T. m-FISH analysis reveals complexity of chromosome aberrations in individuals occupationally exposed with internal plutonium: A pilot study to assess the relevance of complex aberrations as biomarkers of exposure to high-LET α-particles. Radiat. Res.We recently demonstrated that a significant proportion of apparently stable insertions induced after exposure to a mean of 1 α-particle/cell, detected using 3-colour FISH, were part of larger unstable complexes when visualised by 24-colour FISH. Interestingly, regardless of the long-term persistence capability of the cell, the complexity of each α-particle-induced complex appeared to be specific to the nuclear traversal of a single α-particle. To assess whether aberrations of a similar complexity are observed in vivo and also to examine the usefulness of detecting such aberrations as a biomarker of chronic exposure to α-particles, we have carried out a limited pilot study of Russian workers with large body burdens of α-particle emitting plutonium (Pu). We found unstable cells containing non-transmissible complex aberrations in all of the Pu exposed subjects analysed by m-FISH. In addition, all of the complexes seen were consistent with those previously observed in vitro. Non-transmissible complex aberrations were more common than transmissible-type complexes, consistent with on-going/chronic exposure and insertions were dominant features of both type of complex. Accordingly, this preliminary study supports the proposal that aberration complexity and non-transmissibility are the major cytogenetic features of α-particle exposure that could potentially be exploited as a specific indicator of chronic exposures to high-LET α-particles.2

Один из важнейших маркеров нарушений эпигенетической регуляции генома клеткиасинхронность репликации ДНК биаллельно экспрессирующихся генов. Удобным методом оценки нарушения синхронности репликации (асинхронной репликации) в культивированных лимфоцитах периферической крови человека, как правило, является интерфазная флуоресцентная in situ гибридизация (I-FISH). В обзоре проведен анализ работ, направленных на изучение асинхронной репликации биаллельно экспрессирующихся генов в лимфоцитах периферической крови онкологических больных. Этот анализ показал, что асинхронная репликация-неспецифический маркер злокачественных новообразований и наблюдается как у онкогематологических больных, так и у пациентов с солидными опухолями, причем не только в клетках опухоли, но и в лимфоцитах периферической крови. Проведенный анализ продемонстрировал, что встречаемость лимфоцитов с асинхронной репликацией изученных генов у онкологических больных достоверно повышена по сравнению со здоровыми лицами и увеличивается в процессе малигнизации (злокачественного перерождения) заболеваний. Это дает потенциальную возможность использовать асинхронную репликацию как молекулярно-генетический маркер для раннего выявления лиц с онкологическими заболеваниями.
The aim of the study was to identify and quantify lymphocytes with asynchronous replication of the AURKA and TP53 genes in cancer patients versus controls and to assess the diagnostic capabilities of this approach.Materials and Methods. The study was carried out with peripheral blood lymphocytes probed for the AURKA and TP53 genes using the interphase fluorescence in situ hybridization (FISH) method (Vysis, USA and Kreatech, The Netherlands). The control group included 70 people: clinically healthy donors and patients with non-oncological diseases such as gastritis, pancreatitis, chronic calculous cholecystitis, bronchial asthma, peptic ulcer disease, inguinal hernia, arthrosis, myoma, hepatitis, epilepsy, chronic prostatitis, chronic tonsillitis, and rectal adenoma. The group of cancer patients included 219 people with various oncological diseases: gastric cancer (n=68), colorectal cancer (n=30), chronic lymphocytic leukemia (n=52), Hodgkin lymphoma (n=33), and polyneoplasia (n=41).Results. In the control group, the mean frequency of lymphocytes with asynchronous gene replication (AGR) was 22.0±3.4% for AURKA and 18.0±3.2% for TP53; in the group of cancer patients, that was 36.8±4.8 and 28.4±5.1%, respectively. The excessive presence of lymphocytes with the AGR in cancer patients was consistent and statistically significant (p<0.0001). For the AURKA gene, the AGRbased cancer detection showed a sensitivity of 98.6±0.7%, a specificity of 100%, and an accuracy of 98.3±0.8%, and for the TP53 gene -78.6±3.1, 98.5±0.9, and 85.9±2.6%, respectively.Conclusion. This pilot study on lymphocytes with AGR of AURKA and TP53 genes in cancer patients can serve a basis for creating a new molecular cytogenetic technology for detecting malignant neoplasms in humans.
