболевание населения экономи-чески развитых стран [1,2]. В 2014 г. в мире было зарегистрировано более 387 млн больных СД. В России на 2015 г. было зарегистрировано более 4 млн пациентов с СД, и это число постоянно увеличивается. По данным И.И. Дедова с соавт. (2016), среди взрослого населения России почти у 20% наблюдается предиабет, еще более чем 2 млн человек не подозревают о своем диагнозе [1].Нарушение углеводного обмена приводит к пато-логическим изменениям практически во всех органах и тканях, поэтому СД является одной из приоритетных медико-социальных проблем [3]. Гипергликемия -ос-новная причина, запускающая биохимические, гемоди-намические и структурные изменения в сосудах, нервах и органах. Ежегодно растет число больных с СД, нужда-ющихся в квалифицированной помощи офтальмологов. Из-за особой функциональной важности зрительного анализатора часто именно глазные проявления этого за-болевания в первую очередь приводят к инвалидизации больных [4]. Как показывает практика, специалисты многих городских и районных поликлиник оказываются неподготовленными к работе с пациентами этого про-филя.До 20% слепых потеряли зрение из-за диабетиче-ского поражения глаз. Около 70% пациентов с СД имеют осложнения со стороны роговицы -диабетическую ке-ратопатию (ДК) [5]. Учитывая тонкую архитектонику роговицы, диагностика ДК, особенно на ранних ста-диях заболевания, не всегда представляется возможной. Тем не менее нарушение обменных процессов в ор-ганизме при СД касается и этой важной оптической структуры глаза. Исследователи, описывая изменения глаз при диабете, чаще заостряют внимание на распро-страненном позднем осложнении -диабетической ре-тинопатии, упуская изменения роговицы. В литературе имеется немного данных о синдроме сухого глаза (ССГ) среди больных с СД, несмотря на то, что проблема оче-видна и актуальна, учитывая то, что количество таких пациентов ежегодно увеличивается [6,7].Диабетическая полинейропатия (ДПН) является наиболее распространенным осложнением СД, при ко-торой происходит необратимая дисфункция нервов. The article presents historical aspects and information about the treatment of degenerative diseases of the cornea using methods of photodynamic therapy, including modern technology of ultraviolet (UV) crosslinking of corneal collagen. The achievements of the scientific school of the Ufa Research Institute of Eye Diseases in the field of development and implementation of new methods for the treatment of keratectasia, medical devices and products for UV corneal crosslinking are presented. The article presents the stages of introduction of various types of keratoplasty for more than 70 years in the Ufa Eye Research Institute. Modern types of applied selective keratoplasty are described depending on the disease and the degree of stroma damage: anterior deep layer keratoplasty and endothelial keratoplasty. The main conditions for performing high-tech keratoplasty operations including modern equipment and training of highly qualified surgeons are determined.
