Aim. To investigate the antihypertensive effectiveness of a fixed perindopril/ amlodipine combination (Prestans) and its effects on the visit-to-visit variability of blood pressure levels and quality of life (QoL) among in-patients with uncontrolled arterial hypertension (AH). Material and methods. The study included 35 patients (15 men and 20 women; mean age 50,4±8,9 years) who were hospitalised due to uncontrolled AH. The previously administered ineffective antihypertensive therapy was cancelled and replaced by Prestans (5/5, 10/5, 5/10, or 10/10 mg/day, subject to AH severity). The in-hospital monitoring lasted for 14 days and included daily assessment of office, systolic and diastolic BP (sBP, dBP), mean and pulse BP, intra-visit and visitto-visit BP variability, and QoL (sF-36 questionnaire). Results. All hospitalised patients had stage III AH, including 31,4% with degree 2 AH and 68,6% with degree 3 AH. during the in-hospital course of Prestans treatment, the levels of sBP and dBP decreased from 184,2±14,6 to 142,1±13,8 mm Hg (p<0,05) and from 104,6±7,2 to 84,3±5,7 mm Hg (p<0,05), respectively. Mean and pulse BP levels also reduced significantly. The intra-visit variability of office sBP significantly decreased from day 1 (5,07±0,9 mm Hg) to day 14 (1,5±0,1 mm Hg; p<0.05). The visit-to-visit sBP variability also decreased, from 11,6±2,5 to 2,1±1,2 mm Hg, respectively (p<0,01). At baseline, the patients demonstrated a substantial reduction in both physical and mental health QoL parameters. The treatment was associated with a significant improvement in the psychological health domain (emotional and social functioning). Conclusion. In patients with uncontrolled AH, the in-hospital treatment with Prestans resulted in a mean reduction of sBP/dBP levels by 38,4/19,6 mm Hg, a significant decrease in mean and pulse BP levels, and achievement of target BP levels in 82.9% of the patients as early as at day 14 of the treatment. The amlodipine/perindopril treatment was also associated with reduced intra-visit and visit-to-visit BP variability, as well as with improved BP control, emotional status, and QoL. Цель. Оценить возможность применения новых оценочных шкал в повседнев-ном длительном лечении периндоприлом/амлодипином у больных артериаль-ной гипертонией высокого и очень высокого риска. Материал и методы. В исследование включено 30 больных АГ высокого и очень высокого риска (12 мужчин и 18 женщин) в возрасте 57,7±9,3 лет, кото-рые после стационарного лечения неконтролируемой АГ фиксированной комбинацией периндоприла/амлодипина в дозе 10/5, 5/10, 10/10 мг в течение последующих двух лет (с промежуточным контролем через 1 год) продолжили приём препарата. В течение этого периода осуществлялся контроль за дина-микой АД, межвизитной вариабельностью САД/ДАД, приверженностью к терапии, некоторыми показателями гемодинамики (по данным ЭхоКГ), липидного спектра, креатинина и глюкозы. На основании предложенных новых экспертных систем (шкалы ASCORE и шкалы для расчета "сосудистого возрас-та") практикующий врач оценивал динамику индивидуального риска сер-дечно-сосудистых осложнений и "сосудистого возраста". Результаты. Больные, наблюдавшиеся в поликлинике, имели III стадию АГ, из них 2 степень АГ диагностирована у 26,7%; 3 степень -у 73,3%. После первого года лечения периндоприлом/амлодипином на визите к врачу вели-чина САД/ДАД составляла 162,8±7,7/91,5±2,4 мм рт.ст.; приверженность к терапии -63,4%. В течение второго года больные четко придерживались рекомендаций врача с достижением целевого уровня САД/ДАД (привержен-ность -83,4%). Выявлено значимое уменьшение межвизитной изменчивости САД (с исходной 11,6±1,4 в стационаре до 4,3±0,2 мм рт.ст. после двух лет амбулаторного лечения; p<0,001). Достоверно уменьшились показатели липидного спектра и уровня глюкозы. Вероятность развития 5-летнего риска ССО снизилась с 16,1±1,1% (при госпитализации) до 9,2±2,0% (p<0,01) и 4,4±0,7% (p<0,01) через 1 и 2 года, соответственно. "Сосудистый возраст" при поступлении в стационар на 14,4 лет превышал реальный (паспортный), через 2 года лечения параметры паспортного возраста и СВ максимально сблизились (p<0,01). Заключение. В условиях реальной клинической практики апробированы новые европейские экспертные системы: шкала 5-летнего риска развития сердечно-сосудистого события и определение "сосудистого возраста" по шкале SCORE, которые продемонстрировали вероятность снижения риска ССО, подтвердили концепцию уменьшения "сосудистого возраста" под влия-нием эффективной антигипертензивной терапии. Фиксированная комбинация периндоприла/амлодипина, используемая в течение двух лет, показала клини-ческую эффективность у ранее не контролируемых больных с АГ, уменьшала межвизитную вариабельность АД, снижала индивидуальный риск развития ССО, повышала приверженность к терапии. According to a number of authors, the incidence of lymphorrhea after surgical treatment of breast cancer reaches 100%. In recent years, new methods of treating postoperative lymphorrhea have begun to appear, including the use of photodynamic therapy (PDT).Purpose of the study: to determine the possibility of using photodynamic therapy in the treatment of postoperative lymphorrhea and to evaluate its impact on the quality of life of patients.Material and methods. A prospective study involving 40 people was conducted. In 20 patients, photodynamic therapy was used in the treatment of postoperative lymphorrhea (main group), in 20 patients, punctures and pressure bandages were used (comparison group). Quality of life was assessed based on the data of the SF-36 questionnaire before the start of treatment, after 3 and 6 months. After 1 year, the condition of all patients was also assessed to identify signs of lymphostasis.Results. In the main group, in patients who underwent photodynamic therapy, lymphorrhea was completely stopped by the end of the third week. In patients from the comparison group at the same time, the volume of daily lymphorrhea was (41.4 ± 0.3) ml. A statistically significant difference was found between the indicators of the physical component of health in patients of the two groups 3 months after the start of treatment: role functioning associated with the physical component, pain intensity, general health and vitality. At the same time, the use of photodynamic therapy did not increase the risk of developing lymphostasis of the upper limb.Conclusion. The study showed that the use of photodynamic therapy in the treatment of postoperative lymphorrhea can reduce the amount of lymph loss, reduce the time of outpatient treatment, and also improve the quality of life of patients, while not increasing the risk of developing lymphostasis of the upper limb.