Background. The correct genome replication is essential for normal cell division to guarantee that genetic information comes changeless through the next cells generations. DNA replication is a strictly regulated and synchronous process and its disturbances could result to mutations appearances. Aberrant time of DNA replication affects on gene expression causes changes of epigenetic modifications and influences on increasing the structural rearrangements leading to enhanced genome disbalance. Replication time failure as asynchronous replication is common for cancerogeneses. The objective of our study was the assessment of asynchronous replication levels in patients with gastric cancer and patients with multiple tumors.Materials and methods. Fluorescence in situ hybridization (FISH) was used for the asynchronous replication of AURKA and TP53 genes analyses. Interphase FISH on lymphocytes of peripheral blood of 37 healthy donors, 19 patients with non-cancer gastrointestinal pathologies, 68 patients with solitary gastric cancer and 39 patients with multiple tumors having gastric cancer and other second synchronous or metachronous tumor was carried out.Results. Values of lymphocytes with asynchronous replication for AURKA were 19.8 ± 0.5 % for control group, 24.7 ± 0.4 % for non-cancer patients, 32.5 ± 0.5 % for gastric cancer patients, 39.5 ± 0.6 % for patients with multiple tumors and 17.3 ± 0.5, 19.5 ± 0.7, 26.1 ± 0.7 and 32.5 ± 0.6 % for TP53 respectively. Differences between cell populations of examined groups had statistical significance with p <0.01 for both studied gene. Also there was statistical difference between gastric cancer patients having distant metastases and gastric cancer patients without metastases for AURKA (34.4 ± 1.0 % vs. 31.7 ± 0.6 %; p = 0.02).Conclusion. High lymphocytes with asynchronous replication level in oncological patients could serve as potential marker of second tumor or possible metastatic process including the earliest stage of it.
Актуальность. К настоящему времени известно, что в патогенезе хронического лимфоцитарного лейкоза (ХЛЛ) важную роль играют генетические нарушения. Их выявление может быть использовано для оценки прогноза заболевания, что является актуальной задачей, поскольку по клиническому течению ХЛЛ крайне разнороден. Цель исследования. Изучить молекулярно-цитогенетические нарушения в локусе IGH у больных ХЛЛ и их сочетание с нарушениями, выявленными при использовании стандартной для ХЛЛ цитогенетической панели. Материалы и методы. Исследование выполнено методом интерфазной in situ гибридизации (I-FISH) на архивных образцах нестимулированных лимфоцитов 89 больных с впервые выявленным ХЛЛ и 15 клинически здоровых доноров в качестве контрольной группы. Результаты. Нарушения в локусе тяжелых цепей иммуноглобулинов обнаружены у 64% больных. IGH транслокации выявлены у 19% больных, частичная/полная потеря одной копии IGHV региона - у 58% больных ХЛЛ. Мы разделили больных на 3 группы и оценили встречаемость в них нарушений в локусе 14q32. В группу неблагоприятного прогноза вошли 35 человек (16 больных с del17p13 и 19 больных с del11q22), в группу промежуточного прогноза вошли 18 человек (10 больных с трисомией хромосомы 12 и 8 больных без выявленных цитогенетических нарушений, и в группу благоприятного прогноза - 36 больных с del13q14 как единственной. Статистически значимых различий между группами прогноза по встречаемости больных с IGH транслокациями не выявлено. Установлено, что у больных с del17p13 и с del11q22 IGHV делеции встречались достоверно чаще, чем у больных с del13q14 как единственной (74% vs 44%, p = 0,011). В группах неблагоприятного и промежуточного прогноза по сравнению с группой благоприятного прогноза почти в 4 раза (40% и 39% vs 11% соответственно) больше больных с высокой (50% и выше) долей клеток с IGHV делециями (p < 0,05). Заключение. Нарушения в IGH локусе занимают второе место по встречаемости среди нарушений, выявляемых методом FISH при ХЛЛ. Эти нарушения могут быть значимыми, поскольку они достоверно чаще встречаются у больных в группе неблагоприятного прогноза. Дальнейшее наблюдение за больными c нарушениями в IGH локусе позволит оценить их клинико-биологические особенности и прогностическую значимость при ХЛЛ. Возможно, гетерогенность течения заболевания внутри группы больных с благоприятным прогнозом, выделенной на основании иерархической модели, обусловлена наличием дополнительных цитогенетических нарушений, в том числе и нарушений в IGH локусе. Background. Genetic disorders play an important role in the pathogenesis of chronic lymphocytic leukemia (CLL). Identification of these disorders can be used for prognosis of the disease. This is an urgent task since the clinical course of CLL is extremely heterogeneous. The aim of this study was to explore cytogenetic aberrations of the immunoglobulin heavy chain (IGH) locus and their association with a common FISH panel in patients with chronic lymphocytic leukemia (CLL). Materials and methods. The study was performed by a retrospective analysis of samples of non-stimulated lymphocytes from 89 patients with newly diagnosed CLL and 15 clinically healthy donors as a control group using interphase fluorescence in situ hybridization (I-FISH). Results. Rearrangements of the IGH locus were found in 64% of CLL patients. IGH translocations were observed in 19% of patients whereas 58% of patients had total or partial IGHV deletions. The patients were divided into three groups and evaluated for occurrence of aberrations in the locus 14q32. The group of unfavorable prognosis included 35 patients (16 patients with del17p13 and 19 patients with del11q22); the group of intermediate prognosis included 18 patients (10 patients with chromosome 12 trisomy and 8 patients without identified aberrations); and the group of favorable prognosis consisted of 36 patients with del13q14 as the sole abnormality. There were no statistically significant differences between groups with different prognosis in the occurrence of patients with IGH translocations. IGHV deletions were significantly more frequent in patients with del17p13 and del11q22 than in patients with del13q14 as the sole abnormality (74% vs. 44%, p=0.011). Patients with a high (50% and more) proportion of cells with IGHV deletions occurred approximately 4 times more frequently in groups of unfavorable and intermediate prognosis compared to the group of favorable prognosis (40% and 39% vs. 11%, respectively, p<0.05). Conclusion. Rearrangements of the IGH locus detected using the FISH method is the second among all aberrations in CLL. These abnormalities may be important as they are significantly more frequently observed in patients with an unfavorable prognosis. Further observation of CLL patients with IGH aberrations will allow to identify clinical and biological features and prognostic importance of these aberrations. Probably, the intergroup heterogeneity of the clinical course in patients with a favorable prognosis determined on a hierarchical model is due to additional cytogenetic aberrations including IGH locus abnormalities.
Актуальность. По современным представлениям, в патогенезе онкологических заболеваний ведущую роль играют молекулярно-генетические нарушения. Их встречаемость и клиническая значимость при раке слизистой оболочки полости рта (СОПР) недостаточно изучена. Целью исследования было изучение в клетках слущенного эпителия СОПР молекулярно-цитогенетического профиля нарушений хромосом 7, 11 и генов EGFR, CCND1 у больных раком СОПР. Методы. Использован метод флуоресцентной in situ гибридизации (FISH) с применением ДНК-зондов ген/центромера хромосомы: EGFR/CEP7 и CCND1/CEP11. Исследование проводили на мазках с опухоли различной локализации, взятых у 38 больных раком СОПР с различными стадиями заболевания до начала лечения. Контролем служили мазки СОПР 12 клинически здоровых лиц. Результаты. В клетках слущенного эпителия СОПР у здоровых лиц и больных раком наблюдаются клетки с нарушением копийности генов EGFR и/или CCND1 (потеря копии и амплификация), моносомией хромосом 7 и/или 11, а также с их полисомией, представленной в основном трисомией и тетрасомией. У больных выявлены клетки с высокой степенью амплификации исследованных генов и клетки с полисомией, содержащие 5 и более центромер, которые не встречались у здоровых лиц. В группе больных среднегрупповые показатели нарушений статистически значимо (р < 0,01) превышали контрольный уровень для частоты клеток с полисомией хромосомы 7 (26,95% vs 0,67%) и хромосомы 11 (21,15% vs 0,21%), а также амплификации генов EGFR (6,45% vs 0,13%) и CCND1 (14,27% vs 0,83%). Частота клеток с амплификацией генов EGFR и CCND1, а также полисомией хромосом 7 и 11 статистически значимо превышала контрольный уровень у 79%, 79%, 95% и 90% больных, соответственно. Заключение. Установлено, что для больных раком СОПР клинически значимыми нарушениями, выявляемыми в клетках слущенного эпителия СОПР могут быть полисомия хромосом 7, 11 и амплификация генов EGFR, CCND1. Эти нарушения являются показателями нестабильности генома и, вероятно, могут служить не только независимыми прогностическими маркерами прогрессии опухоли, рецидива и метастазов, но также иметь потенциальную прогностическую значимость для оценки индивидуальной радиочувствительности, что позволит персонифицировать методы лечения больных раком СОПР.Background. According to recent conceptions molecular genetic abnormalities play a leading role in the pathogenesis of oncologic diseases. The reoccurrence and clinical significance of these abnormalities in oral squamous cell carcinoma (OSCC) are still unclear. Aim. The aim of this study was to investigate abnormalities in the molecular cytogenetic profile of chromosomes 7, 11 and EGFR, CCND1 genes in exfoliated oral mucosa cells from OSCC patients. Methods. The study used fluorescence in situ hybridization (FISH) with a DNA probe for EGFR/CEP7 and CCND1/CEP11. The study was conducted on tumor smears collected from 38 OSCC patients with different localizations and stages of the disease before treatment. Smears of exfoliated oral mucosa cells from 12 healthy individuals were used as the control. Results. Cells from both healthy subjects and oral cancer patients showed deletions of gene EGFR and/or CCND1, their amplification, and monosomy and polysomy of chromosome 7 and/or 11 represented mainly by trisomy and tetrasomy. Cells with high-grade amplification of the studied genes and polysomic cells containing 5 or more centromeres were found in patients but not in control subjects. In the patient group, the average frequency of abnormalities was significantly higher (р < 0.01) than in control, including the frequencies of chromosome 7 (26.95% vs. 0.67%) and chromosome 11 (21.15% vs 0.21%) polysomy. The same was true for gene EGFR (6.45% vs. 0.13%) and gene CCND1 (14.27% vs. 0.83%) amplifications. The frequencies of cells with EGFR and CCND1 amplifications and chromosome 7 and 11 polysomy were significantly higher than in control for 79%, 79%, 95%, and 90% patients, respectively. Conclusion. The chromosome 7 and 11 polysomy and gene EGFR and CCND1 amplifications detected in exfoliated epithelial cells of oral mucosa may be clinically significant for patients with OSCC. These disorders are markers of genome instability and may serve as prognostic factors for tumor progression, recurrence, and metastases. Moreover, they may also be potentially predictive for individual radiosensitivity, which would help individualizing therapy for patients with OSCC.
